January 24, 2003 Introduction Age-related macular degeneration (AMD) can cause a progressive loss of central vision amongst people affected by the disease. Without treatment, the more aggressive form, so-called wet AMD, usually results in a devastating loss of central vision. AMD is the leading cause of legal blindness in people over 50 in the western world, affecting about 500,000 patients worldwide each year. Visual impairment can dramatically reduce the patient's quality of life. People with impaired vision are less independent, e.g. if they cannot drive, they are dependent on others for travel and shopping. They can also have difficulty with performing many easy daily tasks, such as using kitchen utensils, walking around a strange area, and recognizing facial features from across a room. The impact of reduced quality of life is also demonstrated by the fact that visually impaired people need more help from their families, use more community social support services, and are more likely to be admitted to a nursing home than the general elderly population.

In this photo of macular degeneration, a large scar can be seen, which has destroyed the central portion of the macula. Age-Related Macular Degeneration (AMD) is an eye disease that is the leading cause of vision loss in our senior population. This disease impairs central vision. People who are affected by AMD have problems reading, driving and performing activities that require clear central vision. Macular degeneration causes damage to the macula of the eye. The macula is the central part of the retina at the back of the eye that allows us to see fine details clearly. Macular degeneration makes close work like threading a needle or reading a book difficult or impossible. When the macula does not function correctly, we experience blurriness or darkness in the center of our vision. Although macular degeneration reduces vision in the central part of the retina, it does not affect peripheral vision. For example, you could see a clock but not be able to tell what time it is. Macular degeneration alone does not result in total blindness. Most people continue to have some useful vision and are able to live independently. There are two stages of macular degeneration: The Dry Stage.

By Judith Lee and Gretchyn Bailey ; reviewed by Dr. Vance Thompson Although many people are not aware of it, macular degeneration, often called AMD or ARMD (for age-related macular degeneration), is the leading cause of blindness in the world. The eye-health organization Prevent Blindness America estimates that 13 million Americans have evidence of AMD. The disease breaks down the macula, the light-sensitive part of the retina responsible for the sharp, direct vision needed to read or drive. Macular degeneration is diagnosed as either dry (atrophic) or wet (exudative). The dry form is more common than the wet, with about 90% of AMD patients diagnosed with dry AMD. The wet form of the disease usually leads to more serious vision loss. Macular degeneration is more common in people over age 65, and whites and females tend to get the disease more than their counterparts. Most cases of macular degeneration are related to aging. It also can occur as a side effect of some drugs, and it appears to run in families.

The Senate Appropriations Committee, in its report accompanying the fiscal year 1997 Appropriations Bill, has requested a report from the National Eye Institute (NEI). The Committee has urged the NEI to "accelerate and encourage research in age-related macular degeneration (AMD)," and has asked NEI to provide a status report on research and prospects for the future. AMD is a common eye disease of the macula, a tiny area in the retina that helps produce sharp, central vision required for "straight ahead" activities such as reading, sewing, and driving. A person with AMD loses this clear, central vision; in some cases, vision loss is rapid and dramatic. AMD is a leading cause of severe visual impairment and blindness in the United States. It is estimated that AMD already causes visual impairment in approximately 1.7 million of the 34 million Americans over age 65. This report responds to the Committee's request by outlining areas where NEI-supported scientists have focused their research efforts in AMD. Research in this area is a high priority of the Institute; in fiscal year 1997, the NEI is funding approximately $75 million of research in areas generally related to age-related macular degeneration. Of this amount, approximately $16 million is directly related to AMD. This $16 million figure represents a 333 percent increase from the $5 million spent in AMD research in fiscal year 1989.

Washington, DC - (May 15, 2002) - New research from scientists at the University of Utah suggests that the Raman scattering technique shows promise as a screening tool for populations at risk for macular degeneration, the leading cause of blindness. The research, "Raman Imaging of Human Macular Pigments," employed the Raman spectroscopy, to image the spatial distribution of macular carotenoid pigments (MPs) in the human retina. Using excised human eyecups as test samples and resonant excitation of the pigment molecules with narrow-bandwidth blue light from a mercury arc lamp, reseachers recorded Raman images originating from the carbon-carbon double bond stretch vibrations of the molecules. This research demonstrates the potential for using this method in large-scale population studies to provide insight in individual variations in the spatial profiles of MP distributions as well as the total amounts of MP.

What is Age-Related Macular Degeneration?
Age-Related Macular Degeneration (ARMD) is a condition that affects the area of the retina called the macula and is the leading cause of vision loss in persons over age 65. The retina is the light-sensitive layer within the eye that acts like the film in a camera and allows images to be seen. The macula is the special central portion of the retina that provides the vision necessary for such tasks as reading and seeing details.     David R. Guyer, M.D. is a retina specialist and the Director of Residency Training at Manhattan Eye, Ear, and Throat Hospital as well as Clinical Associate Professor of Ophthalmology at Cornell University Medical Center. INTRODUCTION: ANATOMY OF THE RETINA The retina is a tissue in the back of the eye. It is an extremely complex structure which contains millions of cells.
    The macula, an area of approximately 1/4 inch in diameter, is the central part of the retina and is responsible for central or straight ahead vision. The term macular degeneration, therefore, implies a disease in which a degenerative or aging process affects this particular area. The macula contains a very small central area called the FOVEA . It is this part of the macula which is responsible for very sharp vision. Thus, if only a part of the macula is diseased but the fovea is intact, vision can still be good.
