    What is Meniere's Disease?
    PLEASE NOTE:
    This is not intended to replace your doctors point of view. This is solely for information only. I have tried to keep the information as accurate and up to date as much as possible. If you have any medical questions refer those to your Ear Specialist. I am not a doctor. I am a person who suffers from Menieres disease. My goal is to gather information and seek out others with the same problem. I am trying to learn and deal with this debilitating problem and would like to get response from others who suffer from this disease. Menieres is classified as a inner ear disorder that causes repeated attacks of dizziness and the discomfort of fullness from the affected ear. This is due to increase pressure of the inner ear fluids. Fluids in the inner ear chambers are constantly being produced and absorbed by the circulatory system. If there are any disturbance in the delicate relationship, this results is over production or under absorption of the fluids. This leads to increase fluid pressure (which may be felt) that usually produces dizziness which can be associated with fluctuating hearing loss and ringing in the ear. It is rare for someone to be affected in both ears at the same time.

Pilots and "Menieres Disease" - The Silent Illness By Gary Bowers

Menieres Disease is a condition not well known, yet devastating to thousands of people nationwide, including aviators. The disease is named after the French physician Prosper Meniere who, in 1861, linked a particular kind of hearing loss to a lesion of the inner ear. The definition of Menieres Disease is "idiopathic syndrome of endolymphatic hydrops." This means that Menieres Disease has no known cause (is idiopathic), is characterized by a group of signs and symptoms, and is thought to be associated with endolymphatic hydrops, an abnormal enlargement of the innermost of the two fluid-filled spaces of the inner ear. Unfortunately this enlargement cannot be seen or measured with absolute certainty during the llifetime of a human being. The symptoms include aural pressure (a feeling of occasional to constant pressure in one or both ears), tinnitus (ringing in the ears), hearing loss, and vertigo. What does this mean to a pilot? GROUNDED! It ended Alan Shepard's career as an astronaut. A person who suffers from Menieres Disease can experience some or all of the above symptoms. Some people have vertigo attacks that are so bad that they are completely disabled by the disease. The National Institutes of Health estimates that about 545,000 in the U.S. had Menieres as of 1998 and that 38,250 people are diagnosed each year. There are no known causes or cures and only limited treatment options available.

Meniere's Disease
Meniere's disease, first described in 1861 by the French physician Prosper Meniere, is characterized by multiple symptoms, all associated with excessive fluid in the inner ear involving both the hearing and balance organs. Excessive fluid in the cochlea (hearing organ) = cochlear hydrops. Excessive fluid in the semicircular canals (balance organ) results in dizziness and is vestibular hydrops. Almost all Meniere's patients have both cochlear and vestibular hydrops.
NORMAL HEARING:
Sound travels along the external ear canal, causing the eardrum to vibrate. Three small bones of the middle ear (malleus, incus, and stapes) conduct this vibration from the ear drum to the cochlea of the inner ear, causing waves of fluid in the cochlea to stimulate its delicate hearing cells (hair cells). Movement of the hair cells generates an electrical current in the auditory nerve, which in turn transmits signals to the brain. Through various interconnections these signals are recognized as sound.
SYMPTOMS:
The symptoms of Meniere's are dizzy spells (vertigo) associated with hearing loss, tinnitus, and a feeling of fullness in the ear. These spells occur to varying degrees and last from 20 minutes to a full day. About 80 percent of patients with Meniere's experience the problem in only one ear. Meniere's can occur at any age but it most frequently begins between the ages of 30 and 50. The cause of Meniere's is unknown. Possible contributors are stress, excessive salt intake, and, occasionally, endocrine problems, such as low thyroid function, abnormal glucose (sugar) metabolism, or an inability to handle fats (high cholesterol and/or triglycerides.) Uncommon disorders that mimic Meniere's are auto-immune labyrinthitis, syphilis, head injury, or viral infection of inner ear.

Abstract: Menieres disease is an inner ear disorder of unclear origin, characterized by severe attacks of simultaneously appearing fluctuant hearing loss, vertigo, tinnitus and a feeling of aural fullness. The disease has a wellknown natural course. In the early stage symptoms occur episodically, followed by complete remission, but during the course of the disease the hearing impairment progresses and becomes permanent, While the vertigo becomes less prominent with time. The sudden and unexpected attacks can cause a dramatic influence on the patient's quality of life, particularly as it is combined with increasing hearing loss and tinnitus. It is a well known clinical issue that stress might trigger the attacks, but so far the only correlation found has been a same day association between stress and attacks. AIMS OF THE PRESENT INVESTIGATION
1.To assess which of three different surgical treatments of vertigo (Endolymphatic Sac Surgery, ELS, intratympanical gentamicin injections or vestibular neurectomy) was most beneficial to patients with Menieres disease. 2. To evaluate health-related quality of life issues from a general and disease specific perspective. 3 To find if stress can trigger attacks of Menieres disease  4. To propose a policy for treatment of patients with Menieres disease

Introduction
Meniere's Disease (MD), also called endolymphatic hydrops, is a disorder of the inner ear associated with a change in fluid volume within a portion of the inner ear known as the labyrinth. When your head moves, nerve receptors in the labyrinth send signals to the brain about the body's motion.
Symptoms include hearing loss, dizziness, nausea and vomiting. Persons experience symptoms at varying frequencies, durations, and intensities. Symptoms may occur daily or as infrequently as once a year. Attacks commonly occur suddenly and may last several hours. The very first episode may last longer than 24 hours, but subsequent episodes do not.
While the disease may occur in children, it most commonly affects persons between the ages of 20 and 50. It occurs equally in men and women.
