C HILDREN W HO A RE M ENTALLY R ETARDED No. 23 (Updated 8/99) The term "mental retardation" is often misunderstood and seen as derogatory. Some think that retardation is diagnosed only on the basis of below-normal intelligence (IQ), and that persons with mental retardation are unable to learn or to care for themselves. Actually, in order to be diagnosed as a person with mental retardation, the person has to have both significantly low IQ and considerable problems in adapting to everyday life. However, most children with mental retardation can learn a great deal, and as adults can lead at least partially independent lives. Most importantly, they can enjoy their lives just as everyone else. In the past, parents were usually advised to institutionalize a child with significant mental retardation. This is not done anymore. The goal now is for the child with mental retardation to stay in the family and take part in community life. In most states, the law guarantees them educational and other services at public expense. Mental retardation may be complicated by physical and emotional problems. The child may also have difficulty with hearing, sight or speech. All these problems can lower the child's potential.

Send Internet email to: bray@cis.uab.edu Introduction The objective of this chapter is to provide the reader with an overview of mental retardation, a developmental disability with a long and sometimes controversial history. Following a brief historical overview, the current diagnostic criteria, epidemiological information and the status of dual diagnosis will be presented. Comprehensive assessment and common interventions will also be reviewed in some detail. Historical Perspective The plight of individuals with developmental disabilities has been dependent on the customs and beliefs of the era and the culture or locale. In ancient Greece and Rome, infanticide was a common practice. In Sparta, for example, neonates were examined by a state council of inspectors. If they suspected that the child was defective, the infant was thrown from a cliff to its death. By the second century A.D. individuals with disabilities, including children, who lived in the Roman Empire were frequently sold to be used for entertainment or amusement. The dawning of Christianity led to a decline in these barbaric practices and a movement toward care for the less fortunate; in fact, all of the early religious leaders, Jesus, Buddha, Mohammed, and Confucius, advocated human treatment for the mentally retarded, developmentally disabled, or infirmed (Sheerenberger, 1983).

"The softest things in the world often have to overcome the hardest things in the world" -Lao-Tzu "And the day came when the risk it took to remain tight in the bud was more painful than the risk it took to blossom" -Anais Nin Cincinnati's Optimum Residential Environments (CORE), Inc. is one of the largest agencies serving adults with mental retardation and developmental disabilities in southwestern Ohio. 75% of those served reside in family-style group homes and receive 24 hour supports. The other 25% live in their own apartments or houses with assistance provided on an individualized, as-needed basis by ENCORE. Statistics over fifteen years of service tell a story of growth and success. Since its inception in 1986, . CORE has doubled the number of full-time residents it serves and has doubled its workforce. The supported living services of CORE, called ENCORE (Extended Networks of CORE) were established in 1991. While CORE's primary activity is and always will be the provision of optimum services to those who entrust us with their dreams, a commitment to society and the community, at large, inspires us to create new commitment that we perpetuate CORE's mission.

The American Association on Mental Retardation (AAMR) is an organization of professionals working in the field of mental retardation. The association has state/provincial chapters organized into 9 geographic areas and 18 divisions for specific professional disciplines. AAMR publishes two professional journals, 'The American Journal on Mental Retardation' , devoted to research in the field, and 'Mental Retardation' , devoted to program activities. A bimonthly newsletter, , provides up-to-date information on current issues and models of service. The association's testing materials include the Adaptive Behavior Scales, a test to measure the abilities of people who are mentally retarded.

A definition for mental retardation is found in Public Law 101-476, the Individuals with Disabilities Education Act (IDEA) of 1990: Mental retardation means significantly subaverage general intellectual functioning existing concurrently with deficits in adaptive behavior and manifested during the developmental period that adversely affects a child's educational performance. [Federal Register, 57(189), September 29, 1992, p.44801] In its 1992 manual on definition and classification, the American Association on Mental Retardation (AAMR) offers the following definition: Mental retardation refers to substantial limitations in present functioning. It is characterized by significantly subaverage intellectual functioning, existing concurrently with related limitations in two or more of the following applicable adaptive skill areas: Communication, self-care, home living, social skills, community use, self-direction, health and safety, functional academics, leisure and work. Mental retardation manifests before age 18.

Mental retardation is characterized both by a significantly below-average score on a test of mental ability or intelligence and by limitations in the ability to function in areas of daily life, such as communication, self-care, and getting along in social situations and school activities. Mental retardation is sometimes referred to as a cognitive or intellectual disability. Children with mental retardation can and do learn new skills, but they develop more slowly than children with average intelligence and adaptive skills. There are different degrees of mental retardation, ranging from mild to profound. A person's level of mental retardation can be defined by their intelligence quotient (IQ), or by the types and amount of support they need. People with mental retardation may have other disabilities as well. Examples of these coexisting conditions include cerebral palsy, seizure disorders, vision impairment, hearing loss, and attention-deficit/hyperactivity disorder (ADHD). Children with severe mental retardation are more likely to have additional disabilities than are children with mild mental retardation.
