Stephen Barrett, M.D. The expression "multiple chemical sensitivity" ("MCS") is used to describe people with numerous troubling symptoms attributed to environmental factors. Many such people are seeking special accommodations, applying for disability benefits, and filing lawsuits claiming that exposure to common foods and chemicals has made them ill. Their efforts are supported by a small cadre of physicians who use questionable diagnostic and treatment methods. Critics charge that these approaches are bogus and that MCS is not a valid diagnosis.
What Is MCS?
The concepts underlying MCS were developed by allergist Theron G. Randolph, M.D. (1906-1995), who asserted that patients had become ill from exposures to substances at doses far below the levels normally considered safe. In the 1940s, he declared that allergies cause fatigue, irritability, behavior problems, depression, confusion, and nervous tension in children. In the 1950s, Randolph suggested that human failure to adapt to modern-day synthetic chemicals had resulted in a new form of sensitivity to these substances. His concern with foods then expanded to encompass a wide range of environmental chemicals. Over the ensuing years, the condition he postulated has been called allergic toxemia, cerebral allergy, chemical sensitivity, ecologic illness, environmental illness (EI), immune system dysregulation, multiple chemical sensitivity, total allergy syndrome, total environmental allergy, total immune disorder syndrome, toxic response syndrome, 20th century disease, universal allergy, and many other names that suggest a variety of causative factors. These labels are also intertwined with Gulf War syndrome, sick building syndrome, toxic carpet syndrome, and other politically controversial diagnoses.

Stephen Barrett, M.D. The U.S. Environmental Protection Agency (EPA) is considering a proposal from the National Environmental Justice Advisory Council (NEJAC) that advocates the political agenda of MCS proponents. NEJAC was chartered in 1993 to provide independent advice to the the EPA Administrator on matters related to environmental justice. Most of NEJAC's members are from community-based groups; industry and business; academic and educational institutions; state and local governments, federally recognized tribes and indigenous groups; or other nongovernmental and environmental groups.. In May 2000, NEJAC's enforcement subcommittee proposed that MCS be recognized as a disease and that people who claim to have it should be given special accommodations. NEJAC has accepted the resolution and forwarded it to EPA for review. This proposal flies in the face of scientific knowledge and can have serious legal and economic consequences. WHEREAS, people throughout the world have developed a chronic condition, Multiple Chemical Sensitivity (MCS). in response to a single massive exposure or repeated low level exposures to one or more toxic chemicals and other pollutants in the environment; and

Allergies - Multiple Chemical Sensitivity Question: Two years ago there was a chemical spill at my workplace, and many of my co-workers and I became ill immediately after the exposure. Since then, I haven't been the same. It feels like I can't think as clearly now, and I seem to be allergic to almost everything, including perfumes, cosmetics, detergents and household cleaning products. I heard about multiple chemical sensitivity (MCS) on a radio talk show, and I think I am chemically sensitive. When I asked my doctor about MCS, he didn't have much to say about it. What can you tell me about multiple chemical sensitivity? Answer: I can tell you that multiple chemical sensitivity (MCS) is something of a medical mystery. The medical community is divided over whether or not MCS actually exists. Some physicians acknowledge MCS as a medical disorder that is triggered by exposures to chemicals in the environment, often beginning with a short term, severe chemical exposure (like a chemical spill) or with a longer term, small exposures (like a poorly ventilated office building). After the initial exposure, low levels of everyday chemicals such as those found in cosmetics, soaps, and newspaper inks can trigger physical reactions in MCS patients. These patients report a range of symptoms that often include headaches, rashes, asthma, depression, muscle and joint aches, fatigue, memory loss, and confusion.

Synthetic chemicals are all around us. They're in the products we use, in the clothes we wear, in the food we eat, in the air we breathe at work. Because chemicals are everywhere in the environment, it's not possible to escape exposure. No wonder, then, that many people have become sensitized to the chemicals around them. In fact, it is estimated that 15% of the population has become sensitized to common household and commercial products. For some people, the sensitization is not too serious a problem. They may have what appears to be a minor allergy to one or more chemicals. Other people are much more seriously affected. They may feel tired all the time, and suffer from mental confusion, breathing problems, sore muscles, and a weakened immune system. Such people suffer from a condition known as Multiple Chemical Sensitivity (MCS).
What is Multiple Chemical Sensitivity?
MCS is a disorder triggered by exposures to chemicals in the environment. Individuals with MCS can have symptoms from chemical exposures at concentrations far below the levels tolerated by most people. Symptoms occur in more than one organ system in the body, such as the nervous system and the lungs. Exposure may be from the air, from food or water, or through skin contact. The symptoms may look like an allergy because they tend to go with exposures, though some people's reactions may be delayed. As MCS gets worse, reactions become more severe and increasingly chronic, often affecting more bodily functions. no single widely available medical test can explain symptoms.

Multiple chemical sensitivity is heightened (or "hyper-") sensitivity to a variety of commonly used chemical products. People with MCS experience physical reactions to products like cologne, after shave, scented hair products, dry cleaned clothing, air freshener products, latex, etc. The types of reactions include difficulty breathing, skin rashes, and migraines, among others. The types of reactions people experience and the substances to which they react differ widely. In arranging accommodations for MCS (as for any disability), it's wise to consult with the individual with MCS. Isn't saying that you have MCS just a pseudo-medical way of saying that you don't like cologne? No. In fact, up until my chemical sensitivity began to manifest itself clearly, I loved cologne and scented products, and used them regularly. It isn't that I dislike cologne it's that it dislikes me ! When I'm around it, my nose and the areas around my eyes start to swell. Later, the migraine hits. Sometimes for days.

    According to reports from campuses and recent conferences, disability support service providers and institutional administrators have seen an increase in requests for accommodations for students with a diagnosis of Multiple Chemical Sensitivity/Environmental Illness, or MCS/EI.
    Accommodating students in this disability category on college and university campuses illustrates how disability support specialists are often required to show creative sensitivity paired with sophisticated insight into social issues, public laws, and human needs.
    The Americans with Disabilities Act of 1990 (ADA) defines a person with a disability as anyone with a physical or mental impairment that substantially limits one or more major life activities. If a student's functional ability is severely limited by the symptoms of MCS/EI, he or she would be considered a person with a disability under the ADA, and campus support staff must provide reasonable accommodations so that there will be access to the program.

    Multiple Chemical Sensitivity A NEW MECHANISM OF DISEASE? Multiple chemical sensitivity (MCS) is an ailment, or a family of ailments, that has very real consequences for tens of millions of Americans. MCS has been recognized by its symptoms for 50 years because MCS sufferers in many geographical areas, researchers studying them, and doctors treating them, have reported a remarkably consistent picture of disease. However, because MCS sufferers react to chemicals at levels that are hundreds or thousands of times lower than allowable occupational exposures, traditional toxicology dictates that their symptoms cannot be caused by chemical exposures. Nor is MCS a true allergy because there are no IgE-mediated reactions involved, so allergists don't know what to make of it. In sum, because MCS does not fit any of the three currently-accepted mechanisms of disease infectious, immune system, or cancer traditional medicine has not known how to explain MCS, and so has often labeled it "psychogenic" originating in the patient's mind. This has left MCS sufferers in limbo. Told they are crazy, or imagining their disease, or making it up, they find themselves passed from physician to physician without any satisfactory answers and often without relief from their very real distress. (Some MCS sufferers DO have psychological symptoms, but that doesn't necessarily mean their disease ORIGINATES in their mind.) Forty percent of MCS sufferers report having seen more than 10 medical practitioners.
