Institute of Medicine's Immunization Safety Committee Report on Thimerosal-Containing Vaccines and Neurodevelopmental Disorders
On October 1, 2001 the Institute of Medicine’s Immunization Safety Review Committee released its report examining whether or not the use of vaccines containing the preservative thimerosal can cause neurodevelopmental disorders, specifically autism, attention deficit/hyperactivity disorder (ADHD), and speech or language delay. The Committee concluded, “The hypothesis that thimerosal exposure through the recommended childhood immunization schedule has caused neurodevelopmental disorders is not supported by clinical or experimental evidence.” However, the Committee also acknowledged that the epidemiological evidence was inadequate to either accept or reject a causal relationship. A review by the FDA in 1999 found that some children could be exposed to levels of thimerosal higher than Environmental Protection Agency guidelines if they received thimerosal-containing vaccines for each recommended childhood vaccines. At that time, the American Academy of Pediatrics and the U.S. Public Health Service urged manufacturers to eliminate or reduce the amount of mercury in vaccines. In her opening statement to the press on October 1, 2001 IOM Committee Chair, Marie C. McCormick, M.D., Sc.D., stated, “Neither at that time, nor today, does any data exist to prove that thimerosal is dangerous at the level present in vaccines.” Information reviewed by the Committee indicated that:

Institute (M.I.N.D. stands for Medical Investigation of Neurodevelopmental Disorders) has selected Randi Hagerman an internationally recognized expert in fragile X syndrome as its first director. Dr. Hagerman has more than 20 years of experience in the field of neurodevelopmental disorders. The focus of her research is fragile X syndrome the most common inherited cause of mental retardation. Her research focuses on the correlation between an individual's molecular genotype, or genetic make-up, and their physical and behavioral characteristics. She also has a strong clinical and research interest in autism and has conducted research examining the association between autism and fragile X syndrome. UC DAVIS M.I.N.D. INSTITUTE RECEIVES $30 MILLION IN STATE FUNDING (SACRAMENTO, Calif.) "This is tremendously important news for families who struggle with these disorders every single day," said Tom Anders, M.D., interim director of the M.I.N.D. Institute . "These funds will speed the development of new approaches to understanding and treating these devastating disorders, and go a long way toward helping fulfill the dream of families - to see their children live active and healthy lives." Since the M.I.N.D. Institute was launched by parents, researchers and physicians in 1998, it has funded 19 studies investigating autism, dyslexia, Tourette's syndrome, attention deficit hyperactivity disorder, Asperger syndrome and normal brain development. One of the first grants, awarded to a team from the California Birth Defects Monitoring Program, may lead to the development of the first promising test to identify some newborns who may develop autism or mental retardation. The breakthrough, announced at an American Academy of Neurology meeting in May, made the front pages of newspapers around the world. "This is only the beginning," said Anders, who is also executive associate dean of the UC Davis School of Medicine. "With this additional funding from the state, we have resources to support the creative, exciting science under way here; we can recruit additional outstanding scientists; and we can bring the resulting discoveries directly into schools and homes where it can make a real difference in people's lives."

The M.I.N.D. (Medical Investigation of Neuro-developmental Disorders) Institute at UC Davis has gorwn from the dream of 4 fathers of sons with autism and parents of children with neurodevelopmental disorders to become an outstanding, progressive research and clinical resource in Sacramento. They worked with UCD leaders to seek funding for this Institute from the State of California which has provided the University with $28 million in one time funds and $4.5 million in annual appropriations. Two new buildings on the UCDMC campus are to be opened in April of 2003 with over 100,000 square feet of space. The vision statement is that "The M.I.N.D. Institute is a collaborative international research center, committed to the awareness, understanding, prevention, care and cure of neurodevelopmental disorders." Its mission is "To find effective treatments and cures for autism and other neurodevelopmental disorders" and a more immediate goal is to "grow" a major research center that will support this mission. Current priorities include: Develop the necessary professional expertise/ capacity
The M.I.N.D. Institute currently has 76 employees, including nine physician or Ph.D. level clinical specialists, eight Ph.D. level researchers and 59 professional and support staff, and an operating budget of $4.5 million. During the last academic year, the M.I.N.D. Institute recruited several new faculty members including Psychiatry faculty members, Sally Rogers, Ph.D., and Sally Ozonoff, Ph.D., both internationally respected researchers and clinicians in autism spectrum disorders. November 2001 In 1999, the FDA called for removal of the preservative thimerosal from Haemophilus influenzae type b (Hib), diphtheria-tetanus-acellular pertussis (DTaP) and hepatitis B (HepB) vaccines for children to reduce childhood exposure to mercury. Since children who adhered to the complete immunization schedule may have been exposed to levels of mercury above some federal breakpoints, an IOM committee was convened to examine the causal link and biological plausibility of neurodevelopmental disorders as a result of thimerosal exposure. The FDA decision in 1999, however, was not an admission that thimerosal is dangerous. Rather, it was intended to reduce overall mercury exposure in children.

No proven link
The IOM review looked at available epidemiological, biological and clinical evidence relating to thimerosal and its active ingredient, ethylmercury. The panel also looked at analogous evidence from studies on methylmercury, a close chemical relative of ethylmercury found largely in foods such as fish. There was only one study directly examining the link between thimerosal and neurodevelopmental disorders. A review of unpublished data from the Vaccine Safety Datalink revealed a small but significant risk for speech delay, language disorders and attention-deficit/hyperactivity disorder (ADHD). However, the report found no clear evidence that thimerosal in trace amounts can cause neurodevelopmental disorders including autism.

Of all the fatty acids found in foods, Omega 6, Linoleic acid (LA) and Omega 3, Alpha Linolenic Acid (ALA) cannot be manufactured by the body. These are called Essential Fatty Acids (EFA) since they must be present in the diet. The liver converts ALA into the long chain Omega 3 fatty acid, Eicosapentaenoic acid (EPA) and Docosahexaenoic acid (DHA). Omega 6 essential fatty acids undergo a similar conversion. The balance of these two EFAs in our diet and cells is essential for the maintenance of optimum cell wall (membrane) structures. These long chain EFAs are necessary for laying out optimum myelin sheath on brain neurons, and they play a major role in cell membranes of the brain and the retina, as well as lowering blood plasma triglycerides and cholesterol levels. They also play an essential role in all cells, as they are an integral part of their protective membrane . Consequently deficits in these essential nutrients can lead to cellular malnutrition and "leaky" cell membranes that may allow antigens into the cell body. Multi-system dysfunction in children with ADHD and Autism The World health Organisation (WHO), the National Institute of Health (NIH) in the US and here in Australia the National Health and Medical Research Council (NHMRC) have all identified that our modern diet lacks Omega 3 EFAs compared to the diet of our genetic ancestors in the Paleolithic period. Instead of a ratio of 1:1 of O3:O6, current dietary estimates are that we consume these EFAs in a ratio of 1:20 to 1:40. In addition these authorities have identified that many of us are depleted of certain essential micronutrients probably due to modern inorganic agricultural methods. We now consume a more limited range of fresh fruit and vegetables, and these are lacking in essential micronutrients.

While very few vaccines given to children in the United States today still contain thimerosal, prudence dictates that precautionary measures be taken to decrease thimerosal exposure even further. Thimerosal is used in some vaccines and other pharmaceutical products to prevent bacterial contamination. Vaccines against measles, mumps, and rubella; varicella; and polio have never contained the preservative. However, until recently, several other vaccines on the recommended childhood immunization schedule in the United States did. They are now manufactured without thimerosal, but an unknown, probably small number of vaccine doses for hepatitis B; diphtheria, tetanus, and pertussis; and haemophilus influenzae type B, a form of bacterial meningitis, are still on clinic shelves. These supplies should not be used when alternatives are available, said the committee that wrote the report. "Most children in the United States being immunized today and in the future are unlikely to receive a vaccine that contains thimerosal," said committee chair Marie McCormick, professor of maternal and child health at Harvard School of Public Health, Boston. "In those few cases where only supplies containing the preservative are available, the vaccines should be administered rather than foregoing immunization. While the health effects of thimerosal are uncertain, we know for sure that these vaccines protect against real, proven threats to unvaccinated infants, children, and pregnant women."

    Fragile X syndrome is a common hereditary cause of mental retardation and learning disability. Neurodevelopmental disorders encompass a diverse group of individuals with a variety of specific disorders such as attention deficit disorders (ADHD), developmental language disorders (DLD), learning disabilities (LD), epilepsy, pervasive developmental disorders/autistic spectrum disorders (PDD/ASD), Tourette syndrome, and other less specific conditions such as mental retardation, cerebral palsy, and behavioral problems. The prevalence of some of these disorders is presented below: Disorder Prevalence Attention Deficit Disorder 3-5% of school age children Developmental Language Disorders 3-5% of school age children Learning Disabilities 5% of school age children Epilepsy 1% of individuals Pervasive Developmental Disorders 0.1% of individuals Tourette Syndrome 0.5 per 1000 individuals
