NEUROLEPTIC MALIGNANT SYNDROME (NMS) What is NMS? NMS is characterized by development of muscle rigidity and fever after taking an antipsychotic medication. There can also be changes in mental alertness, difficulty with drooling or swallowing, problems with blood pressure and sweating. What causes NMS? It is a rare but serious side effect of a drug from a category called neuroleptics or antipsychotics. These are typically used to treat psychotic disorders like schizophrenia. When does NMS occur? Typically, symptoms appear in the first 2 weeks after starting a neuroleptic or increasing the dosage of a neuroleptic. It may develop over hours or a few days. It also may occur at any time while on the medicine. Who is most likely to develop NMS?

Appendix Introduction - this guidemap has been designed to help an ED physician solve the diagnostic dilemma of "fever + rigidity + altered LOC" in an ED patient - this guidemap is mainly focused on helping the physician differentiate between neuroleptic malignant syndrome (NMS) and serotonin syndrome (SS) , with a lesser emphasis on other common causes of "fever + rigidity + altered LOC" History of the present illness - NMS usually develops over 24 - 72 hours , although it may progress more slowly over many days (3 - 10 days) in a patient taking a neuroleptic medication (* SS develops more abruptly within hours of a change in the dose of a provoking drug, or following the addition of an interacting drug eg. IM meperidine administered to a patient taking a MAOI) - NMS typically begins with mental status changes and muscular rigidity => hyperthermia and autonomic dysfunction - the mental status in NMS is always impaired and ranges from agitation => stupor => coma (* SS is usually associated with anxiety, restlessness , agitation or hypomania; and rarely with seizures or stupor/coma - the increased anxiety may be falsely misinterpreted as an exacerbation of the underlying condition requiring treatment with a SSRI)

  • Risk Factors
    Males, high environmental temperatures, high potency antipsychotics, initiation or recent increase in dose of antipsychotics, concurrent lithium treatment, dehydration, preexisting medical/neurologic illness, affective illness.

  • Signs and Symptoms:
    Fever, rigidity, diaphoresis, dysphagia, labile blood pressure, increased pulse rate, altered mental state (delirium), increased white blood count, increased CPK, SGOT and LDH.
    Mortality reported from 5 to 20 %.
    Rhabdomyolysis results in Acute Renal Failure
    Thrombosis can lead to Pulmonary Embolism
    Onset is relatively sudden hours to days, which helps distinguish NMS from Lethal Catatonia.
  • Treatment:
    • Obtain medical consultation due to high mortality.
    • Discontinue the antipsychotic immediately.

    "The symptoms of the serotonin syndrome are: euphoria, drowsiness, sustained rapid eye movement, overreaction of the reflexes, rapid muscle contraction and relaxation in the ankle causing abnormal movements of the foot, clumsiness, restlessness, feeling drunk and dizzy, muscle contraction and relaxation in the jaw, sweating, intoxication, muscle twitching, rigidity, high body temperature, mental status changes were frequent (including confusion and hypomania - a "happy drunk" state), shivering, diarrhea, loss of consciousness and death. Clozapine is now known to produce neuroleptic malignant syndrome. To date 14 cases have been reported. Can clozapine be used as treatment in cases of NMS? Clozapine has been used in patients with previous history of NMS (Weller, 1992, 8 cases) and also been successfully re-challenged after 6 months in a case of NMS due to clozapine (Chatterton, 1996), however caution is advised by these authors.
