Learning Objectives: To present an overview of inherited disorders of ocular motility. To discuss the role molecular genetics can have in enhancing our understanding of these disorders. GENETICS OF CONCOMITANT STRABISMUS
GENETICS OF INCOMITANT STRABISMUS

Incomitant strabismus occurs when the misalignment or the angle of deviation of the eyes varies with gaze direction. This results from mechanical dysfunction in the orbit or neuromuscular dysfunction at the level of the muscle, nerve, or brainstem. Restricted neuromuscular disorders in this group include Duane syndrome, CFEOM, strabismus fixus, Möbius syndrome, and isolated cranial nerve palsies. Of these, the neuropathology of Duane and CFEOM have been described and have interesting parallels to one another. In addition, CFEOM, strabismus fixus, and some rare forms of Duane syndrome show clear patterns of inheritance, making them accessible to molecular genetic studies.
1. "CLASSIC" CFEOM
CFEOM has been a major focus of our laboratory investigations. Individuals with CFEOM have ptosis, infraductive strabismus, and severely restricted eye movements. The ophthalmoplegia is virtually complete with the eyes fixed at 20 to 40 degrees below the horizontal. Residual eye movements are typically in the horizontal plane and are often notable for aberrant and jerky convergent or divergent movements on attempted vertical gaze as well as retraction of the globe into the orbit. Affected individuals lack binocular vision, and frequently have significant refractive errors and amblyopia. We have noted hypotonia and mild gross motor delay in young affected children, and mild facial weakness in all affected individuals.

Duane Retraction Syndrome
A syndrome characterized by marked limitation of abduction of the eye, variable limitation of adduction and retraction of the globe, and narrowing of the palpebral fissure on attempted adduction.
Ophthalmoplegia
Paralysis of one or more of the ocular muscles due to disorders of the eye muscles, neuromuscular junction, supporting soft tissue, tendons, or innervation to the muscles.
Pathologic Nystagmus
Involuntary movements of the eye that are divided into two types, jerk and pendular.
Strabismus
Misalignment of the visual axes of the eyes. In comitant strabismus the degree of ocular misalignment does not vary with the direction of gaze. In noncomitant strabismus the degree of misalignment varies depending on direction of gaze or which eye is fixating on the target.

- This site details the different research that is being done at John Hopkins School of Medicine . This research is directed toward how the brain controls the movements of the eyes (including eye movements induced by head motion) using studies in normal human beings, patients and experimental animals. The focus is on mechanisms underlying adaptive ocular motor control.

Alec M Ansons and Helen Davis Blackwell Science, published December 2000 The aim of this book has always been to give guidance on the diagnosis and treatment of ocular motility disorders which is based on clinical experience rather than to provide a comprehensive treatise on the subject drawn from the primary literature. The Third Edition sees a new team of authors who have kept very much to this priniciple in their nevertheless thorough revision of the book. Whilst there are no new chapters as such, the immediate impact of the revision is in the improved page layout with increased use of diagrams and flowcharts. There are new sections on feigned visual loss in adults and children, the management of residual defects and the section on botulinum toxin treatment in chapter 8 has been re-written to take account of the great advances in this form of treatment.

Acquired ocular motility disorders in idiopathic intracranial hypertension

Rowe University of Liverpool, Department of Orthoptics, Liverpool, United Kingdom OBJECTIVES: The purpose of this paper is to report the acquired ocular motility findings in a review of a cohort of patients with idiopathic intracranial hypertension (IIH), many of which had been followed prospectively from their initial diagnosis. METHODS: A total of 38 patients with a diagnosis of IIH were recruited between 1993 and 1997. There were 33 females and five males with an age range of 11 to 57 years. Each patient had a full orthoptic, ophthalmic, and neurologic evaluation. RESULTS: Thirteen patients had an ocular motility deficit documented of which two had long-standing strabismus, one congenital paresis, and the remainder acquired defects of motility. Cranial nerve pareses other than sixth nerve paresis included partial third and fourth nerve pareses. CONCLUSIONS: Acquired ocular motility defects may be in the form of secondary strabismus, neurogenic palsies, or mechanical limitations of motility. Ocular motility signs in IIH may involve many of the cranial nerves, not just the sixth nerve as typically expected with a raised ICP.

These conditions may be divided into infranuclear, nuclear, and supranuclear disorders. Diseases of the eye muscles or oculomotor cranial nerves (III, IV, and VI) are considered infranuclear. Nuclear disorders are caused by disease of the oculomotor, trochlear, or abducens nuclei in the brain stem. Supranuclear disorders are produced by dysfunction of higher order sensory and motor systems that control eye movements, including neural networks in the cerebral cortex; basal ganglia; cerebellum; and brain stem. Ocular torticollis refers to a head tilt that is caused by an ocular misalignment. Opsoclonus refers to rapid, conjugate oscillations of the eyes in multiple directions, which may occur as a parainfectious or paraneoplastic condition (e.g., opsoclonus-myoclonus syndrome).
