Acute perforations of peptic ulcers continue as one of the real emergencies of surgery which require immediate attention and prompt operation. Incidence
During the past fourteen months 31 patients with perforated peptic ulcers were treated at the Permanente Foundation Hospital. As indicated in Table 1 , 28 patients (90.4 percent of this group) were treated surgically, with no fatalities. Three patients (9.6 percent) were treated non-operatively, with two deaths and a resultant mortality of 66.6 percent. Although our series is relatively small, several factors are revealed which may account for the absence of operative mortality. For comparison, Table 2 lists several reported series of perforated peptic ulcers and their respective operative mortality incidence. The operative mortality rate as shown in the large series collected by DeBakey of 23.4 percent can be considered as an average. The low mortality rate in the 51 cases presented by Graham

With the discovery in the late 1980s that bacteria were involved in causing peptic ulcers, many doctors dropped the notion that stress causes ulcers and declared them merely an infectious disease. Not so, says an expert panel convened by the Academy of Behavioral Medicine Research. The experts looked over the evidence and concluded that ulcers are not merely an infectious disease and that psychological factors do play a significant role. According to the panel's chair, Dr. Susan Levenstein, of the Nuovo Regina Margherita Hospital in Rome, more than four out of five people who are infected with Heliobacter never develop ulcers, while some people who develop ulcers have never been exposed to Helicobacter pylori (H.pylori), the bacterium blamed for "causing" ulcers. "This shows that other factors must be involved," points out Levenstein, who has researched the causes of ulcers for over 10 years. "Stress is probably one of them. Studies show that people who live through wars, earthquakes, or even large doses of the ordinary stresses of life are especially likely to develop ulcers, and ulcer patients who are anxious or under stress have a harder time healing and staying healed." Other research has suggested that psychological stress may push a H.pylori infected person over the edge into developing an ulcer, and may even help H.pylori multiply in the body. Researchers are trying to trace in both animals and humans how stress might promote H.pylori infection (present in most adults) into ulcers (which develop in few).

NSAIDs and Peptic Ulcers
A peptic ulcer is a sore that forms in the lining of the stomach or the duodenum (the beginning of the small intestine). An ulcer can cause a gnawing, burning pain in the upper abdomen; nausea; vomiting; loss of appetite; and weight loss. Most peptic ulcers are caused by infection with the bacterium Helicobacter pylori (H. pylori). But some peptic ulcers are caused by prolonged use of nonsteroidal anti-inflammatory drugs (NSAIDs) or pain relievers such as aspirin, ibuprofen, and naproxen sodium. NSAIDs cause ulcers by interfering with the stomach's ability to protect itself from acidic stomach juices. Normally the stomach has three defenses against digestive juices: mucus that coats the stomach lining and shields it from stomach acid, the chemical bicarbonate that neutralizes stomach acid, and blood circulation to the stomach lining that aids in cell renewal and repair. NSAIDs hinder all of these protective mechanisms, and with the stomach's defenses down, digestive juices can damage the sensitive stomach lining and cause ulcers.

A large amount of excess acid is produced in response to the overproduction of the hormone gastrin, which in turn is caused by tumors on the pancreas or duodenum. The microorganism Helicobacter pylori (HP) plays an important part in pathogenesis of peptic ulcers. HP penetrates through the mucous layer and proliferates on the epithelial cells, thus being protected not only from the hydrochloric acid of the gastric juice, but from the majority of antibacterial medicines as well. Recent reports on the effectiveness of millimeter waves (MMW) in fighting peptic ulcers motivated the authors to put this method on trial for HP-contaminated ulcers. This Russian study was carried out with 51 patients with duodenal ulcers and 9 patients with gastric ulcers. The authors concluded that MMW treatment decreased HP contamination and facilitated recovery of gastric and duodenal ulcer patients. [BENER

Biology of Helicobacter pylori Helicobacter are Gram-negative, aerobic or microaerophilic, spiral-shaped bacilli that are motile by way of flagella at one end of the cell. Closely related to Helicobacter are species of the genera Aquaspirillum Azospirillum Spirillum , and Camplyobacter . The genetics of Helicobacter pylori are complex; there are many strains of H . pylori which are distinguished by the human disease with which they are associated. Once Helicobacter pylori colonizes its host, it lives in the interface between the surface of gastric epithelial cells and the overlying mucus gel layer, often clustering at the junctions of epithelial cells. In addition H. pylori can also be found on top of the gastric epithelium in the duodenum and esophagus. It was not until 1983 that H . pylori was recognized as having any medical importance. Ulcers i n the stomach or duodenum were attributed for years to excess stomach acid and treated with antihistamines which reduce acid secretion and antacids. In the past, milk was recommended, but dairy products can be the cause of the problem and are often contra-indicated; milk allergy may be an original cause of gastritis which leads to ulceration. A common cause of stomach ulcers is the regular use of ASA or related anti-inflammatory drugs - NSAIDs. If ulcer symptoms occur while taking these drugs, their use should be discontinued. An acute duodenal ulcer will present with pain high and central in the abdomen; the pain comes on as the stomach empties 3 or 4 hours after eating and during the night. Eating food, especially bland foods, milk or antacid tends to relieve the pain. Antihistamines ( H2 blockers - examples are tagamet, pepcid and zantac) have been prescribed for years to treat ulcers and acid reflux into the esophagus and are now available as over-the counter medications.

The inflammation may be precipitated by aspirin and nonsteroidal anti-inflammatory medications (NSAIDs).
Duodenal ulcers are commonly associated with the presence of the bacteria Helicobacter pylori in the stomach. Risk factors are aspirin and NSAID use, cigarette smoking , and older age. Duodenal ulcer has historically occurred more frequently in men, but more recent data suggest similar rates in both men and women. The lifetime prevalence of a peptic ulcer 5 to 10% and approaches 10 to 20% in patients who are Helicobacter pylori positive.
