Treatment of POEMS Syndrome at Mayo Clinic in Rochester
POEMS syndrome is a very rare type of bone marrow disorder whose cause is unknown. Neither an inherited nor contagious disease, it involves an overgrowth of bone marrow cells called plasma cells. In patients with POEMS syndrome, this abnormal collection of cells produces chemicals that damage other parts of the body. Patients sometimes confuse POEMS syndrome with multiple myeloma. The symptoms and potential complications of these diseases are very different. The average expected life span is better in patients with POEMS (who may have osteosclerotic myeloma ) than it is in patients with multiple myeloma.
How is POEMS syndrome diagnosed?
The diagnosis is made by combining the information from a thorough history, physical examination, and blood, urine, X-ray and bone marrow tests. The overgrowth of plasma cells can often be detected by finding an abnormal protein in the blood or urine (a monoclonal protein), by an abnormality on a bone X-ray, and/or a bone marrow biopsy. POEMS is a syndrome, which means that several features must be present to make the diagnosis. Because it is so rare, the diagnosis is often missed. Initially, you may have been told that you had a nerve problem without any known cause. Or you may have been told that you have multiple myeloma, which is a type of bone marrow cancer.

Background: POEMS (polyneuropathy, organomegaly, endocrinopathy, monoclonal gammopathy, skin changes) syndrome is a rare multisystemic disease that occurs in the setting of a plasma cell dyscrasia. The pathophysiologic link between the constellation of symptoms and the underlying disease is not well understood, but the link may be related to changes in the levels of a cytokine or growth factor. POEMS syndrome first was described by Crow in 1956 and then by Fukase in 1968. The syndrome was termed Crow-Fukase (by which it is known in Japan) syndrome by Nakanishi in a study of 102 cases in Japan. In 1980, the acronym POEMS was coined by Bardwick et al based on the 5 main features of the disease, namely, polyneuropathy, organomegaly, endocrinopathy, monoclonal gammopathy, and skin changes. No specific case definition for POEMS exists; however, most authors agree that patients with POEMS syndrome should have 3 or more of the 5 systems involved. The pathophysiology of the disease is not known; however, elevated levels of cytokines and growth factors have been implicated. Polyneuropathy: The polyneuropathy associated with POEMS syndrome is a bilateral symmetric disturbance. It involves both motor and sensory nerves, begins distally, and has a progressive proximal spread. Associated cranial or autonomic nerves are not involved. Both demyelination and axonal degeneration are noted.

Other common abnormalities included in the syndrome include:
  • Fatigue and weakness Edema (swelling of the ankles and legs) Enlarging breast tissue in both men and women Generalized aches and pains Ascites (fluid collection in the abdomen) Changes in vision
Some patients have a coexisting disorder called Castleman's disease, which is an abnormality of the lymph nodes. Though the majority of patients have abnormal bone X-rays, most will not have bone pain. In order to have the syndrome, you do not need to have all the features. All patients, however, have nerve damage and a "plasmaproliferative disorder" and/or Castleman's disease. It is important to identify an incomplete POEMS syndrome because you may be at risk to develop some of the other symptoms later.
