Polycystic ovarian syndrome is a common cause of anovulation and infertility in women. Women with this syndrome do not ovulate regularly and have ovaries that contain multiple small cystic structures, usually about 4-9 mm in diameter. This gives the ovaries a characteristic appearance on ultrasound. The basic problem is that although the polycystic ovaries contain many follicles with eggs, the follicles do not develop and mature properly so that they do not ovulate. Women without polycystic ovaries normally develop one mature follicle each month which ovulates (releases) one egg.

Women develop polycystic ovarian syndrome (PCOS) when they have too many male hormones and not enough female hormones, making ovulation rare or irregular. Normally a woman ovulates when an egg-containing follicle grows and releases a mature egg. Polycystic ovarian syndrome keeps those follicles from growing, causing them to build up in the ovaries and form small benign estrogen-releasing cysts. The combination of the estrogen and the male hormones throws the production of two other hormones, follicle-stimulating hormone (FSH) and luteinizing hormone (LH), which also helps inhibit ovulation, off balance. Without ovulation, male hormone levels remain high, and the cycle repeats itself. Polycystic ovarian syndrome, also called Stein-Leventhal syndrome, affects between one and 4 percent of women in the United States.

    What is Polycystic Ovarian Syndrome? by Tracee Cornforth Polycystic ovarian syndrome (PCOS) is a hormonal disorder that affects an estimated seven percent of all women. It is the most common hormonal disorder among women. According to experts, the actual number of women affected by PCOS may be as high as one out of ten simply because so many cases remain undiagnosed. Why are so many cases of polycystic ovarian syndrome undiagnosed? Because the symptoms can vary from woman to woman it is often difficult to accurately diagnose polycystic ovarian syndrome. Its symptoms are often heartbreaking both emotionally and physically. Most women have never even heard of PCOS, yet it causes a wide variety of symptoms that often affect female reproductive health in ways that can be truly devastating.     Polycystic Ovarian Syndrome (PCOS)
    Polycystic ovarian syndrome (PCOS) is the most common hormonal disorder in women of reproductive age, affecting up to 10% of the population. PCOS is one of the leading causes of female infertility and is associated with long-term health risks of heart disease and diabetes. Three features of PCOS are 1) enlarged ovaries containing multiple, small cysts; 2) excess androgens (male hormone); and 3) chronic amenorrhea (missed periods). Symptoms of PCOS
    • Hirsutism (excess body and/or facial hair), particularly on the chin, upper lip, breasts, inner thighs and abdomen
    • Irregular ovulation or lack of ovulation - along with irregular or no periods. Women with PCOS may have oligomenorrhea (8 or fewer periods a year), amenorrhea (absence of periods), or polymenorrhea (too frequent periods)
    • Obesity
    • Acne and/or oily skin (particularly severe acne as a teenager or acne that persists into adulthood
    • Infertility
    • Ovarian cysts
    • Insulin resistance
    • Hair loss or balding
    • Acanthosis nigricans (darkening of the skin, usually on the neck - is also a sign of insulin problems)

    What is PCOS?

    PCOS or Polycystic Ovarian Syndrome is the most common hormonal disorder in women of reproductive age. It affects up to 10% of pre-menopausal women and causes 75% of all cases of anovulatory infertility. In PCOS, ovulation fails to occur on a regular basis. Symptoms can include irregular menstrual cycles, increased amounts of body hair, and a tendency to easily gain weight, especially in the trunk and abdomen. Recent research indicates that the underlying defect in as many as half of all PCOS patients may be "insulin resistance" or decreased sensitivity to the effect of insulin. This can result in elevated insulin levels which then eventually lead to the symptoms of PCOS.

    Polycystic Ovarian Syndrome (PCOS)
    What is PCOS?
    PCOS is a term used to describe a common endocrine disorder(syndrome) that occur in about 6% of women. The most common symptoms are:
    • Hirsutism - an excessive amount of body hair growth compared to other women in your ethnic community, since women of different racial backgrounds can have very different amounts of body hair normally.
      Anovulation - The monthly release of an egg in the middle of the menstrual cycle is called ovulation. When something prevents that release, it is called anovulation. Women who do not ovulate have missed periods (oligomenorrhea), and sometimes stop having menstrual periods completely (amenorrhea).
      Cystic Ovaries - Cysts are fluid filled sacs that are found on the outside part of the ovary. Cysts are a common occurrence in many women, but in PCOS there are an increased number of small cysts in most (but not all) women. These cysts are seen during an ultrasound test. Some women (about 20%) have many cysts in their ovaries, but do not have PCOS.

    Making the Diagnosis of PCOS

    In no other gynecological condition is the general medical history more important than in PCOS. There are 3 different ways to make the diagnosis of PCOS: 1) by symptoms and physical findings, 2) hormonal testing, and 3) ultrasound. Probably most individuals will have abnormalities in all three, some only in two, and possibly only in one. Some may argue that findings in only a single category may not constitute PCOS. But, until we have PCOS better characterized, or find a different diagnosis for these patients, the diagnosis of PCOS should remain and provides a good starting point for communication. Even the most minor of apparent problems may have significant implications for future general health and well-being. The classic findings of PCOS are: menstrual cycle abnormalities, increased sexual hair growth and obesity.
    Menstrual Disturbance
    Often in PCOS patients, the menarche occurs at the usual age of 12-13 years. Some PCOS patients may start menstruating earlier. Not uncommonly, PCOS patients may first be seen by a physician for lack of menses. Any female who has not had menses by age 16 should be evaluated. The menstrual cycle may at first be regular, but by high school, cycles start to lengthen and may be skipped. Often during this time, oral contraceptives are started. The "pill" usually regulates the menstrual cycle and may give the false impression that all is well. Usually in the teenage years, the other symptoms of skin and weight problems also start to be seen. Some PCOS patients easily establish a pregnancy in these early years. Occasionally, birth control pills may even increase the chance of pregnancy by suppressing abnormal hormonal production.
