What research is being done? ... Related NINDS Publications and Information What is Polymyositis? Polymyositis is an inflammatory muscle disease that causes varying degrees of decreased muscle power. The disease has a gradual onset and generally begins in the second decade of life. Polymyositis rarely affects persons under the age of 18. The most common symptom is muscle weakness, usually affecting those muscles that are closest to the trunk of the body (proximal). Eventually, patients have difficulty rising from a sitting position, climbing stairs, lifting objects, or reaching overhead. In some cases, distal muscles (those not close to the trunk of the body) may also be affected later in the course of the disease. Trouble with swallowing (dysphagia) may occur. Very rarely, the muscles ache and are tender to touch. The disease may be associated with other collagen vascular, autoimmune or infectious disorders

Myositis literally means 'inflammation of the muscles'. This term covers a number of related illnesses including Dermatomyositis, Polymyositis, Juvenile Dermatomyositis and Inclusion Body Myositis. These diseases fall under the classification of 'Autoimmune' diseases, in that they are the result of abnormal immune responses (which the body would normally use to fight off diseases) attacking normal tissues - in this instance, the muscles. Thankfully these diseases are very rare, and to date, no-one really knows what causes them. As these diseases affect the muscles and connective tissues of the body, the main symptom is muscular weakness, which may be progressive and can be severely disabling, affecting in the majority of cases the whole body. People of any age and sex can develop 'Myositis', and depression and a general feeling of misery can frequently ensue. These diseases are not catching and there is no way to predict who may become affected. They can usually be suppressed by drugs which often have side effects which can promote worrying problems of their own. As yet there is no true cure for the disease, but many sufferers can go into partial or complete remission. Many sufferers need physiotherapy to prevent muscle shrinkage and to keep them mobile. Some fortunately do get better, but for the majority it is a case of coping with the disease and trying to live as near a normal life as possible. Some less fortunate do need a wheelchair from time to time while the disease is at its height.

Polymyositis
Polymyositis is an inflammatory myopathy. It is believed to result from an autoimmune reaction, whereby the body's own immune system attacks the muscle cells. This results in the breakdown of muscle. The result of this attack on the muscles is inflammation, with muscle tenderness as an occasional complaint. The onset of symptoms may be acute, but the condition usually progresses slowly. If left untreated, the patient's ability to ambulate may be compromised. Once the diagnosis is established, treatment is straight forward and often successful in reversing the symptoms.

Polymyositis (adult)
Definition: A systemic connective tissue disease, which is characterized by inflammation and degeneration of the muscles.
Causes, incidence, and risk factors: The cause of this disorder is unknown. It is thought that an autoimmune reaction or a viral infection of the skeletal muscle may cause the disease. It can affect people at any age, but most commonly occurs in those between 50 to 70 years old, or in children between 5 to 15 years old. It affects women twice as often as men. Muscle weakness may appear suddenly or occur slowly over weeks or months. There may be difficulty with raising the arms over the head, rising from a sitting position, or climbing stairs. The voice may be affected through weakness of the throat muscles.

General Information on Polymyositis
The word polymyositis (PM) means "many muscles inflammed". PM is a subacute or chronic (long lasting) general disorder of striated muscle. A common feature found chronically is progressive, symmetric muscular weakness in muscles closest to the trunk or torso. Many individuals with PM experience periods of increased symptoms called relapses or exacerbations and decreased symptoms known as remissions.
Polymyositis is an uncommon disorder with an annual incidence of 1 per 100 000. The disorder has a female predominance of 2:1, most frequently affecting African-American women. The disorder usually starts between the ages of 45-64.
PM is one out of five categories of inflammatory muscle disease that is distinguished under the term myositis (tissue swelling). The other four categories are: Dermatomyositis (DM), myositis and malignancy, childhood PM and DM, PM associated with other inflammatory disease (Rheumatoid Arthritis, Sjorgen's disease, mixed connective tissue disease).
The cause is unknown. It is believed to originate from the spontaneous attack on muscle cells by cytotoxic T lymphocytes (white blood cells), the immune cells of inflammation. The result involves impaired power in proximal muscles (muscles closest to your trunk or torso). There is some indication of autoimmune and viral infection causes to the disorder, however, no conclusive evidence has been reported.

Polymyositis And Dermatomyositis
Systemic connective tissue diseases characterized by inflammatory and degenerative changes in the muscles (polymyositis) and frequently also in the skin (dermatomyositis), leading to symmetric weakness and some muscle atrophy, principally of the limb girdles. Classification of the types of myositis includes primary idiopathic polymyositis; childhood polymyositis or dermatomyositis; primary idiopathic dermatomyositis in adults; inclusion body myositis; polymyositis or dermatomyositis associated with malignant tumors; and polymyositis or dermatomyositis associated with various connective tissue disease overlap syndromes, including mixed connective tissue disease (see below ) and sclerodermatomyositis. Polymyositis and dermatomyositis also share certain clinical findings with systemic sclerosis or, less frequently, SLE or vasculitis.

Classification Criteria for Dermatomyositis and Polymyositis
Patients presenting at least one item from the first criterion and four items from the second through ninth criteria are said to have dermatomyositis. Patients presenting no items from the first criterion and at least four items from the second through ninth criteria are said to have polymyositis.
  • Skin lesions
    • Heliotrope rash (red purple edematous erythema on the upper palpebra)
    • Gottron's sign (red purple keratotic, atrophic erythema, or macules on the extensor surface of finger joints)
    • Erythema on the extensor surface of extremity joints: slightly raised red purple erythema over elbows or knees
  • Proximal muscle weakness (upper or lower extremity and trunk)
  • Elevated serum CK (creatine kinase) or aldolase level
  • Muscle pain on grasping or spontaneous pain
  • Myogenic changes on EMG (short-duration, polyphasic motor unit potentials with spontaneous fibrillation potentials)
