In conclusion, in a population of non-diabetic patients, with initial excellent kidney function: 1)Patients who lost their grafts did not differ from those who maintained graft function as far as lipid profile, hypertension, gender, race or age. 2) Degree of proteinuria predicted time to graft failure, independent of creatinine or GFR at the time of measurement, and thus did not appear to be simply a marker of poorer kidney function; 3)Proteinuria greater than 1 gram increased risk of graft loss 5-fold over the ensuing 4 year follow-up; 4) The possibility that proteinuria per se is injurious to the kidney should be entertained. Proteinuric transplant patients may benefit from treatment with ACE inhibitors or angiotensin-receptor blocking agents as is the recommendation for patients with proteinuric disease of their native kidneys, in order to offset the potentially deleterious effects of protein trafficking on the transplanted kidney.

Proteinuria
From Wikipedia, the free encyclopedia. Proteinuria (from protein and urine ) means the presence of an excess of serum proteins in the urine Serum protein that get filtered into primary urine are readily absorbed. If protein is present in urine sample it usually means some damage to renal structures or excess of some pathologic protein in serum. With gross proteinuria hypoproteinemia can develop and it results in diminished oncotic pressure ( ascites , edema, hydrothorax). Proteinuria is a leading symptom of:

Background
Patients with diabetes are at increased risk for death from cardiovascular disease (CVD). Aggressive treatment of hypertension in diabetes has been recommended but in 1 study an increase in mortality was observed in patients with diabetes who received diuretics. This study was done to investigate whether the mortality was attributable to other risk factors or independently related to the antihypertensive treatment.

    Peer Review Status: Internally Peer Reviewed (1) Introduction Proteinuria may be the first and only evidence of renal disease. Most healthy persons excrete up to 150 mg of urinary protein daily, which may vary from negative to 2+ on urine dipstick analysis. Although the prevalence of proteinuria on routine screening of healthy individuals has been as high as 3.5 percent, the incidence of renal disease in this population is very low. It is clear that the discovery of proteinuria necessitates a thorough evaluation to separate these healthy individuals from those with significant renal disease. (2) Urinalysis (3) Evaluation Once proteinuria is detected by dipstick, the next step should be to obtain a detailed history, physical exam, and urine microscopy. This includes family history as well as prior evaluations and medications (i.e., NSAIDs). The exam should look for evidence of renal or systemic disease, focusing on blood pressure, pulse, fundi, cardiovascular, skin, abdominal (palpable kidneys or bruits), and the presence or absence of edema. (4) Functional Proteinu ria If no such evidence is found, repeat dipstick analysis should be done two or three times. If these subsequent qualitative tests, in well-concentrated specimens, are negative for protein, the initial proteinuria is transient or functional. Functional proteinuria may be seen in conditions such as high fever, emotional stress, cold exposure, strenuous exercise, infection, and other acute medical illnesses. Idiopathic transient proteinuria is a common cause of proteinuria in young adults, especially men. It is characterized by proteinuria on a routine urinalysis that disappears on repeat testing and is found in asymptomatic, healthy individuals with normal renal function.

    HEMATURIA
    Se considera hematuria la presencia de cinco o más hematíes en un parcial de orina, lo que puede estar presente por patología a cualquier nivel del árbol urinario, desde la parte renal más extrema hasta la parte terminal de la uretra, como respuesta a un proceso neoplásico, vascular, infeccioso o inmunológico. Se considera hematuria microscópica cuando es detectada solamente a través del microscopio y el aspecto de la orina es ámbar, y macroscópica cuando el paciente observa en su orina un cambio en la coloración. El color rojo rutilante habla de proceso en vías urinarias bajas (vejiga, uretra) y el color oscuro o tipo cocacola habla en favor de tener su origen en las vías urinarias altas (riñón, pelvis). El hecho de que la hematuria sea macro o microscópica, no habla de la severidad de la lesión, sino solamente de patología adyacente.
