Background: Respiratory Syncytial virus, the most important cause of lower respiratory tract infections in infants and young children in industrialized countries, is associated with increased morbidity in premature infants with or without bronchopulmonary dysplasia as well as those with congenital heart disease. Because of observations that lower rates of disease occur immediately after birth, presumably due to vertical transmission of maternal antibody, and animal studies where protection from pneumonia was observed through administration of immune globulin, the efficacy of passive prophylaxis in premature infants has been studied. Objectives: This meta-analysis was performed to assess the effects of polyclonal respiratory syncytial virus hyperimmune globulin or monoclonal antibody in preventing RSV hospitalization, receipt of intensive care, mechanical ventilation, and mortality in those with underlying prematurity, bronchopulmonary dysplasia, or congenital heart disease.

Ribavirin is approved for treatment of these infections, but its use is controversial because of questions on its efficacy, concerns about occupational exposure and its high cost. Objectives: Respiratory syncytial virus (RSV) is a common cause of pneumonia in infants. Ribavirin is the only antiviral therapy available against RSV. The objective of this review was to assess the effects of aerosolized ribavirin for infants with RSV lower respiratory tract infection. Search strategy: We searched MEDLINE from 1975 to 1999, we scanned reference lists of articles, and we contacted experts in the field.

Description : Palivizumab or Synagis is a lyophilized (freeze-dried) formulation of a recombinant humanized (chimeric murine-human) IgG1kappa monoclonal antibody glycoprotein with specificity for an epitope in the A antigenic site of the F (fusion) protein of respiratory syncytial virus (RSV). Palivizumab is expressed from a stable mouse myeloma cell line (NS0) which can produce about 1 gram/liter of culture. Synagis was the first monoclonal antibody product approved in the U.S. for an infectious disease indication. Its initial launch was the most successful of any biopharmaceutical product, in terms of rapid gain of sales and market share. Humanization of the murine monoclonal antibdoy prevents human anti-mouse antibody (HAMA) reactions or recognition of the antibody as foreign (i.e., palivizumab presents itself as a human antibody). Humanized antibodies, such as Synagis, can have a longer in vivo half-life and be less immunogenic than conventional murine monoclonal antibodies. An article describing the development of Synagis was published in the Journal of Infectious Diseases (vol. 176, no. 5, p. 1215-24, November 1997).

Respiratory syncytial virus (RSV), a common cause of winter outbreaks of acute respiratory disease, results in an estimated 90,000 hospitalizations and 4500 deaths each year from lower respiratory tract disease among infants and young children in the United States (1). Outbreaks occur annually throughout the country (2,3). RSV activity in the United States is monitored by the National Respiratory and Enteric Virus Surveillance System (NREVSS), a voluntary, laboratory-based system. This report summarizes trends in RSV reported by NREVSS for July 1992-June 1997 and presents provisional surveillance results for July-November 1997. These data indicate onset of widespread RSV activity for the 1997-98 season. Since July 1992, a total of 100 clinical and public health laboratories in 47 states have participated in NREVSS and have reported weekly to CDC the number of specimens tested for RSV by the antigen-detection and virus-isolation methods and the number of positive results. RSV activity is considered by NREVSS to have become widespread during the first of 2 consecutive weeks during which at least half of participating laboratories report any RSV detections. This definition generally indicates a mean percentage of specimens positive by antigen detection in excess of 10%.

RSV, respiratory syncytial virus
RSV is the common abbreviation for respiratory syncytial virus, the causative virus for many common colds, but more importantly for the majority of cases of infant bronchiolitis . It occurs in outbreaks chiefly during the cooler months. In and of itself it is a harmless cold virus but is sometimes mentioned in news stories as a terrible new plague. However, it is one of the most common infections of childhood: one half of all children will have an RSV infection in the first year, and essentially all children have had an RSV infection by age 2. These respiratory infections cause an estimated 120,000 hospital admissions in young children in the US annually. Interest in the virus accelerated in recent years for two reasons:
  • we developed a rapid viral identification test performed on bronchial secretions to confirm the the virus
  • new therapies were developed to treat and now, to prevent the disease; the therapies and preventative treatments are all quite expensive and debate continues about how medically effective and cost effective they are
In general, prematures and babies who had other significant respiratory problems in the immediate postnatal period are the target population for the preventative treatment strategies, as well as babies with immune deficiencies or

A: Respiratory Syncytial Virus is the most common respiratory virus in infants and young children. It infects virtually all infants by the age of two. In most infants, the virus causes symptoms resembling those of the common cold. In infants born prematurely and/or with chronic lung disease, RSV can cause a severe or even life-threatening disease. Prior to the introduction of Synagis, RSV disease resulted each year in over 125,000 hospitalizations. There was a high mortality risk in approximately 2 percent of those infants. Q: Is RSV contagious? A: Yes, RSV is highly contagious. Each year, up to 50 percent of infants are infected. The infection can be spread through the air, via coughing and sneezing. Transmission can also occur by touching the respiratory secretions of an infected person, and then rubbing your own eyes, nose, or mouth. RSV can survive for 4-7 hours on surfaces such as cribs and countertops. Transmission may be prevented by standard infection control practices, such as hand washing and proper cleaning. Respiratory syncytial virus (RSV) is a virus that can cause severe lower respiratory infections in children under the age of two, and milder upper respiratory infections in older children and adults. RSV infection is also called bronchiolitis, because it is marked in young children by inflammation of the bronchioles. Bronchioles are the narrow airways that lead from the bronchi to the tiny air sacs (alveoli) in the lungs. The result is wheezing , difficulty breathing, and sometimes fatal respiratory failure Description RSV infection is caused by a group of viruses found worldwide. There are two different subtypes of the virus with numerous different strains. Taken together, these viruses account for a significant number of deaths in infants. RSV infection is primarily a disease of winter or early spring, with waves of illness sweeping through a community. The rate of RSV infection is estimated to be 11.4 cases in every 100 children during their first year of life. Respiratory syncytial virus (RSV) Respiratory syncytial virus (RSV) is a very common virus. This virus is a major cause of infections of the lungs and breathing passages in young children. RSV infections often occur in epidemics, usually from late fall through early spring. The highest rates of RSV infection happen in infants from about two to eight months of age. While most infections are mild, RSV can cause severe illness in infants who were born premature or who have lung problems. RSV is highly contagious. RSV is usually passed from person to person by contact with infected body fluids from the nose or mouth of someone with a RSV infection. RSV is most contagious during the first two to four days of infection, but RSV may be spread for up to three weeks (or longer) after the infection has begun. Following exposure, symptoms start within four to six days. Symptoms range from severe to mild. In adults and children over age three, RSV usually causes upper respiratory tract cold symptoms such as: stuffy or runny nose, sore throat, mild headache, mild cough, fever, and a general "ill" feeling. In children younger than age three, RSV infections are more severe with lower respiratory tract illness like bronchiolitis. Symptoms include high fever, severe cough, wheezing, abnormally rapid breathing, and difficulty breathing. In cases of severe RSV infection, the child may have retractions (drawing in) of the chest muscles and skin between the ribs, which indicates major breathing difficulty. RSV infections usually last about a week but may be longer.
