Ness Motley filed in August one of the first cases against the manufacture of the biological product, Enbrel (etanercept). Enbrel (etanercept) was approved by the U.S. Food and Drug Administration on November 2, 1998, as a second-line agent for the treatment of juvenile rheumatoid arthritis. At the time Enbrel was co-marketed by Immunex, Corp. (in 2001 Amgen, Inc. bought Immunex Crop. of Seattle and Enbrel for some $16 billion, the largest biotech buyout ever) and Wyeth-Ayerst Pharmaceuticals. Although Enbrel has been touted in the press and through marketing as a wonder "drug", it is a potent product whose long term effects and mechanism of action are not known or fully understood. The adverse effects that are known are serious and fatal. Patients have neither been warned adequately of these risks nor their severity.
Enbrel is a biological product (a genetically engineered antibody), which reduces the actions of chemicals in the body that are involved in inflammatory and immune responses. Specifically

Kineret (anakinra) is a recombinant, nonglycosylated form of the human interleukin-1 receptor antagonist (IL-1Ra). Kineret differs from native human IL-1Ra in that it has the addition of a single methionine residue at its amino terminus. Kineret consists of 153 amino acids and has a molecular weight of 17.3 kilodaltons. It is produced by recombinant DNA technology using an E. coli bacterial expression system. Web posted at: 7:53 p.m. EDT (2353 GMT) BOSTON (CNN) More than 2 million Americans who suffer from the crippling effects of rheumatoid arthritis could benefit from a new drug that appears to significantly reduce their symptoms, say study results published in Thursday's issue of the New England Journal of Medicine. A genetically engineered protein in the drug, known by the brand name Enbrel, blocks a natural substance in the body called tumor necrosis factor, or TNF. Researchers have known for about a decade that TNF levels rise in the joint fluid of arthritis patients. A study at the University of Alabama at Birmingham found that blocking TNF reduced the number of swollen or sore joints in people with rheumatoid arthritis. CNN's Al Hinman reports. In the UAB study, 180 patients were given the drug for three months. Those receiving the highest dosage had a 61 percent reduction in the number of swollen or sore joints, compared with 25 percent in those taking placebos.

Juvenile Rheumatoid Arthritis
Arthritis beginning at or before age 16. Juvenile rheumatoid arthritis (JRA) is similar to adult RA (see Ch. 50 ). The disease tends to affect large and small joints and may interfere with growth and development. Micrognathia (receded chin) due to impaired mandible growth may occur.
Symptoms and Signs
Systemic onset (Still's disease) occurs in about 20% of patients. High fever, rash, splenomegaly, generalized adenopathy, serositis, and a striking neutrophilic leukocytosis and thrombocytosis occur often. At times these manifestations precede the appearance of the arthritis. Rheumatoid factor and antinuclear antibodies are absent. Pauciarticular onset occurs in about 40% of patients, usually young girls. Antinuclear antibodies are present in up to 75% of affected children. Chronic iridocyclitis occurs in nearly 20% of these patients; it is often asymptomatic and is detected only with periodic slit-lamp examinations. A subgroup of affected older boys includes many who have HLA-B27 antigen. Most of these boys subsequently develop classic features of one of the seronegative spondyloarthropathies: ankylosing spondylitis, inflammatory bowel disease, psoriatic arthropathy, or Reiter's syndrome.

Web posted at: 5:43 p.m. EDT (1743 GMT) WASHINGTON (CNN) The Food and Drug Administration on Friday approved the first oral treatment to slow the progression of rheumatoid arthritis. The FDA said Arava, marketed by Hoechst Marion Roussel, is not a cure, but has been shown to relieve the painful swelling and slow the joint damage associated with the disease. Rheumatoid arthritis is an auto-immune disease that affects more than 2 million Americans. It often develops between the ages of 25 and 50. Arava blocks production of immune cells that cause joint inflammation. In one study, the drug improved the condition of 41 percent of 480 arthritis patients. The current treatment for rheumatoid arthritis is low doses of the cancer drug methotrexate. It has serious side effects and its potency in the body can diminish over time.

When my Grandmother was diagnosed with RA a few decades ago, the milder cases were simply put on aspirin, over 20 a day! If they didn't die of a bleeding ulcer, they were quickly disabled from the disease. The more severe cases might be put on prednisone or gold but was pretty much it. Not a lot to choose from.
As much as I hate having RA, I am glad I have it now. Never before have we had so many treatment options. In the last two years we've been bombarded with new drugs and treatments. We have so much to choose from. Most of us will live full productive lives, unlike those a generation ago.
There are a host of new drugs in development, and maybe even a cure in the works. The next generation of RAers may never even have to consider a joint replacement or take a liver damaging drug, what a wonderful world that will be! Disease Description
