My name is Blanche Dillon, I live just outside Dallas, Texas. I had suffered from sciatic pain for ten years and severe sciatic pain constantly for over three years.     Compression or trauma of the sciatic nerve or its roots, esp., that resulting from ruptured inverbertebral disk or osteoarthritis of lumbosacral vertebrae. Inflammation of sciatic nerve resulting from metabolic, toxic or infectious disorders. Pain referred to the sciatic nerve from other parts of the body.

    Symptoms
    May begin abruptly or gradually and is characterized by a sharp shooting pain running down the back of the thigh. Movement of the limb generally intensifies the suffering. Pain may be uniformly distributed along the limb but frequently there are certain spots where it is more intense; numbness, tingling; nerve may be extremely sensitive to the touch. Symptoms grow worse at night and on approach of stormy weather. Duration of attack varies from a few days to several months. In long-standing cases, muscles grow atrophied and rigid. Recovery follows in majority of cases when treatment is instituted early and is carried out persistently. Some people claim that sciatica, in men, may be caused by "wallet-itis" as they term it. Persistently carrying the wallet in the hip pocket of trousers, sitting on the wallet every day, causes an irritation of the sciatic nerve. To alleviate the sciatic pain and irritation of the sciatic nerve, move the wallet to the other side, or better still, don't use the hip pocket for carrying a wallet at all.

You've stepped off the stairclimber after a 30-minute workout, and suddenly a sharp pain shoots down your right leg. Your whole leg feels weak and numb, and you start hopping on your left foot, like a marionette that's had the strings to one leg cut off. You may have just experienced an attack of sciatica. Sciatica is an inflammation of the sciatic nerve cord, which branches off the spinal cord in your lower back, runs beneath your buttock muscles and continues all the way down your leg. This large bundle of nerves, almost three quarters of an inch wide, supplies sensation and motor control to your buttocks, hips and legs. Pain can originate in your lower back and travel down to your buttocks, thighs or lower legs, or it can flare up anywhere along the length of the nerve. Sciatica can lead to muscle weakness - and it can cause you to lose your reflexes in your knee or ankle; yet this loss of reflex is difficult to detect unless a doctor checks for it.
The Disc Connection
One common cause of sciatica is a damaged, or herniated, disc in the spinal column that pinches a nerve at the root of the sciatic cord. These discs sit like cushioning jelly doughnuts between your vertebrae, the bones that make up your spinal column.

Sciatica Sciatica refers to pain that begins in the hip and buttocks and continues all the way down the leg. This condition is often accompanied by low back pain, which can be more or less severe than the leg pain. The term "sciatica" indicates that the sciatic nerve, which travels from the lower back through the buttocks and into the leg, is thought to be the cause of the pain in this condition. True sciatica is a condition that occurs when a herniated lumbar disc compresses one of the contributing roots of the sciatic nerve. This type of low back pain is less common than other causes and conditions that produce back pain . For instance, sporting activities, recreational activities, and heavy labor can cause back and leg pain, which is commonly misdiagnosed as sciatica. The challenge for a physician is to distinguish between radicular pain , which is caused by an inflamed nerve root, and referred pain, which is a result of a musculoskeletal sprain or strain. Symptoms
The most common symptom of true sciatica is posterior thigh, lower leg or foot pain that can be much worse than the accompanying lower back pain. Usually a patient will experience moderate to severe pain, which begins in the buttocks and runs down through the leg or foot. It is important to know that true sciatica will produce pain that radiates beyond the knee. Often a patient will have a previous history of lower back pain beginning a few days or weeks before the leg pain occurs, then the leg pain becomes worse than the back pain, and in some cases the back pain will completely disappear.

Sciatica and exercise March 30, 2000 Question I enjoy physical activities such as weight lifting, walking and swimming, but I have sciatica. How much exercise should I do?
Answer The first thing is to be sure that you have got sciatica, which is due to pressure on the nerve roots in the areas of the spine known to doctors as the lower lumbar and upper sacral spine. It is caused by spinal disc protrusion. The pressure on the nerve root gives rise to pain and neurological (nervous system) signs on the same side in the thigh and calf. The same pain may well come from joint or ligament disease but of course there will be no neurological symptoms. It is also true that any pressure on the nerve root will cause sciatica-like pain; so perhaps in certain people other things that may be squeezing the nerve need to be excluded. What to do: In most cases a few days in bed is all that is required and after that activity is encouraged. This activity of course is progressive depending on pain and may require analgesics, such as paracetamol (acetaminophen), to allow the exercises to take place. With more severe prolapse, continuous bed rest and traction (with weights and pulleys) may be needed for 2 weeks and this will reduce the herniation (i.e. disc protrusion) in over 90% of cases. If symptoms and signs have not improved, then maybe an injection of steroids and a local anaesthetic will do the trick. Very occasionally it is necessary to surgically operate on the disc.
