Extra Large Market.
Growing market due to growing aging population.
The most conservative estimates suggest that over 30 million men in the U. S., over 30 million men
in the E. U., and over 150 million men worldwide are affected with erectile dysfunction (ED). More liberal estimates estimate that over half of all men 40 years of age and older have some form of erectile dysfunction. However, it appears that only one in 20 affected men seeks medical treatment.
Incidence of sexual dysfunction, primarily erectile dysfunction, resulting from nerve damage, is particularly high in patients with prostate cancer, following prostatectomy, affecting 60 to 85 percent
of patients (JAMA. 2000; 283:354-360).
Currently, the worldwide annual market for the treatment of erectile dysfunction (ED) exceeds
$2 billion, and has been projected to grow rapidly with increasing of the aging population worldwide. Approved Products

SEXUAL DYSFUNCTIONS
It is very important to note that many of the difficulties that people have in their sexual relationships are not the result of specific sexual dysfunctions, but are rather the result of poor communication. It is very common for couples who otherwise have very intimate, close relationships to never openly discuss sex - their likes and dislikes, etc.
For example, just because a woman does not frequently experience orgasm when making love with a partner does not mean that she suffers from a sexual dysfunction. It may be that her partner is simply unaware of how to bring her to orgasm. In this case some honest, frank discussion about sex can greatly improve the quality of a sexual relationship. However many people do suffer from a specific sexual dysfunction.

This can refer to an inability to perform or reach an orgasm, painful sexual intercourse, a strong repulsion of sexual activity, or an exaggerated sexual response cycle or sexual interest. A medical cause must be ruled out prior to making any sexual dysfunction diagnosis and the symptoms must be hindering the person's everyday functioning.

SEXUAL DYSFUNCTION OBJECTIVES: To acquaint the reader with:
  • female sexual dysfunctions male sexual dysfunctions dysfunction of both males and females likely causes of the various dysfunctions some case examples
Labeling the various dysfunctions has been an ongoing debate in the field of sexology. For the purpose of this text I refer to the major female dysfunctions as: Female Sexual Arousal Disorder B>; Female Orgasmic Disorder ; and Vaginismus . I refer to the major male dysfunctions as: Erectile Dysfunction Male Orgasmic Disorder ; and Premature Ejaculation . Additionally, Inhibited Sexual Desire can be experienced by either males or females. Although it may simply be a matter of semantics, labels are important and often have a psychological impact. For example, Female Sexual Arousal Disorder and Female Orgasmic Disorder are often lumped together and referred to as frigidity. Not only is this term pejorative, implying that the woman is lacking warmth, it also does not describe the dysfunction. Another example of the importance of a label is that most people refer to erectile dysfunction as impotence. Here again an pejorative imprecise term implying that if the man is unable to have an erection, he is without power. Parkish Kothari (1987) has taken this another step. He believes that Male Orgasmic Disorder should be called "delayed orgasmic response," and premature ejaculation should be called "early orgasmic response." By doing this the focus is not on the reproductive aspects but rather on the pleasure aspects of sexuality.

Articles about the Treatment of Sexual Dysfunctions
The first article published here: The Treatment of Vaginismus (difficult penetration) using Perry brand (inserted) pelvic muscle sensors. When women are given control over the timing and action, problems disappear! Well, it's a little more complicated than that. Just added, The Functions of Erection Reconsidered , a 1982 SSSS convention speech by Dr. John Perry, puts forth the radical idea that the erection serves to connect the sensitive organs of each sex (The G spot in the female and the prostate in the male) during intercourse. Clinical Guide Series #3, Impotence: The Next Frontier , should be especially of interest to those with patients suffering post-prostatectomy incontinence, since it suggests that the same biofeedback-assisted pelvic muscle exercises that can restore continence may also restore erection, based on a Belgian study.
The Gräfenberg Spot (The "G Spot")
Dr. Perry's research leading to the discovery of the Gräfenberg Spot is described in the best-selling book, The G Spot (1982), written with Alice Ladas and Beverly Whipple.

Common sexual dysfunctions. The following list, while far from exhaustive, describescommon sexual dysfunctions, their possible causes, and treatments.

Common Sexual Dysfunctions
In diagnosis and history taking, it is important to remember that each dysfunction covered in this course can be lifelong (has always been present), acquired (has not always been present), situational (occurs in some situations and not others), or generalized (occurs regardless of the situation). The following list, while far from exhaustive, describes common sexual dysfunctions, their possible causes, and treatments. Note:
Inhibited sexual desire (ISD)
Sexual desire changes over the course of our lives, and occasional loss of desire in either sex is not uncommon. In ISD, however, there is persistent loss of desire that disrupts sexual relationships. It is characterized by diminished sexual attraction, decreased sexual activity, few or no sexual dreams or fantasies, and diminished attention to erotic material by one or both partners. ISD is the most common presenting sexual dysfunction in women and is less commonly reported in men. Female sexual arousal disorder (FSAD) is the name for persistent or recurrent inability to achieve or maintain an adequate lubrication-swelling response. (A woman with FSAD may or may not find enjoyment in physical contact.)

Sexual Dysfunctions, Causes, and Treatments
There are a number of dysfunctions that can occur in the expression of sexuality. This sheet provides basic information on common conditions.
Loss of desire
Loss of desire affects both men and women, and is characterized by infrequent sexual activity or lack of desire, including few or no sexual dreams or fantasies. Physical factors that contribute to loss of desire or inhibited sexual desire (ISD) include hormone deficiencies, depression, alcoholism, kidney failure, and chronic illness. Psychological factors that contribute include stress, relationship problems, sexual trauma, major life changes, and pairing negative memories with sexual interactions. Treatment is based on the causes of the loss, which may be physical, medical, or psychological. If testosterone deficiency is the cause, replacement therapy is indicated.
Dyspareunia
Dyspareunia is a condition in women characterized by recurrent genital pain with sexual activity (usually with vaginal penetration but can occur during nonpenetrative genital stimulation). Symptoms of dyspareunia include burning, itching, stinging, and feeling inflamed, in any area of the perineum. The main causes are vulvovaginitis, genital herpes, atrophic vulvitis, urethral problems, episiotomy, radiation vaginitis, and sexual trauma, as well as inadequate lubrication and topical irritants such as spermicides or latex. Deep pelvic pain is often associated with thrusting by the partner hitting an ovary during sexual intercourse, pelvic inflammatory disease (PID), pelvic or abdominal surgery, postoperative adhesions, endometriosis, genital or pelvic tumors, irritable bowel syndrome, urinary tract infection, and ovarian cysts. Dyspareunia can also have psychological causes.
