What is shingles?
Each year, an estimated one million Americans are afflicted with herpes zoster , a painful viral infection commonly called shingles, which is caused by the chicken pox virus. Shingles can develop in anyone who has had chicken pox. More than 1 of every 10 people who had chicken pox as children get shingles as adults, usually over age 60. The chicken pox virus remains dormant or inactive in nerve root cells of the body. In some people, the virus "awakens" within the nervous system to cause shingles. Shingles may include a blistering rash and severe burning pain, tingling or extreme sensitivity to the skin, usually limited to one side of the body and lasting about a month. What causes shingles?
Varicella zoster
virus, the same virus that causes chicken pox, causes shingles. This virus is in the herpes family. Shingles is also called herpes zoster. Shingles is not a new infection; rather, it is a second outbreak of the chicken pox virus. Some of the virus germs that cause chicken pox stay in the body, remaining inactive in the nerve cells near the spine for many years. Then the herpes zoster virus suddenly wakes up from the dormant state and grows. Once active, the germs travel along the nerve paths to the skin, leaving a path of destruction along the nerves in which they travel. The result is the pain and rash of shingles.

Chickenpox (varicella) and shingles (herpes zoster) are distinct diseases caused by the varicella-zoster virus (VZV). The VZV is a herpesvirus. Chickenpox is a highly contagious rash that usually occurs in childhood. It is the manifestation of a primary infection. Shingles is usually a localized, unilateral (occurs on one side of the body) eruption that follows a linear pattern along a nerve. Shingles is a reactivation of a dormant varicella-zoster virus. Causes Approximately 90% of chickenpox cases occur in children under 10 years of age. The infection is usually transmitted by airborne droplets containing the virus. Less often, the virus is spread by direct contact with the blisters. After exposure, the typical incubation period is 2 weeks. One is contagious a couple of days before the rash appears. Rarely, chickenpox is transmitted through the blood of an infected mother to a fetus. Shingles cannot be acquired from another person. Shingles is caused by reactivation of the dormant virus. While shingles can occur at any age, more than two-thirds of cases occur in people over 50 years of age. Shingles is 20 to 100 times more common in immune compromised individuals, such as HIV patients and the elderly, as well as those whose immune systems are suppressed by medication or chemotherapy. An individual who has never had chickenpox could develop the infection by coming into contact with shingles blisters.

    Shingles What is shingles, and who can get it?
    Shingles is another name for a condition called "herpes zoster." It causes a painful rash. You can only get shingles if you had chickenpox in the past. After you have chickenpox (usually as a child), the virus that causes it stays in your body in certain nerve cells. Most of the time your immune system keeps the chickenpox virus in these cells. As you get older, or if your immune system gets weak, the chickenpox virus may escape from the nerve cells and cause shingles. Most people who get shingles are more than 50 years old or have a weak immune system. For example, you might get shingles if you have cancer, take medicines that weaken your immune system or have the virus that causes AIDS (acquired immunodeficiency syndrome). What are the symptoms of shingles? Shingles causes a painful, blistering rash. Sometimes the pain starts a few days before the rash appears. The rash begins with reddish bumps. In a few days, these bumps turn into blisters. You might feel a stinging or burning pain. The rash may wrap around your back and chest, or it may be on one side of your face. The blisters usually crust over and fall off after 7 to 10 days. You may see changes in the color of your skin when the scabs fall off. In bad cases of shingles, these color changes last forever.

    Famciclovir is used to treat shingles (herpes zoster) as well as herpes simplex virus infections which can cause painful blisters and lesions. Famciclovir reduces the severity of outbreaks and prevents the development of new blisters. Famciclovir has also been shown to be effective for preventing genital herpes outbreaks. Managing side effects
    Famciclovir has few side effects. The main side effects seen in studies were headache and nausea. Dosage
    For shingles the recommended dosage is 500 mg every 8 hours for 7 days. For genital herpes simplex infections, the recommended dose is 125 mg twice a day for 5 days. The dosage used to prevent genital herpes outbreaks is 500 mg twice a day. In people with reduced kidney function, the dosage should be reduced. The amount of dose reduction is dependent on how well the kidney is functioning. How long it may take to work
    Famciclovir is usually given for 7 days to treat shingles and 5 days to treat genital herpes simplex infections. Managing side effects and drug interactions
    Other drugs such as probenecid that are processed through the kidneys may increase the amount of famciclovir in the body. This might increase the chances of getting side effects from famciclovir.
