What is cancer
Our bodies are made up of tiny structures called cells, which are not visible to the naked eye. Groups of cells form the tissues and organs of the body and each of these, such as brain, liver, kidneys and lungs, has a very particular function. Normally cells reproduce themselves by dividing in a regular orderly fashion so that growth and repair of the body tissues can take place. Disease can disrupt this normal function in various ways. Sometimes there is an uncontrolled growth of cells causing a swelling or a tumour. Benign tumours remain contained within a limited area and once treated, often by an operation, don't usually cause any further problems. Cancers or malignant tumours can spread to nearby organs or tissues. Cancer cells may be carried in the bloodstream to distant sites in the body where they may form new tumours called

Testicular Cancer: What to Look For What is cancer?
Cancer cells are cells that have mutated (changed) and grow out of control. They keep on growing and dividing so that too many cells are produced. These cells clump together to form tumors. If these tumors are malignant, they can invade and kill your body's healthy tissues. From these tumors, cancer cells can metastasize (spread) and form new tumors in other parts of the body. (Tumors that are not cancerous and don't spread are called "benign tumors.")
Male reproductive system Who gets testicular cancer? Cancer of the testicles is the most common cancer in young men (15 to 34 years old). Men who are more likely to get testicular cancer:
  • Are white
  • Have a father or brother who had testicular cancer
  • Have a testicle that did not come down into the scrotum (the skin "sack" that holds the testicles), even if surgery was done to remove the testicle or bring it down
  • Have small testicles or testicles that aren't shaped normally (most testicles are round, smooth and firm)
  • Have Klinefelter's syndrome (a genetic condition)

Cancer of the testis can affect men of any age, but it is primarily a cancer of younger men. The cure rate is extremely good and most men can go on to father children. What are the symptoms of testicular cancer? The most common symptom is an enlarged, hard, painless testicle. Among the other possible signs are swelling or a feeling of heaviness in the groin area or scrotum. If you notice any of these symptoms, see a doctor as soon as possible. They do not necessarily mean cancer, but only a doctor can tell. Almost all testicular tumors occur in cells that produce sperm, known as germ cells. However, it's one of the most common cancers found in young males. Men who had one or both testicles fail to descend from the abdominal cavity into the scrotum prior to birth, and men with a family history of inguinal hernia during childhood or testicular inflammation in conjunction with the mumps, are at greatest risk for developing this cancer. Although there's no way to prevent testicular cancer, monthly self-examinations can detect this cancer early so it can be successfully treated. SYMPTOMS a firm, painless lump in one of the testicles a feeling of heaviness or hardness in a testicle testicle pain breast growth or nipple tenderness swollen lymph nodes abdominal or back pain fatigue weight loss DIAGNOSIS AND TREATMENT FOR TESTICULAR CANCER To determine if the lump or swelling is cancerous, a biopsy is done. A small sample of tissue is removed and examined. If cancer is present, surgical removal of the diseased testicle is the treatment. Since the testicles are not connected by lymphatic tissue, the cancer is unlikely to spread from one testicle to another. Surgery will only remove one testicle, most often leaving fertility and potency intact. Radiation therapy, chemotherapy and removal of nearby lymph nodes may also be done to further treat the cancer.

The origin and nature of scrotal masses must be determined accurately because most testicular masses are malignant and extratesticular masses benign.
Pathology
Malignant testicular tumors arise from the primordial germ cell and differentiate to reveal seminoma, teratoma, embryonal carcinoma, endodermal sinus tumor (yolk sac tumor), and choriocarcinoma. Histologic combinations are common, eg, teratocarcinoma contains teratoma plus embryonal carcinoma. Functional interstitial cell carcinomas of the testis are rare. Tumors arising in the epididymis, testicular appendages, and spermatic cord usually are benign fibromas, fibroadenomas, adenomatoid tumors, and lipomas. Sarcomas, most commonly rhabdomyosarcoma, occur occasionally, mostly in children.

  • Testicular cancer is very rare and is seen in less than 1% of the population . Usually it is seen in young men.
Signs and Symptoms
  • A lump or swelling in one of the testicles.
  • Possible local pain, tenderness, or lower back pain.
  • Sometimes a feeling of heaviness in a testicle.
Causes
  • Doctors still do not know what causes testicular cancer.
  • It occurs most frequently in young men between the ages of 15 and 35.
  • Men whose testicles have not descended or dropped have a greater risk of getting testicular cancer.
Treatment And Self-helps
  • The best way to identify testicular cancer is self examination done monthly after showering
  • If, during the self examination, you feel a hard lump on the testicle, see your doctor.
When Do I Call The Doctor?

Facts about Testicular Cancer What is Cancer? Cancer describes a range of diseases that affect the cells that make up the tissues and organs in the body. Cells normally grow and divide in a controlled way, but sometimes, this process gets out of control and the cells continue to divide to form an abnormal growth or lump called a tumour. Tumours can be benign or malignant. A benign tumour is not cancerous as the abnormal cells do not invade other tissues or spread to other parts of the body. Benign tumours may cause problems to nearby healthy organs by pressing on them and affecting their normal function if they continue to grow. A malignant tumour is cancerous as cells continue to grow in an uncontrolled way and have the ability to invade nearby tissues and can spread to other parts of the body through the bloodstream or lymphatic system. If cells reach a new site they may continue to divide and form a secondary tumour (sometimes referred to as a metastasis).

But it hasn't been an easy road. You might say Rob went on a wild goose chase before he found Cancer Treatment Centers of America (CTCA). "When I first got cancer, I had surgery to remove one of my testicles," says Rob. "About a year and a half later, my tumor makers were elevated, but the doctors couldn't find a tumor. So then I did chemotherapy for four months." "The first few treatments at home were great," says Rob. Then the insurance company told him the home-care treatments were not covered, and he had to get his chemotherapy at a hospital.

Surgical Oncology Radiation Therapy ... Treatment Options Testicular cancer is one of the most common cancers in young men between the ages of 15 and 35, but the disease also occurs in other age groups, so all men should be aware of its symptoms. While testicular cancer is common among young men, young men typically do not develop any kind of cancer, so overall, testicular cancer is a relatively rare disease. This form of cancer is almost always curable if it is found early. Most testicular cancers are found by men themselves, either as a painless lump, a hardening or change in size of the testicle, or pain in the testicle. Children born with an undescended testicle have an increased risk of getting testicular cancer, regardless of whether surgery is done to correct the problem. However, the surgery should still be done to preserve fertility. The theory is that the increased risk of testicular cancer is due to whatever caused the undescended testicle, not just by the fact that the testicle was undescended. In the year 2000, an estimated 7,600 cases of testicular cancer were diagnosed in the United States. Caucasians are more likely to get it than Hispanics, and much more likely to get it than African Americans or Asians. The incidence of testicular cancer is highest in Denmark, and lowest in the Far East. Disturbingly, the incidence of testicular cancer around the world has basically doubled in the past 30-40 years. However, the cure rate in testicular cancer has risen dramatically over that same time period. In 1970, ninety percent of men with metastatic testicular cancer died of the disease. By 1990, with the introduction of chemotherapy drugs the ratio is reversed, with about ninety percent of men who have metastatic testicular cancer surviving.
