POLICY ON PROPHYLAXIS AGAINST THROMBOEMBOLISM
ROYAL INFIRMARY OF EDINBURGH NHS TRUST DIRECTORATE OF ORTHOPAEDIC SURGERY Assess each patient for risk of thromboembolic disease according to chart below.
  • Prescribe prophylaxis according to patients level of risk. Record level of risk and prophylactic measures taken in patients case notes. It is expected that all normal prophylactic measures will be applied to all patients. These include: appropriate hydration, early mobilisation, physiotherapy and avoidance of local calf pressure. Specific prophylaxis should commence as soon as practicable after completion of surgery. Relative contra-indications to pharmacological anticoagulation with heparin in this unit include :
    • recent CNS or eye surgery Pre-existing haemorrhagic state,
      • e.g. liver disease, renal failure, haemophilia thrombocytopaenia.
        • pre-existing structural lesions,
          • e.g. peptic ulcer, recent cerebral haemorrhage, uncontrolled hypertension.
            • dementia With exception of patients over 80 years, prophylaxis should continue until patient is fully mobile or discharged from the unit. In patients over the age of 80 years, pharmacological measures should not exceed 5 days unless otherwise instructed.

Deep vein thrombosis is the clotting of the blood in vein. The most common site of a deep vein thrombosis is in a deep leg vein. Complications occur when a clot breaks away from the wall of the vein to which it is attached and travels through the blood stream to the heart or lungs. Although anyone can develop blood clots, certain people are at greater risk including pregnant women, people who are overweight or obese, and those with certain blood conditions. The Hemophilia and Thrombosis Center of Nevada (HTCN) was founded in 1997 as the first treatment center in the state of Nevada dedicated to the diagnosis and treatment of patients with bleeding or clotting disorders. The HTCN is a non profit corporation. We support ourselves with educational grants and research grants. All team members are employed through community organizations who donate their time to the treatment center. We currently care for patients and families throughout Nevada and the neighboring underserved regions. While clinics are held in Las Vegas, we attempt to coordinate care through the primary care physician's office locally. We have an active advisory board composed only of affected and concerned persons. This board promotes the mission of the HTCN and helps to guide the activities of the treatment center staff. If you are interested in volunteering at the HTCN or helping support us, please call the center at the number below. We always need help with our newsletters published three times per year and special events.
Our Mission
The HTCN is dedicated to improving the care of patients with bleeding or clotting disorders regardless of the ability to pay.

What the Researchers say:
Recently there have been many reports relating to incidents of thrombosis (or blood clots) occurring in deep veins of the legs of otherwise healthy people who had recently flown long distances. Such reports referred to this problem as "Economy Class Syndrome", but in fact, the reports showed that this medical condition also occurred in passengers who had travelled in "Business Class". A number of detailed studies were conducted bringing together all the incidents reported and looking at a large group of patients suffering from Thrombosis in the deep veins of the leg. These studies were unable to establish flying as a contributory cause of this type of Thrombosis. Then in May 2001 the results of the first objective study were published in Britain’s premier Medical Journal "The Lancet" . A group of researchers from The Middlesex Hospital, London headed by Vascular Surgeon, Thrombosis Expert and Aviator John Scurr and Haematologist Professor Sam Machin examined the veins and blood from 231 volunteers before they embarked on long haul flights (more than 8 hours). On their return to London they were again examined and it was found that in one group of 116 volunteers 10% of the passengers had a thrombosis in the deep veins of their legs. These clots were detected by ultra sound examination and the patients received immediate treatment without which the condition may have become more serious. However, a single mild incidence of deep vein thrombosis can predispose to further incidents of a more serious nature.
