"At least 1 in 10 women can expect to suffer from some sort of thyroid disorder during her lifetime. Whether thyroid disorders strike at puberty, during peak reproductive years, during or after her pregnancy, around menopause, or after age sixty, a woman's body is uniquely and dramatically affected." For women affected with a thyroid disorder, these statistics probably come as no surprise, often they have a family history that may include a variety of thyroid disorders. The Thyroid Sourcebook for Women by M. Sara Rosenthal is a must read for women who have a diagnosed thyroid disorder. As the author, herself a thyroid patient, states in the introduction, "...what Dr. Purdy points out is no different than what women have been saying to me for years about their experiences with thyroid disease: many are ignored or misdiagnosed with stress-related or psychiatric disorders; many are unable to obtain adequate information from their doctors about thyroid disease; many are abused and infantilized (i.e., treated like children) by their physicians." For this reason, Rosenthal has written a book that sets out to be a thorough guide to understanding the thyroid's function, it's unique relationship to the female hormone and immune systems, and they ways in which thyroid disorders manifest themselves in women.

WHAT IS IT?

Your thyroid gland sits in your neck just below the Adam's apple. It makes hormones that regulate the pace of body functions, such as your heart rhythm and the rate at which your body metabolizes food and produces cholesterol. The thyroid affects organs and tissues throughout the body. Normally, the thyroid helps to keep your body functioning the way it should. But sometimes, thyroid activity can go awry. Hypothyroidism, or underactive thyroid, causes the body processes to slow, and may make you feel sluggish. It affects as many as 17 percent of women and 9 percent of men by age 60. Hyperthyroidism, or overactive thyroid, causes you to feel like you're revved up, both mentally and physically. It affects less than 1 percent of Americans. If left untreated, both hypothyroidism and hyperthyroidism can lead to problems with the cardiovascular system, reproductive system, and major organs. But if treated, most people with these conditions do well and are able to live normal, active lives. Thyroiditis, or inflammation of the thyroid, affects roughly 5 percent of women who have recently given birth. This problem is called postpartum thyroiditis, and usually clears up by itself in a few months, though treatment can speed recovery. Thyroiditis is not always related to pregnancy. Some people get it after a viral infection, for example. Thyroiditis can cause either hypothyroidism or hyperthyroidism.

About 200 million people in the world have some form of thyroid disease. Yet before the founding of the Thyroid Foundation of Canada in 1980, no lay organization existed in North America to promote public education about thyroid disease and to promote public support of thyroid research. Thyroid disorders for the most part are treatable; however, untreated thyroid disease can produce serious results in other parts of the body. Improved public awareness and understanding of thyroid disorders will enable patients and their families to cope more effectively with the sometimes disturbing course of thyroid illness. In this way individuals will also be better equipped to play a role in alerting their physicians to a suspected thyroid condition that may otherwise be difficult to diagnose in the sometimes slowly developing initial phases. It is estimated that 13 million American have some sort of thyroid disorder, yet more than half remain undiagnosed. Women are five to eight times more likely than men to suffer from a thyroid disorder. Since thyroid disorders are a major women's health issue, AMWA has called for action by developing a consumer education campaign. The initiative, Gland Central: Your Thyroid, is aimed at increasing public awareness of thyroid disorders. Since its inception, Gland Central: Your Thyroid has succeeded in alerting the pubic to this problem by screening over 8,000 people in 29 cities throughout the United States, which resulted in a 20 percent awareness of thyroid disorders. The project continues to strive to alert the public to the thyroid gland's central role in the body and teaches consumers to recognize early signs and symptoms of thyroid disorders. It encourages consumers to get a simple test for thyroid dysfunction. Gland Central: Your Thyroid was awarded the 1998 Silver Anvil Award by the Public Relations Society of America (National award), and the Big Apple Award form the Public Relations Society of America (Regional - New York award).

Your internal medicine office is overflowing with Friday afternoon appointments. First up is Mrs. Slowauk, a 56 year old lady who came to you last week with vague symptoms of fatigue and depression. Her biochemical panel is back on the chart and is normal. Her thyroid status is another matter. You are glad you ordered that TSH level on her last week. What value does TSH testing have over the older thyroxine (T4) based assays?
Answer:
Thyroxin based assays can be increased or decreased by thyroxine binding globulins and other proteins which can be altered in organ failure or hormone usage. Recall that only about 0.02% of the total plasma T4 (and 0.3% of the plasma T3) is not protein bound. Thyroid hormone cannot be biologically active when bound.

Description The thyroid is a gland located in the neck. The thyroid gland produces hormones which help to control temperature, growth, behavior, blood pressure and heart rate. The thyroid gland can become too active ( hyperthyroidism ) or not active enough ( hypothyroidism ). Treatments for problems with the thyroid include medication, surgery or radiation. Thyroid Problems and Pregnancy Pregnant women and their unborn children generally tolerate mild and moderate thyroid disorders without significant health problems. Infertility is common with severe thyroid disorders. Some treatments for thyroid disease cannot be used in pregnancy because of risk of harm to the developing foetus.

Thyroid Underactivity (Hypothyroidism)
Underactivity of the thyroid gland (hypothyroidism) can arise for a number of reasons. Hypothyroidism can occur in newborn infants (Congenital Hypothyroidism, CHT) as well as later on in childhood (acquired Hypothyroidism, AHT.) Affected newborns look, and usually act, normal for the first few days and the national screening programme for CHT, by blood spot collection, ensures that affected infants are identified and treated as early as possible. The commonest cause of CHT is a poorly developed or absent thyroid gland; the underlying basis to this abnormality is not understood. CHT can also occur if there is a defect in thyroxine production within the gland. These defects may be permanent or transient. Although every child with CHT should be started on thyroxine promptly, they should be evaluated at some time later to exclude a transient problem. The commonest causes of acquired hypothyroidism include an immune reaction against the thyroid gland by the body itself (Autoimmune Hypothyroidism, Hashimoto's Disease) or damage to the thyroid gland or the part of the brain which controls the thyroid.
