Arizona Seeing More Cases of Valley Fever ASSOCIATED PRESS March 1, 2002 The number of cases reported to the state Department of Health Services has risen to 1,917 in 2000 from 190 a decade ago. Valley fever, a regional disease caused by mold spores in the ground, can cause symptoms ranging from mild flu-like feelings to severe illness and even death in especially vulnerable victims. The largest spike came in November and December last year when 383 and 531 cases were reported, respectively. That compares with an average 139 cases per month for the first 10 months of the year. "The honest answer is, we don't know why," said Dr. John Galgiani, director of the Valley Fever Center for Excellence at the University of Arizona in Tucson. "The population is growing, but the percentage of increased cases is greater than the growth in residents." The center, the only one in the country that studies valley fever, is working on a vaccine and better treatment for the worst cases. Researchers believe 60,000 Arizona residents are infected with the soil fungus, Coccidioides immitis, each year. But only a third of those, or 20,000 people, become ill enough to miss work or school and seek medical attention.

Nature of the disease Rift Valley fever (RVF) is an acute insect-borne virus (arbovirus) zoonotic disease caused by a Bunyaviridae of the gens Phlebovirus. It mainly affects ruminants and humans. In ruminants it causes abortion in pregnant animals and high mortality in young animals.
In humans RVF causes a severe influenza-like disease with occasionally more serious complications Classification OIE List A disease, high level of biosecurity Susceptible species RVF virus is highly pathogenic for sheep and cattle, buffalo camels, mice, rats and humans. Goats appear to be moderately susceptible and pigs, horses, carnivores and poultry have only transitory viremia. Distribution To date RVF is limited to some part of Africa, it is present in Eastern Africa and recently extended to Madagascar and Western African countries (up to Senegal) probably in relation with El Nino climatic phenomena. Clinical signs Clinical disease occurs in animals of all ages but is most severe in young. The incubation period is very short (12 hours) and within 36 hours m ortality can reach 95% in lambs and 70% in calves.

The article in the Ventura County Star linked the disease ( coccidioidomycosis ) to a dust cloud that resulted from the January 1994, 6.7 magnitude Northridge earthquake. Spores of a fungus occurring naturally in local soil were carried in a dust cloud over Simi Valley where it caused a miniature epidemic, according to Ventura County Public Health Officer, Dr. Gary Feldman.
Valley fever accounted for four deaths out of 52 reported cases in 1993 and at least five deaths since the 1994 earthquake. And while Feldman does not consider it a public health threat, he states that there might be more people who have the disease who don't know it because of its flu-like symptoms.
"Actually, I rather enjoy being the angel of government; good benefits, liberal vacation and sick leave policy and an excellent retirement. It's just that I thought I'd be on top of the cloud where I could see— not stuck here inside."
Clearly, valley fever presents a concern, if not a threat, whenever soil dust enters the human environment, as it invariably does with construction projects, unpaved roads and highway shoulders, vacant lots and fields and agricultural operations. But for those of us who live in or around its environs, or are still caught up in quaint techno-fascination, the quest to understand the Valley continues. Never mind that the exercise now sometimes seems more like picking a scab than uncovering the ineluctable Future of Capitalism. The itch must be scratched; the Valley is still there, even if Excite and Webvan are not." Last week Salon's Andrew Leonard reviewed two books about SV: one a noir mystery and the other an academic study. I'm not sure either of them can be as juicy as Leonard's lead. She also mentions that in order to have had "Valley Fever" he must have grown up in the San Joaquin Valley in California. Dr. Kovac expresses regret at having killed a "farm boy" and then the camera pans to the body on the table. I am sure that I saw the actor on the table smirk at this point. The actor that played the attacker is named Derek Mears. I know for a fact that he actually was from the San Joaquin Valley (Bakersfield) because he was one of my best friends while growing up there. There is nothing that is very distinctive about a valley fever infection. It may feel like a viral flu episode. During the acute illness it may be possible to culture the fungus from the sputum. Chest X-rays may show a pneumonia that looks similar to a bacterial pneumonia. As mentioned above, for most people with normal immunity, no anti-fungal treatment is usually required. If valley fever is suspected as the cause of the illness, blood tests may be done, which demonstrate antibodies to the fungus and confirm a diagnosis of valley fever. As a general rule, anti-fungal medicines are only used when it appears that the infection will overwhelm the immune defense of the patient. Traditionally the most potent anti-fungal has been a drug that is given by IV, called amphotericin. This drug has many side effects, especially involving the kidney. Several anti-fungals in pill form are now available. They are not as effective as antibiotics are in treating bacterial infections, but can be effective in controlling the fungal infection.

Rift Valley fever: the background
Red Cross officials say the outbreak of deadly Rift Valley Fever in Kenya has now spread from the north east to parts of the coast and central provinces.They say the numbers who have died from the disease may be double the government figure of 350 since late last year. Graham Easton of BBC Science has this report on the outbreak, and the nature of the disease.
Experts initially thought the outbreak in Kenya was due to anthrax, but now scientists from the World Health Organisation have found the Rift Valley Fever virus in samples from several human victims of the mysterious outbreak. Rift Valley fever is a viral disease spread by mosquitoes. It's primarily an illness of livestock such as sheep, goats and camels. During epidemics in livestock the virus causes spontaneous abortions and deaths in young animals. But the disease can have devastating effects on humans too.
Symptoms include fever, vomiting and often severe bleeding After an incubation period of about 3-6 days, the early symptoms in humans are fever and headache which can progress to bleeding from the gums and nose, and sometimes a fatal coma. Rarely the infection results in damage to the eyes, inflammation of the brain, and, as in this latest outbreak, severe bleeding.

More than 70 people are reported to have died in Yemen over the last two days from a suspected outbreak of Rift Valley Fever. Yemen's Health Minister, Abdelwali Nasher, said teams of doctors had been rushed to the affected area - a remote part of Yemen near the Saudi border. About half of all livestock in the Wadi Moor area in Al-Hudaydah province are said to have died.
The disease is spread by mosquitoes
Yemen has appealed to the World Health Organisation and the US Centre for Disease Control in Atlanta for help. Mr Nasher told the BBC that at least 17 people had died from the fever, but another Yemeni Health Ministry official put the death toll as high as 77. Insecticide Mr Nasher said Saudi Arabia would soon be sending planes across the border to spray insecticide to kill the mosquitoes which are thought to be spreading the disease. At least 16 people died from Rift Valley Fever in Saudi Arabia last week, in the first outbreak of the viral disease outside Africa.

In fact, the high rate of infection during that time has made many livestock animals immune to the virus and is contributing to the low risk of infection at this time, the agencies report. "Remote sensing satellite data of climatic conditions fully support ground observations that conditions in the Horn of Africa countries are highly unfavorable for multiplication of mosquito vectors of the Rift Valley fever virus. Therefore, the risk of a Rift Valley fever epidemic occurring soon is virtually nil," the two agencies said in a statement. The suffering is not limited to humans. Like people, dogs are very susceptible to Valley Fever. Dogs primarily contract Valley Fever in the low desert regions of Arizona, New Mexico, Southwestern Texas, Nevada and parts of California, such as the Central Valley, Bakersfield, and San Diego County. Dogs contract Valley Fever by inhaling spores distributed by wind and construction, digging in the dirt, or uprooting plants. Soil that is being trenched, graded, or bulldozed increases the risk of spores in the air. Noticing the symptoms is the key to saving your dogs life. It can start out in different forms. One being that it involves the respiratory tract/pulmonary system (lungs and airways) or cutaneous (pertaining to the skin). The Pulmonary form may be a mild infection with symptoms typical of a common cold. Coughing, lack of appetite, etc. If the primary form goes unnoticed and untreated the infection can continue to develop and disseminate to other areas, such as bones, joints, nervous system, kidney and liver.

Coccidioidomycosis (Valley Fever)
What is coccidioidomycosis?
Coccidioidomycosis is a disease caused by a fungus (mold). The fungus lives in the soil of the central valley of California and the deserts of the southwestern US and northwestern Mexico. It usually affects the lungs but can affect other parts of the body as well. It is commonly called cocci or valley fever.
How does it occur?
You can get infected by breathing in the fungus while you are camping, working, or digging in the desert. The fungus can also infect you when it is picked up by desert winds and blown into the area where you live. It cannot be passed from one person to another or through contact with an infected animal.
What are the symptoms?

Sunday, November 10, 2002 Secession lost heavily in the city and maintaining a 4,000 vote majority in the San Fernando Valley. But secession's legacy is important to this city. Already, the issues raised by secessionists are becoming top priorities for the city. Boroughs are getting a revival. Businesses too are benefiting from the attention paid by cityhood advocates on the city's jobkilling regulations. As for the candidates who ran for Valley offices, many of them are heartened by the efforts so much so that they are running again, this time for the Los Angeles City Council. The winners of Valley offices are not retreating from the good fight, they have formed the "Collective" to press on for reforms.
More importantly, the independence movement forced people all over to pay attention to the Valley's unique culture. Before, what the country knew about the Valley was filtered through Hollywood's lens, that of a bland suburbia peopled with vapid white girls and their obnoxious dialect. Now, the ethnic diversity, the slumminess, its relationship with Los Angeles and its pastoral roots are what the country is seeing. I prefer the nuanced gritty picture to the plastic fantastic vision pitched by developers. I've always felt secession was a radical plan but it was the only solution offered and not a bad one at that. I can't pass on any judgment on whether the voters' decision was a disappointing colossal mistake until I see the sort of city Los Angeles will fashion itself as come the next city council elections in April.
