Feline vestibular disorders. Part II: diagnostic approach and differential diagnosis
K M Vernau, R A LeCouteur
Results of a neurological examination usually permit localisation of a vestibular disorder to either the central or peripheral parts of the vestibular system. Many different disorders located in the same part of the vestibular system will produce similar clinical signs. Therefore, additional diagnostic tests beyond a neurological examination are required in order to make an accurate diagnosis. The objectives of this review are to outline a diagnostic approach for disorders affecting the feline vestibular system, and to summarise the clinically important features of frequently diagnosed diseases affecting the vestibular
system of cats. Return to Table of Contents Return to INNO-VET Index Page

Symptoms
The most frequently reported symptoms of vestibular disorders are dizziness, unsteadiness or imbalance when walking, vertigo, and nausea. These symptoms may be quite mild, lasting minutes, or quite severe, resulting in total disability. Because the vestibular system interacts with many other parts of the nervous system, symptoms may also be experienced as problems with vision, muscles, and thinking, and memory. In addition, people with vestibular disorders may suffer headache and muscular aches in the neck and back, increased tendency to suffer from motion sickness, and increased sensitivity to noise and bright lights. Patients with vestibular disorders often report fatigue and loss of stamina and an inability to concentrate. Difficulty with reading and speech may occur during times of fatigue. When these symptoms are constant and disabling, they may be accompanied by irritability, loss of self-esteem, and/or depression.

http://www.balance-central.com/adisord.html
Unilateral Vestibular Dysfunction
This is a weakness on side of the vestibular system. Symptoms you may suffer from could be unbalance and or dizziness when turning your head. In early stages there can be vertigo and the sensation of spinning. Vestibular rehabilitation can provide significant benefits to patients with these symptoms.
Bilateral Vestibular Dysfunction
A weakness on both sides of the vestibular system. A person with his disorder may suffer imbalance and or dizziness when turning the head. They may also suffer oscillopsia, or the illusion that things are bouncing up and down with movement. There can be significant benefit to patients with this disorder by using vestibular therapy. Balance exercises, and compensatory techniques.
Benign Paroxysmal Position Vertigo (BVPV)
This disorder is caused when some very small crystals of calcium carbonate, located in the inner ear, become dislodged. When a person lies down, these crystals can float down into one of the three canals that are located in the inner ear. The person then stands up, the crystals can move furthest into the canals, and become trapped. If the person moves in certain ways the crystals can move, giving the person a sensation of spinning repeatedly. BPPVs most common symptom is spinning (Vertigo) shortly after lying down or rolling over in bed. The symptoms usually last only a few seconds to a minute, but can actually wake a person up at night. The symptoms can occur with other head movements and cause brief periods of severe imbalance.

There are several types of vestibular disorders which may be helped by Vestibular Rehabilitation:
The first type is Unilateral Vestibular Dysfunction or weakness on one side of the vestibular system. A person with this type of a vestibular disorder may suffer imbalance, wooziness when turning the head, and, in the early stages, vertigo or the sensation of spinning. The second type of vestibular disorder is Bilateral Vestibular Dysfunction or weakness on both sides of the vestibular system. A person with a bilateral vestibular dysfunction may suffer imbalance, wooziness when turning the head, and also may suffer oscillopsia, or the illusion that things are bouncing up and down with movement. The third type of vestibular disorder is Benign Paroxysmal Positional Vertigo , better known as BPPV Canalith Repositioning Maneuver and is a series of head positionings done by the physical therapist, audiologist, or physician to clear the crystals from the canal and deposit them back into the part of the inner ear called the utricle , where they belong. The procedure is painless, although the patient may need to endure some dizziness, and can be done right in the office in less than one-half hour.
