DEAR DOCTORS, DRUG COMPANIES AND RESEARCHERS: Vulvodynia is a painful chronic vulvar disease with enormous discomfort and pain. Complaints range from burning, stinging, swelling, itching, irritation and rawness of the female genitalia however Vulvodynia pain is highly individualized. The condition varies in persistence and location. Pain may be constant or intermittent, localized or diffuse. In many cases of Vulvodynia, pain occurs spontaneously. Upon examination, the vulvar tissue may appear inflamed and swollen or it may look perfectly normal. It is common for a woman to consult up to 12 doctors before obtaining an accurate diagnosis. Currently there is no cure for Vulvodynia. Treatments are only directed toward symptom relief. They include drug therapies such as tricyclic antidepressants or anticonvulsants, nerve blocks, interferon, biofeedback, and diet modification. While Vulvodynia, was first described as a syndrome at the end of the last century more than a hundred years have passed and the cause of the disorder remains undiscovered. Vulvodynia interferes with daily functioning, such as sitting walking and most other physical activity. It often affects one's ability to engage in sexual activity. It can impair one's ability to work, and participate in a social life. These limitations often lead to depression and thoughts of suicide.

A patient information handout on vulvodynia, written by the author and Elizabeth S. Smoots, M.D., is provided on page 1561. V ulvodynia is frequently misdiagnosed. In a general gynecologic practice population, the prevalence of this condition may be as high as 15 percent. Before the 1980s, very little was written about vulvodynia. In the 1980s, renewed interest was generated with the publication of articles by Friedrich, Lynch and McKay. Epidemiology Vulvodynia is a syndrome of unexplained vulvar pain that is frequently accompanied by physical disabilities, limitation of daily activities (such as sitting and walking), sexual dysfunction and psychologic disability. Originally suggested by McKay, the term "vulvodynia" has also been suggested by the International Society for the Study of Vulvar Disease Task Force to describe any vulvar pain. The incidence and prevalence of vulvodynia have not been well studied. Age distribution for the condition may range from the 20s to the 60s, and it is limited almost exclusively to white women.

Vulvodynia literally means 'pain in the vulva.' Vulvodynia is actually a symptom, like stomach cramps, which may have many different causes. Women who experience it often describe vulvodynia as a burning or shooting pain throughout the vulva area. This condition is closely related to a condition called vulvar vestibulitis where women experience pain around the opening of the vagina. Both of these conditions will be discussed in this health centre.
There are no clear studies to tell us how common this condition is. Some estimates suggest that up to 15 percent of women may experience some symptoms. Women can experience this condition throughout the lifecycle, but it is most commonly a disease of women in their reproductive years. Many never seek treatment.
Experts used to believe that sexual abuse was the primary cause of vulvodynia and that vulvodynia was a psychological condition not a physical one. While some women with vulvodynia have experienced abuse, studies have now shown that a history of sexual abuse is not more common among women with this condition. Sexual abuse in no longer considered the cause of vulvodynia, although a past experience of abuse may influence a woman's attitudes about her body and the course of her treatment.

Romantic and Sexual Relationships
Communication is key
Women with established sexual relationships are often worried about the feelings of their partners. Partners may feel rejected when a woman is unable to participate in sexual activities the couple previously enjoyed. Communication will be the key here, and you will need to explain to your partner exactly what is happening to you. It may be helpful to include your partner in some of your doctor's visits. Remember, that even without intercourse there are still lots of sexual activities you and your partner can enjoy, without causing you pain! It may also help to remind your partner that you also miss the option of sexual intercourse.

by Tori Hudson, ND Vulvodynia or vulvar pain syndrome is a multifactoral clinical syndrome of vulvar pain, sexual dysfunction, and psychological distress. Recognizing the four specific subtypes of vulvodynia is important in the management approach. The most common four subtypes are vulvar vestibulitis syndrome, cyclic vulvovaginitis, dysesthetic vulvodynia, and vulvar dermatoses. Simple clinical guidelines can be developed to improve the evaluation and treatment of these often long-suffering patients. Vulvodynia is different from itching or vulvar pruritus. Vulvodynia actually precludes itching because the burning and pain cause an intolerance to scratching. Over the years, the terminology used to describe vulvodynia has varied. The term vulvodynia has now been recommended by the International Society for the Study of Vulvar Disease (ISSVD) to describe any vulvar pain, regardless of etiology. The incidence of vulvodynia is not known but it is clearly more common than is generally thought. In a general gynecological practice the prevalence can be as high as 15% when actively looked for.

What is Vulvodynia?
Vulvodynia is a syndrome of unknown etiology characterized by chronic vulvar discomfort that can result in both physical and emotional pain in the lives of affected women.
Medical textbooks recognized Vulvodynia around the turn of the 20th century. This disease went unnoticed in society until a dramatic increase in the number of cases once again brought Vulvodynia to attention in the mid-1980's. Although an estimated 150,000 to 300,000 women have Vulvodynia in the United States, very little is known about how this painful condition can be prevented, treated or cured.
Symptoms
Women with Vulvodynia suffer from chronic debilitating pain of the vulvar area for which doctors can find no physical cause or cure. Careful physical examination will show some or all of the following signs:
  • An irritated, dry change to the skin of the vulva. Dilated, enflamed blood vessels in the underlying connective tissue. Severe inflamation of various glands within the vulvar vestibule. Scarring and contracture of the vulva and lower vagina.

    One of the causes of pain during sexual intercourse is vulvodynia. Vulvodynia means pain in the vulva. It is a syndrome characterized by chronic vulvar discomfort, especially burning, stinging, irritation or rawness. Typically, the vulva is so sensitive that even light pressure can be painful. Women with vulvodynia may even experience discomfort from clothing such as tight jeans. The condition can be extremely debilitating and can affect her whole outlook on life. Career, family life and relationships may suffer as a result. Causes of Vulvodynia The exact causes of vulvodynia are unknown. However, it is thought that neurological injury may be involved in some types of vulvodynia. In other cases, a skin condition or dermatoses may cause the pain. Women experiencing vulvodynia may have a history of recurrent vaginal yeast infections. Also external causes such as previous treatment such as laser surgery or sensitivity to corticosteroid drugs may play a role. In some cases, the entire vulva may be involved and in other cases only a portion of the vulva surrounding the entry to the vagina – the vestibule - may be involved. This type of vulvodynia is known as the vestibulitis.

    This is a syndrome or a combination of signs and symptoms about which traditional medicine does not know the cause. It is characterized by pain of various types in the vulvar area. Millions of health care dollars are spent annually for this disorder in the United States alone, not only on management, but also on the large proportion of cases that are misdiagnosed and inadequately treated. This pain, associated with allodynia and hyperpathia, has a strong genetic predelection, with African-American women rarely being affected. Background and Significance Significance of vulvodynia: Vulvodynia, or "burning vulva syndrome", is a debilitating, painful, chronic syndrome affecting hundreds of thousands of women daily. It is a syndrome without a home challenging to the numerous specialties called upon to see these women gynecologists, dermatologists, family physicians, neurologists, anesthesiologists, rheumatologists, and infectious diseases specialists. The entity has been trivialized, excised, lasered, anesthetized, and the women have been medicated for pain, depression, infection, inflammation, and at times for hysteria. Women are routinely misdiagnosed, and accurate diagnosis often comes after many months of pain, distress, and frustration for the women and for those trying to help them.
