ADs are exaggerated responses to identifiable stressors. These responses may be in the form of emotional symptoms (depressed mood, crying, hopelessness, nervousness, etc.) or behavioral symptoms (truancy in children, vandalism, fighting, etc.). The stressors that lead to ADs are the same sorts that lead to problems in livingmarital discord or breakup, the loss of a loved one, relocation, finances, parental rejection, work, substance abuse, and so on. The person with AD reacts more intensely and over a longer period than normal. This reaction may be because the person is naturally vulnerable or is in a vulnerable stage at the time, or it may be for a different reason altogether. A person should be diagnosed with an AD only if he or she returns to normal or near-normal within 6 months after the stressor stops. Also, AD symptoms must begin within 3 months of the occurrence of the stressor. As with other diagnosable mental disorders, AD must produce significant impairment in some aspect of life, or distress severe enough to merit clinical attention. The stressor cannot be the recent death of a loved one, unless the symptoms are so severe or are lasting so long that it is beyond what would be considered normal bereavement.

Persons with AD are often viewed as disproportionately overwhelmed or overly intense in their responses to given stimuli. The disorder is time-limited, and symptoms lessen upon removal of the stressor or when new adaptation occurs. AD is considered one of the subthreshold disorders, which are less well defined and share characteristics of other diagnostic groups. Subthreshold disorders fall between defined disorders and problem level (V Code) diagnoses (Strain, 1998). Subthreshold disorders allow for the "classification of early or temporary states when the clinical picture is vague and indiscreet and yet the morbid state is more than expected in a normal reaction" (Strain, 1998). Because of insufficient behavioral criteria for patients with AD, reliability and validity of this disorder remain problematic. Diagnosis is constructed to allow for the classification of psychiatric conditions that are clinically significant but do not meet major criteria for major syndromes. In 1998, Strain et al asserted that the diagnostic construct for patients with AD is "clinically significant and deemed to be in excess of a normal reaction to the stressor in question, and not solely the result of a psychosocial problem (V Code) requiring medical attention."
