Extractions: Sleep apnea afflicts more than 12 million people in the United States. It takes its name from the Greek word apnea, which means "without breath." People with sleep apnea stop breathing repeatedly during their sleep, often for a minute or longer, and as many as hundreds of times during a single night. Sleep apnea can be caused by complete obstruction of the airway (obstructive apnea) or partial obstruction (obstructive hypopnea; hypopnea is slow, shallow breathing), both of which can disrupt sleep. There are three types of sleep apnea obstructive, central, and mixed. Of these, obstructive sleep apnea (OSA) is the most common. OSA occurs in approximately 2% of women and 4% of men over the age of 35. Physiology and Causes Complications Physiology and Causes The cause of OSA is unclear. The site of obstruction in most patients is the soft palate, extending to the region at the base of the tongue. The body has no rigid structure, such as cartilage or bone, in this area to keep the airway open. During the day, muscles in the region keep the passage open. As a person with OSA falls asleep, these muscles relax to a point where the airway collapses and becomes obstructed. When the airway closes, breathing stops, and the sleeper awakens to open the airway. The arousal from sleep usually lasts only a few seconds, but brief arousals disrupt continuous sleep and prevent the person from reaching the deep stage of rapid eye movement (REM) sleep, which the body needs to rest and replenish its strength. Once normal breathing is restored, the person falls back to sleep, only to repeat the cycle throughout the night.

There are two basic types of apnea: Obstructive : An obstructive apnea occurs when the body (especially the chest walls and abdomen) attempts to breathe in, but the air flow is blocked by some obstruction in the upper portion of the airway. Typical symptoms of obstructive sleep apnea are daytime sleepiness, awakening at night with a choking sensation, morning headaches, restlessness during sleep, and snoring. During sleep, the individual with OSA may block off his or her airway 30 to 100 times an hour. Each blockage causes them to leave the deep stages of sleep and partially awaken until the airway is reopened. As a result, the sleep is severely disrupted and the patient will feel sleepy throughout the day. The daytime sleepiness can be so severe that individuals will fall asleep while driving or while sitting in a chair.

Go to top of page Snoring is caused by a narrow airway. That's because air travels faster through a slender tube than through a broad one. This rapidly moving air causes the soft tissues of the throat (the tonsils, soft palate, and uvula) to vibrate. It is this vibration which is the sound of snoring. It's like putting a flag in front of a fan: the faster the fan, the greater the flutter. Why is the airway narrow in snorers? Many things can take up space in the airway reducing its diameter. These can include large tonsils, a long soft palate or uvula, and, in people who are overweight, excessively flabby tissue. Extractions: There can be large falls in arterial oxygen saturation during sleep in patients with chronic obstructive pulmonary disease. In general, these falls can be predicted from daytime oxygen levels and are not due to classical OSA syndrome, but to a number of factors which combine to produce worsening hypoxaemia during sleep. These patients are often at the limits of their respiratory capacity, and the loss of the intercostal and accessory muscle contribution to breathing, and the increase in upper airways resistance associated with REM sleep, cause marked falls in ventilation. In addition, patients may be hypoxic while awake and small falls in ventilation and PaO2 lead to large falls in arterial oxygen saturation during sleep. The hypoxic episodes demonstrated by overnight oximetry in a patient with chronic obstructive pulmonary disease differ in character from those seen in patients with OSA syndrome (figure 10). There is some evidence that increases in upper airway narrowing during sleep, short of apnoeas and even snoring, may contribute to the development of CO2 retention in these patients. However classical sleep apnoea is not significantly more common in unselected COPD patients with raised CO2 levels, and excessive nocturnal hypoxaemia is not one of the predictors of long- term survival in patients with COPD.
