Pain with maneuvers that stress the scaphoid can suggest fracture on physical exam. Radiographs are crucial: Determining the location, stability, and orientation of the fracture guides treatment and predicts outcome. Distal fractures and horizontally oriented fractures generally heal well and can be managed by immobilization in a short-arm thumb spica cast. Proximal fractures and vertically oriented fractures have the most morbidity, and open reduction should be considered. When prolonged immobilization is required, range-of-motion and strengthening exercises are prescribed to restore hand function after cast removal. T he scaphoid bone is crucial to the intricate function of the wrist. It endures a myriad of complex forces, including a common injury in active peoplea fall on an outstretched hand, particularly when the wrist is radially deviated. The scaphoid is the most frequently injured carpal bone. In the wrist, only distal radius fractures are more common. Scaphoid injuries are most commonly seen in young men, are often misdiagnosed as sprained wrists, and are rarely seen in children because the distal radial physis usually fails first. Scaphoid fractures have a warranted reputation for needing close attention. Fractures of this bone can be complicated by a tenuous blood supply and a tendency for some of the fractures to be unstable. Certain scaphoid fractures require prolonged immobilization and have an increased risk of poor results. However, 60% to 70% of scaphoid fractures are stable and 90% will heal with prompt diagnosis and proper immobilization (1,2).

Extractions: Michael L. Richardson, M.D. Facial Fractures The bones of the skull and face collectively make up the most complex area of skeletal real estate in the body. Analysis of the fractured face requires a knowledge of not only normal anatomy, but also of common fracture patterns in the face. Although they represent serious injuries, the workup and treatment of facial fractures is often properly delayed until more pressing problems have been addressed, such as the establishment of an adequate airway, hemodynamic stabilization, and the evaluation and treatment of other more serious injuries of the head, chest and skeleton. Once these problems have been managed, it is time to work up facial fractures. At our institution, high resolution CT is currently the imaging procedure of choice for most facial fractures. The complex anatomy and fractures of the facial bones are shown extremely well by CT, and soft tissue complications can be evaluated to a far greater degree with CT. Therefore, the plain film facial series has taken a back seat to CT in the past few years, and is now used only in certain situations, such as when the facial trauma is very focal (nasal fracture), or when CT is unavailable. However I find it easier to initially teach the anatomy and fracture patterns of the face with plain films. Once these concepts have been grasped by the resident, one can then move on to the axial and coronal anatomy shown by CT.

Whenever you get this history, a red flag should pop up in your visual cortex and a bell should go off in your auditory tracts. Generally, the differential diagnosis here is fairly logical. The thing to remember is that a bone needs a reason to break. Usually, that reason is that a substantial force has been applied to the bone. If there is no evidence of such a force, we must look for some other reason. To do this, you must first answer two questions: 1) did the fracture occur through normal or abnormal bone; 2) is there normal or decreased amount of bone present? Once you have answered these two questions, you can use the following handy-dandy flowchart to arrive at the most likely mechanism for the fracture. Fractures through abnormal bone are called "pathological" fractures. Probably the most common cause of this is an underlying tumor, either benign or malignant. However, almost any underlying process of bone that weakens the bone can lead to a pathological fracture. Therefore, one should also consider infection, osteomalacia, and Paget's disease, among other causes.

Extractions: The wrist joint is made up of the two bones in your forearm (the radius and the ulna) and the many tiny bones in the base of your hand (carpal bones). The radius is the bone on the thumb side of the forearm, and the ulna is the outer bone of the forearm located on the side of the pinky (Fig. 1). Fractures of these bones in the forearm are the most common wrist fractures in all age groups. These fractures generally occur during a fall on an outstretched hand. Children with these fractures may have only a small amount of swelling and deformity (the bone is out of its normal position). In adults, particularly the elderly, fractures near the wrist can cause a large amount of swelling and deformity. (Smith's fracture and Colles' fracture) Treatment The physician's first step in treating anyone with wrist pain after an injury is to obtain and evaluate an x-ray. This will usually be all the testing necessary to tell if a bone is fractured (broken, cracked, chipped, or smashed). Sometimes, however, more testing may be necessary (such as an MRI, CT, or bone scan) to help determine the best form of treatment. The treatment of a wrist fracture varies widely and depends on its location, position, and stability, and how many pieces of broken bone have been created by the injury. Most fractures are treated simply with a cast; sometimes the bone fragments must be gently pushed back to a more normal position before casting. Some fractures are unstable enough that a cast cannot hold the pieces of bone in a normal position for healing, and surgery may then be necessary. Patients with wrist fractures usually do very well, although some may have problems with slight stiffness or loss of motion. Arthritis can also develop sometime in the future, particularly if the smooth, shiny joint surfaces were disrupted by the fracture pieces.

Extractions: Our Mission Falls are the most common cause of non-fatal injury and the second most common cause of injury-related death in the elderly. In terms of numbers and cost, hip fractures represent the most serious injury related to falls, followed by fractures of the distal radius. Current research in the Injury Prevention and Mobility Laboratory at Simon Fraser University is focused on developing improved methods for preventing falls and fall-related injuries. Safe experiments with living humans are used in combination with mathematical models to assess how age-related declines in neuromuscular performance alter one's ability to prevent falls, and avoid injury in the event of a fall. Engineering tools are used to develop and evaluate specific fracture prevention strategies, from exercise training for fall prevention, to hip pads and energy-absorbing floors for attenuating impact forces. Current projects are also examining the links between age-related changes in sensorimotor function, cognition, and balance, and the mechanics and prevention of forearm fractures from falls on the outstretched hand (the most common fracture in the under-65 population). Around one quarter of all patients in a fracture clinic will have suffered a fracture of the wrist. The usual cause of a fracture of the wrist is a fall. The person falling tries to break their fall by putting the hand out to save themselves and in doing so the wrist is forced backwards (figure one). Extractions: Introduction The scaphoid is the most commonly injured bone in the carpus, accounting for 60-70% of all carpal fractures. Only distal radius fractures are more common. 90 to 95% will go on to union if treated adequately (thus, 5-10% non-union may occur, in spite of treatment.) Anatomy The scaphoid is the most complex carpal bone, making up the base of the lateral (mobile) column of the wrist (Navarro). It lies obliquely at about 45 degrees to the longitudinal axis of the two carpal rows. Most of its surface is made up of articular cartilage. Distally, it articulates with the trapezium and trapezoid in a gliding motion, The articulation with the trapezium forms a base for independent movement of the thumb. On the ulnar side, it articulates distally with the capitate, and proximally with the lunate in a rotary motion. Proximally, its large, biconvex surface allows articulation with the radius. VOLAR (top) and DORSAL (bottom) LIGAMENTS of the scaphoid and wrist The dorsal radiocarpal ligament inserts obliquely across the dorsal surface. The waist, or central indentation, provides a path for the palmar radiocarpal ligament to the capitate. The scaphoid tuberosity acts as a pivot for the tendon of the flexor carpi radialis. The scaphoid is bound to the radius and carpal bones by a complex ligament system. Since the scaphoid spans the proximal and distal carpal rows, it acts as a fink, or structural strut between them and its integrity is necessary for wrist stability. Because of these complex articulations, the scaphoid moves in nearly all carpal motions, especially in volar flexion. Any alteration to the integrity of the joint surfaces (fracture) or to the ligamentous stability (dislocation) may ad to degenerative changes and cause a great deal of morbidity.
