    Extractions: Frozen tumor and normal gastric mucosa were collected from gastrectomy specimens from Queen Mary Hospital, The University of Hong Kong. 90 primary gastric adenocarcinomas, lymph node metastases from 14 of the 90 primary gastric cancers, and 22 samples of non-neoplastic gastric mucosa were analyzed using DNA microarrays; details of this analysis are presented in another publication (Chen et al, manuscript in preparation). Another 59 adenocarcinomas were later tested independently by quantitative RT-PCR as described below. This study was approved by the Ethics Committee of the University of Hong Kong and the IRB from Stanford University. Tissues were frozen in liquid nitrogen within half an hour after they were resected. Samples of non-neoplastic mucosa from stomach were dissected free of muscle when fresh, and histologically confirmed to be tumor-free by frozen section. The tumors were unselected with respect to grade, but there was, in both cases, selection for specimens judged to have at least 50% tumor cells in the block. The clinical records were surveyed and the clinical parameters (for this study, overall survival) recovered after the biological experiments for each dataset were complete.

    Extractions: Cancer of the stomach, also called gastric cancer, is a disease in which cancer (malignant) cells are found in the tissues of the stomach. The stomach is a J-shaped organ in the upper abdomen where the food is broken down (digested). Food reaches the stomach through a tube called the esophagus that connects the mouth to the stomach. After leaving the stomach, partially digested food passes into the small intestine and then into the large intestine called the colon. If there are symptoms, a doctor will usually order an upper gastrointestinal x-ray (also called an upper GI series). For this examination, the patient drinks a liquid containing barium, which makes the stomach easier to see in the x-ray. This test is usually performed in a doctor's office or in a hospital radiology department. The doctor may also look inside the stomach with a thin, lighted tube called a gastroscope. This is called a gastroscopy, and it finds most cancers of the stomach. For this test, the gastroscope is inserted through the mouth and guided into the stomach. The doctor may spray a local anesthetic (a drug that causes loss of feeling for a short period of time) into the throat or give the patient other medicine before the test so that no pain is felt.

    Extractions: Gastric cancer has been one of the great malignant scourges affecting mankind for as long as medical records have been kept. Until operative resection pioneered by Bilroth and others became available no effective treatment was feasible, and death from cancer was virtually inevitable. Even with resection by total gastrectomy, the chances of tumor eradication remained small. Over recent years, however, the situation has been changing. Some changes have resulted from better understanding of the disease, early detection and better management techniques with applied clinical research, but reasons for other changes are poorly understood. The incidence of gastric cancer is decreasing, especially in westernized societies; still, it remains the primary killer of male adults in Japan and Korea. Improvements in outcome have been reported from mass screening and early detection; more refined techniques of establishing early diagnosis, tumor type and tumor extent; more radical surgical resection and resection at earlier stages of disease. A most exciting potential for improvement has come with clinical studies using combinations of treatment modalities, the most promising of these being the use of regional chemotherapy with operative surgery and possibly combined with radiotherapy or immunotherapy.

Results of an esophagogram were suggestive of achalasia. Gastric adenocarcinoma infiltrating the gastric cardia was seen on gastroscopy. The mode of presentation of gastric cancer in this case has not been previously reported in association with pernicious anemia. Patients with pernicious anemia are at higher risk of having gastrointestinal neoplasms than is the general population. We review the current literature and address the controversy concerning the need to subject patients with pernicious anemia to surveillance with upper gastrointestinal endoscopy. CASE REPORT Fig 1 ). The mucosa of the narrowed part was smooth, and barium entered the stomach poorly and in bursts. The possibility of achalasia of the esophagus was raised. Endoscopy of the upper GI tract showed normal esophageal mucosa and a moderately dilated esophageal lumen. The gastroesophageal junction was entered with mild resistance. A retroflexed view revealed a mass in the cardia ( Fig 2 The patient had subtotal proximal gastrectomy and distal esophagectomy with primary anastomosis and splenectomy. The tumor measured 2 cm in diameter and penetrated the gastric wall through the serosa. Ten of the 12 resected lymph nodes were involved with the neoplasm. The tumor was classified as T3, N1, M0. Subsequently, the patient received 25 sessions of external radiation delivering a total of 45 Gy, as well as 2 cycles of 5-fluorouracil and leucovorin. Within 4 months, two new masses were detected in the liver on computed tomography. The patient died of pneumonia 6 months after the initial hospital admission, with evidence of metastatic gastric adenocarcinoma.
