Extractions: Causes, incidence, and risk factors: Several different types of cancer can occur in the stomach. The most common type is called adenocarcinoma, which refers to the way the cancer looks under the microscope. There are several types of adenocarcinoma. Other types of gastric cancer occur much less frequently so this article is focused on adenocarcinoma of the stomach. Adenocarcinoma of the stomach is a common cancer of the digestive tract worldwide, although it is relatively uncommon in the US. It occurs most frequently in men over 40 years old. The incidence of this form of gastric cancer is extremely high in Japan, Chile, and Iceland. The incidence of most types of gastric adenocarcinoma in the US has declined over the years. Experts propose that the decrease may be related to reduced intake of salted, cured, and smoked foods and increased vitamin C consumption.

Extractions: Eur J Cancer, Vol. 31A, No. 6, 928-931, 1995 Resection rates and postoperative mortality rates were studied in patients with gastric cancer, diagnosed from 1982 through 1992 in the southwestern part of The Netherlands. Overall, 51% of the patients underwent resection. For patients aged 0-59, 60-69, 70-79 and 80 years and over, resection rates were 64%, 55%, 54% and 35%, respectively. Tumours located in the cardia were less often resected than tumours of the antrum, 39% versus 71%. The postoperative mortality after resectional operations was 8.3%; 9.2% for men and 6.7% for women. The operative risk increased markedly after the age of 70. For patients under 70 the rate was 3.4% against 12.4% for those 70 years and older. These results indicate that elderly patients can be operated at acceptable risk and that palliative resections may be considered, especially in patients younger than 70 years.

However, the rate of stomach cancer has been declining in the United States and western Europe over the past half-century, a trend which has been felt attributable in large part to changes in diet during this period of time. Interestingly, the incidence of cancers located in different portions of the stomach appear to be heading in opposite directions: while those located in the more distal (lower) portion of the stomach have been declining in incidence, there has actually been an increase in cases occurring in the proximal portion of the stomach (closer to the esophagus) and at the gastroesophageal junction (where the esophagus meets the stomach). It is tempting to speculate that Hamlet recognized the infectious etiology of peptic ulcer disease, thus presaging recent developments in modern medicine. As old microbes are resurging and new pathogens are emerging, the study of infectious causes of disease is again in the forefront of medicine. Helicobacter pylori , the elusive, acidtolerant bacteria first associated with gastritis, peptic ulcer disease, nonulcer dyspepsia and, finally, gastric cancer, serve as a paradigm for the evolution of infectious disease in modern medicine. Until recently, the pathogenesis of ulcer disease was attributed to overproduction of acid and mucosal barrier destruction as a result of stress, smoking, or dietary factors. Extractions: CASE REPORT Pernicious Anemia and Gastric Cancer By David Sutin, MD Abstract An elderly man had undergone a total gastrectomy, which had revealed a gastric adenocarcinoma. When he was seen in the clinic several months later, test results were compatible with a diagnosis of vitamin B deficiency. It is likely that this patient had pernicious anemia for several years and had developed an associated gastric carcinoma. ( Annals of Long-Term Care Case Report An elderly man presented to the geriatric clinic in early September 1997. He reported that in May 1997 he had black stool. An endoscopic evaluation at the time had shown a 20 x 20-mm gastric ulcer, the initial biopsy of which had been negative for malignancy. A second endoscopy with biopsy 2 weeks later, however, was positive for adenocarcinoma, moderately differentiated. The patient then had an abdominal computerized tomography scan, which had shown no metastasis. He had undergone a total gastrectomy, which had revealed a gastric adenocarcinoma, with metastasis in the omental tissue and in one of eight lymph nodes. The patient had done well postoperatively and was then discharged and followed in the geriatric and oncology clinics. When he was seen in the geriatric clinic, he had no complaints except for frequent flatus and occasional soft stool postoperatively and occasional dyspnea on climbing up a hill. He had a history of smoking one pack of cigarettes per day until 2 months prior to the visit, denied having any allergies, and, except for occasional Mylanta, was on no medication. He also denied any previous medical history, except as above, and had not seen a physician for 50 years. The patient lives with his son, and they alternate the shopping and the cooking responsibilities.
