Glaucoma is a frequent cause of blindness in humans and animals. To understand glaucoma, it is necessary to understand how the normal flow of intraocular fluid maintains normal intraocular pressure. The fluid inside the eye is called the aqueous humor which is produced in the ciliary body which is located behind the iris. This fluid flows through the pupil and drains from the eye through a sieve-like network located at the junction of the cornea and the iris called the iridocorneal cleft or drainage angle. The aqueous humor is produced and drains from the eye at approximately the same rate, resulting in a stable pressure inside the eye of 15 to 30 mmHg (millimetres of mercury). Glaucoma occurs as a consequence of inadequate outflow of aqueous humor and a subsequent buildup of pressure inside the eye. The resulting high pressure damages the optic nerve and results in blindness. There are two categories of glaucoma.

As the diagram opposite shows, this nerve carries information from the light sensitive layer in your eye, the retina, to the brain where it is perceived as a picture. Your eye needs a certain amount of pressure to keep the eyeball in shape so that it can work properly. In some people, the damage is caused by raised eye pressure. Others may have an eye pressure within normal limits but damage occurs because there is a weakness in the optic nerve. In most cases both factors are involved but to a varying extent. Eye pressure is largely independent of blood pressure. A layer of cells behind the iris (the coloured part of the eye) produces a watery fluid, called aqueous. The fluid passes through a hole in the centre of the iris (called the pupil) to leave the eye through tiny drainage channels. These are in the angle between the front of the eye (the cornea) and the iris and return the fluid to the blood stream. Normally the fluid produced is balanced by the fluid draining out, but if it cannot escape, or too much is produced, then your eye pressure will rise. (The aqueous fluid has nothing to do with tears.) Why can increased eye pressure be serious?

(Some types of glaucoma are primary open-angle, angle-closure, secondary, congenital, juvenile and low-tension.) The increase in pressure happens when the passages that normally allow fluid in your eyes to drain become clogged or blocked. The reasons that the passages become blocked are not known. Glaucoma is one of the leading causes of blindness in the U.S. It most often occurs in people over age 40. People with a family history of glaucoma, African Americans, and those who are very nearsighted or diabetic are at a higher risk of developing the disease. The most common type of glaucoma develops gradually and painlessly, without symptoms. A rarer type occurs rapidly and its symptoms may include blurred vision, loss of side vision, seeing colored rings around lights and pain or redness in the eyes. Glaucoma cannot be prevented, but if diagnosed and treated early, it can be controlled. Vision lost to glaucoma cannot be restored. That is why the American Optometric Association recommends annual eye examinations for people at risk for glaucoma (your doctor may, depending on your condition, recommend more frequent examinations). A comprehensive optometric examination will include a tonometry test to measure the pressure in your eyes; an examination of the inside of your eyes and optic nerves; and a visual field test to check for changes in central and side vision.
