View Guestbook Sign Guillain-Barre Syndrome Scuba Diving Concerns : This condition has skeletal muscle weakness which can involve extremity, truncal or bulbar groups and typically evolves over a matter of several hours to a few days can affect diving and boat crew abilities creating safety in the water as well as buddy rescue concerns. In the C. Miller-Fisher variant, ataxia as well as ophthalmoplegia (internal and external) accompany the obligatory findings of areflexia. Dysautonomia may also be present, posing an additional concern regarding tolerance of water pressure and immersion changes, blood pressure and cardiac rhythm disturbances that may be especially life-threatening in the underwater environment. Return to diving can be considered after full recovery of strength and autonomic nervous system function. Tendon-stretch reflexes may never return but would not prohibit return to diving. Data needed to decide return to diving Therapy : Plasmapheresis and/or intravenous immunoglobulin (IVIG) therapy is warranted in those cases which involve weakness progressing to the point of impairing walking or respiratory abilities. Adrenocorticosteroid therapy is not beneficial and may actually worsen the outcome.

The pathophysiology of the syndrome can be explained through GQ1b ganglioside autoantibodies, which are seen in 96 percent of cases of Miller Fisher Syndrome. These autoantibodies recognize epitopes expressed specifically in the nodal regions of oculomotor nerves and cerebellar neurons. Organisms such as Mycoplasma Pneumoniae and Campylobacter jejuni contain cell walls that are capable of inducing the production of these antoantibodies. Treatment is thus involved in immune modulation, through the use of IVIg, plasma exchange, and steroids. Our patient required intubation and wan treated with all three modalities over a three-month hospitalization. He eventually recovered to be discharged to long term rehabilitation. Article abstract West Nile fever is a mosquito-borne febrile illness seen in Africa, Asia, and Europe, but reported in North America only once. The recent outbreak in New York City represents the first time this virus has been detected in North America. West Nile virus usually causes mild symptoms, though rarely it can cause neurologic diseases, with a fatal outcome or permanent neurologic sequelae.     Extractions: Guillain-Barre' Syndrome: causes segmental demyelination (destruction or loss of the myelin sheath of a nerve or nerves) of the peripheral nerves. Guillain-Barre syndrome is an acute, rapidly progressive, and potentially fatal form of polyneuritis (inflammation of many nerves simultaneously). It causes both the posterior (sensory) and anterior (motor) nerve roots, signs of sensory and motor losses occur simultaneously. Cause: True cause for Guillain-Barre syndrome is unknown, but it may be a cell mediated immunologic attack on peripheral nerves in response to a virus. Predisposing factors may include mild febrile illness, surgery, viral illness, Hodgkin's disease or some other cancer, or systemic lupus erythematosus. Symptoms: Along with a history of preceding febrile illness (usually a respiratory tract infection) - other signs includes: Paresthesia ( morbid or perverted sensation; and abnormal sensation, as burning, prickling, formication, etc.) Muscle weakness - usually appears in the legs first, then extends to the arms and facial nerves in 24 to 72 hours. In some patients it may develops in the arms first or in the arms and legs simultaneously. May have facial diplegia (paralysis of like parts on either side of the body).
