P atients with irritative voiding symptoms and negative urine cultures frequently present a diagnostic challenge for primary care physicians. Interstitial cystitis is a severely debilitating disease of the urinary bladder. Symptoms of interstitial cystitis include excessive urgency and frequency of urination, suprapubic pain, dyspareunia and chronic pelvic pain. Interstitial cystitis interferes with employment, social relationships and sexual activity. The course of the disease is usually marked by flare-ups and remissions. Interstitial cystitis is a chronic disease that persists throughout the patient's life. On clinical evaluation, patients with interstitial cystitis should have no other definable pathology, such as urinary infections, carcinoma, or radiation-induced or medication-induced cystitis (Parsons CL. Interstitial cystitis, new concepts in pathogenesis, diagnosis and management. Presentation at 3rd annual meeting of the American Urological Association, June 4, 1998, San Diego, Calif.).

An article on this topic is available in this issue of AFP What is interstitial cystitis? Interstitial cystitis is a chronic bladder problem. About 750,000 Americans have interstitial cystitis. Most of them are women. People with interstitial cystitis have a bladder wall that is inflamed and irritated (red and sore). This inflammation can scar the bladder or make it stiff. A stiff bladder can't expand as urine fills it. There may be pinpoint bleeding from the walls of the bladder. A few people get sores in the bladder lining. People with interstitial cystitis may have many of the following symptoms: An urgent need to urinate, both in the daytime and during the night Pressure, pain and tenderness around the bladder, pelvis and perineum (the area between the anus and vagina or the anus and scrotum). This pain and pressure may increase as the bladder fills and decrease as it empties in urination.

Female Urology: Interstitial Cystitis Overview. interstitial cystitis (IC) is a chronic inflammatory disease ofthe bladder that results in symptoms of urgency, frequency, and pain. http://www.clevelandclinic.org/urology/patients/female/cystitis/default.htm
Extractions: Overview Interstitial Cystitis (IC) is a chronic inflammatory disease of the bladder that results in symptoms of urgency, frequency, and pain. Several causes have been proposed for this disease, including immune and/or neural dysfunction, infectious agents, and alterations in the permeability of the tissue lining the inside of the bladder (urothelium). It is currently thought that two or more of these factors may be involved with the progression of the disease. Treatments for IC are limited, and researchers at the Clinic are currently investigating several new substances that may effectively relieve the pain associated with debilitating disorder. [Also check the bottom of this page for links to pages containing quotes from the many e-mails I have received from IC sufferers.] I understand that many doctors and urologists don't know about interstitial cystitis since the symptoms are like a bladder infection. Some men and women have been told since no infection is present, it must be "all in their head". One book I read said some women have had hysterectomies or their bladders removed to try to alleviate their symptoms. How tragic. With interstitial cystitis, there is repeated bleeding and scarring of the bladder lining. PERSONAL EXPERIENCE: Since 1988 I've suffered from a chronic urinary condition called interstitial cystitis. It caused sleep deprivation for 8 1/2 years. I was uptight, stressed out, tired, irritable, and very discouraged. Twenty-four hours a day I felt like my bladder was so full it would burst. The only way to relieve this often excruciating pain was to repeatedly empty my bladder. I was up to the bathroom from 6 to 12 times a night, suffering from sleep deprivation as I desperately sought relief. Top of Page COMMON SYMPTOMS: Severe urinary bladder urgency [feeling like your bladder will explode], Burning sensation in urethra [bladder outlet], Relief only upon emptying bladder, Delayed emptying of bladder in stages [having to sit about 10 - 15 minutes to complete this], Need to empty bladder every hour all night.

Extractions: INTERSTITIAL CYSTITIS Interstitial Cystitis (IC) is a chronic bladder disease/disorder within the group of painful bladder syndromes. Interstitial Cystitis or more commonly known among us sufferers as (IC) is a chronic, debilitating bladder disease, which is often overlooked by the medical profession. Many patients with IC find that it is the only health problem they will have, but a large percentage of patients also suffer with various other syndromes including; Fibromyalgia, Urethral Syndrome, Vulvodynia, IBS, Chronic Fatigue Syndrome, Allergies, and other Auto-immune disorders such as Scleroderma. The Symptoms of Interstitial Cystitis Pain. This can be in the abdominal, urethral or vaginal area. Pain is also frequently associated with sexual intercourse. Urgency. The sensation of having to urinate immediately, may also be accompanied by, pressure or spasms. Frequency. This can be day and/or night frequency of urination. In severe cases this can be as much as 60-70 times a day. Diagnosis of IC is performed using a Cystoscope (Cystoscopy), hydro-distention and biopsies are normally carried out at the same time. Biopsies will rule out anything more sinister going on in the bladder. Cystoscopies/distention/biopsies should be performed under general anesthetic but some doctors have been known to perform these investigations under local anesthetic.

Research Overview The urinary system consists of the kidneys, ureters, bladder, and urethra. The kidneys, a pair of purplish-brown organs, are located below the ribs toward the middle of the back. The kidneys remove liquid waste from the blood in the form of urine, keep a stable balance of salts and other substances in the blood, and produce erythropoietin, a hormone that aids the formation of red blood cells. Narrow tubes called ureters carry urine from the kidneys to the bladder, a triangle-shaped chamber in the lower abdomen. Like a balloon, the bladder's elastic walls relax and expand to store urine and contract and flatten when urine is emptied through the urethra. The typical adult bladder can store about 1 1/2 cups of urine.

Extractions: INTERSTITIAL CYSTITIS Gabe Mirkin, M.D. When people have severe pain every time their bladders fill and no cause can be found, doctors often diagnose interstitial cystitis. Two recent reports offer hope for people with this previously hopeless incurable disease (1,2,3). If you have terrible discomfort when your bladder is full, and your doctor cannot find a vaginal, prostate or urinary tract infection, he usually refers you to a urologist who will examine the bladder directly. If he finds no cause, he usually diagnoses interstitial cystitis (4). The bladder is a balloon lined by muscles that contract when it is full. The severe pain of interstitial cystitis may be caused by contractions of bladder muscles. The body produces a chemical called nitric oxide that relaxes muscles and blocks spasms. For example, hemorrhoids are felt to be caused by lack of nitric oxide with resultant spasms of the anal sphincter that cause blood vessels at the end of the intestines to enlarge. Several studies in the last year show that chemicals that raise nitric oxide levels relax the anal sphincter muscles and get rid of hemorrhoids (5,6,7,8,9,10). The study in the Journal of Urology shows that people with interstitial cystitis have low tissue levels of nitric oxide also, and that L-arginine, a protein building block, raises tissue levels of nitric oxide and relaxes bladder muscle spasms to help control the pain of interstitial cystitis (1,16).

Extractions: Who we are ... Donations Interstitial Cystitis is a condition causing discomfort and pain in the bladder and surrounding pelvic region. Interstitial cystitis is a challenging disease that is difficult to define, since a general consensus has not been reached as to how it is to be diagnosed. The National Institute of Arthritis, Diabetes, Digestive and Kidney Diseases (NIADDK) established criteria for diagnosis in 1987 but they were not meant to define the disease, but rather to ensure that groups of patients studied would be relatively comparable. There is no agreed upon pathologic finding with which to diagnosis a person as having IC. IC can have a devastating effect upon a person's life. It has been reported that using well-developed quality of life indicators, IC patients scored extremely low- in fact, even lower than a sample of patients with chronic renal failure undergoing dialysis.

Extractions: INTERSTITIAL CYSTITIS The term, interstitial cystitis (IC), is used for a large group of patients whose symptoms include bladder as well as urethral, vaginal or pelvic pain often relieved after urination, irritative urinating symptoms such as urgency, frequency and night time urination, accompanied by sterile urine. Painful intercourse is also common. IC is one of the most challenging urological conditions to manage as there are currently no known causative factors. The general opinion is that the cause is multifactorial, and that we may be dealing with a collection of symptoms rather than a specific disease. IC can only be diagnosed with cystoscopic examination under a general anesthetic where we look for bleeding areas or bladder ulcerations. While it most commonly afflicts middle-aged women with a peak incidence between 30-40 years of age, the condition occurs in all age groups. Generally, the symptoms do not progress continuously, but rather reach their final stage rapidly and then stabilize at that level. Major deterioration of symptoms is unusual as there is no evidence that mild cases progress to more sever forms. Up to fifty percent of patients experience spontaneous remissions, having flare-ups in between. Relatively few patients suffer such unremitting pain and frequency that they cannot maintain a normal lifestyle. There may be almost half a million patients with IC in the United States. Numerous studies have failed to find an infectious cause for IC, such as bacteria, fungal or viral. The most popular etiology postulates a deficiency in one of the chemicals that lines the surface of the bladder. Biopsies from the bladder in patients with IC have revealed more inflammatory cells in the lining of the bladder than usual.
