    Extractions: April 26, 2001 Researchers at the CIIT Centers for Health Research and the University of Texas M.D. Anderson Cancer Center have discovered that a tumor suppressor gene that inhibits kidney cancer also controls a major gene for polycystic kidney disease. The findings provide insights that may lead to new therapies. The research team also included collaborators from Genzyme Genetics and the Mayo Clinic. "The biological behavior of the renal cysts that develop in patients with polycystic kidney disease shares many similarities with kidney cancer, and suggested to us that there could be a link between these two diseases," said Professor Cheryl Walker of the University of Texas MD Anderson Cancer Center. "The finding that the same tumor suppressor gene can control both polycystic kidney disease and tumor development establishes this link for the first time, and proves that a connection exists between polycystic kidney disease and kidney cancer at the molecular level." More prevalent than better-known diseases such as cystic fibrosis or sickle cell anemia, autosomal dominant polycystic kidney disease or ADPKD is the most common lethal genetic disease in humans. It is a leading cause of kidney failure, end-stage renal disease, and the need for dialysis.

    Extractions: Historically, incidence rates for kidney cancer have included cancers of the renal cells (in the main part of the kidney) and the renal pelvis (the lower part of the kidney where urine collects before entering the ureter and continuing to the bladder), although there is evidence that these cancers have different characteristics. They are presented together here for continuity. About one of five kidney cancers occur in the renal pelvis. Internationally, the highest incidence rates occur in the United States, Canada, Northern Europe, Australia, and New Zealand. The lowest rates are in Thailand, China, and the Philippines. Rates in these countries are about one-third the rates in the high risk countries. Kidney cancer has a relatively high mortality rate in all racial/ethnic populations. Following the incidence pattern, mortality rates are about twice as high in men as in women, regardless of age. There are too few deaths among American Indian (New Mexico), Alaska Native and Hawaiian populations to calculate reliable rates. Mortality rates for blacks are comparable to those for white non- Hispanics. Rates for the other races are lower. In all racial/ethnic groups the mortality rates increase with age. Cancers of the kidney and renal pelvis share many risk factors although the strengths of the associations differ. For both types of cancer the only well-established risk factor is cigarette smoking. Compared to nonsmokers, smokers have about twice the risk for renal cell cancer and about four times the risk for renal pelvis cancer than nonsmokers. Other probable risk factors include obesity and, especially for cancer of the renal pelvis, heavy long-term use of analgesics (medications used to relieve pain). Cessation of cigarette smoking is the best single step in preventing these cancers. It is estimated that this measure alone would reduce by one-half the number of renal pelvis cancers and by one-third the number of renal cell cancers.

Patient Ernie Jones, 60, of Long Beach, was diagnosed in September 2000 with kidney cancer - often referred to as the "silent killer" because most symptoms appear after the cancer has spread. In Jones' case, there were no signs until he felt an ache in his shoulder while preparing to go fishing, according to his wife, Carol. He was directed to UCLA, where he was treated by Dr Robert Figlin, a professor of hematology/oncology in the Department of Urology and a researcher at UCLA’s Jonsson Cancer Center. "Ernie put out the word that he wanted all of his friends and everyone they know to be aware of this disease," Carol Jones said. "He was so happy with the treatment he received at UCLA that he wanted to be an asset to the program."
