    Extractions: Polycystic Kidney Disease (PKD) Polycystic kidney disease (PKD) is an inherited disease that affects the kidneys. Sacs of fluid (called cysts) grow in the kidneys. If too many cysts grow or if they get too big, the kidneys become damaged. The cysts may also cause pain or may get infected. PKD is the most common inherited disease in the United States. Children of parents with PKD have a 50% chance of getting the disease. Most people with PKD can lead a normal life. In many people, the disease is mild and causes only minor problems. PKD is more severe in some patients and can cause kidney failure. About 60% of patients develop high blood pressure, which can be treated with blood pressure medicine. About 50% of patients with PKD have kidney failure by age 60. Dialysis (blood filtering) and kidney transplants are both effective treatments for kidney failure. PKD is generally worse in men, blacks and patients with sickle cell disease. There is also a childhood form of PKD which is usually more severe than the type that occurs in adults. Ultrasound has evolved from SONAR. It has been in clinical practice since the 1950s and now is one of the most important modalities and diagnostic imaging. The technology originally was developed to detect objects in water. SONAR was used to detect submarines. SONAR is responsible for the "pinging" one hears on a submarine. Shortly after the close of World War II, researchers began to explore medical diagnostic capabilities of ultrasound. Initially, the ultrasound equipment recorded blips on an oscilloscope. Subsequently, refinements allowed for today's gray scale imaging. Doppler technology was incorporated to detect moving structures such as blood within the body. 3D imaging with ultrasound is now possible and in use today. Sound is produced over a wide range of frequencies. The human ear only detects sound within a certain frequency range. Dog's ears can detect higher frequency sounds, hence the dog whistle humans cannot hear. In order to detect fine structures within the body, very high frequency sound waves must be used. The frequencies in today's ultrasound machine range from 2 Megahertz to 15 Megahertz. Megahertz denotes a million waves per second. The sound is concentrated into a thin beam and directed into the patient by a hand held transducer. That same transducer also acts as a receiver. The sound waves enter the body and bounce off internal structures. The returning echoes are recorded and processed by a computer and converted into an image.

The incidence of the dominant condition in patients of all ages has been estimated from autopsy studies at 1/220 to 1/1100. As anticipated with autosomal inheritance, the male:female ratio of both disorders is approximately 1:1. Patients with autosomal recessive polycystic kidney disease usually follow one of two different clinical courses, suggesting variable expression among affected individuals. The disease most often presents in the neonatal period, frequently after a pregnancy remarkable for oligohydramnios. The newborn may have physical manifestations of the Potter syndrome and massively enlarged, poorly functioning kidneys. Neonatal death due to pulmonary hypoplasia occurs in 84% of children with recessive polycystic kidney disease. However, a small number of affected patients follow a milder clinical course. In these children, the diagnosis may be made when enlarged kidneys are palpated at birth or in infancy. Hepatosplenomegaly may also be noted.

Even though very few people have heard of it, Polycystic Kidney Disease affects 600,000 people in the U.S. alone, and 12.5 million world wide. There are more people with PKD than cystic fibrosis, muscular dystrophy, hemophilia, Downs syndrome and sickle cell anemia combined. Many PKD patients find it useful to have access to the same information as their doctors because many doctors are unfortunately pretty unfamiliar with the disease.
