    Extractions: One of the most common signs is the enlargement of lymph nodes that the owner can actually feel and is one of reasons for the first seeking of treatment. Other nonspecific signs include: lethargy, weakness, loss of appetite, weight loss, diarrhea, shortness of breath, difficulty swallowing, increased thirst or increased urination. Other symptoms may be evident depending on what organ or organs the lymphoma has spread to and the degree of hypercalcemia. The liver, spleen, kidneys, lungs, intestinal tract, bone marrow, central nervous system and skin can all be affected. DIAGNOSIS: The mainstay treatment of lymphoma is combination chemotherapy in which a variety of drugs are given according to a specific plan or protocol. Many chemotherapy protocols exist and they are ever-changing. Commonly, prednisone, a steroid drug will be given alone or in conjunction with other chemotherapy drugs to decrease the inflammation that occurs in the dog's body due to the action of the disease or the effect of the chemotherapy. A word of caution is offered here and that is if you decide not to proceed with chemotherapy and to use prednisone alone and later decide to begin chemotherapy, the chemotherapy will be less effective because of the initial prednisone use. PROGNOSIS: Treatment is aimed at remission of disease. Without treatment, dogs on average will live 30 days; if placed on prednisone alone, several weeks may be added to this. Chemotherapy can provide a remission of up to 2 years, but beyond that, survival is not expected because lymphoma has a high propensity for recurrence and when this happens, it is more resistant to chemotherapy drugs. Thyroid nodules are more common in women and increase in frequency with age. Fewer than 10% of solitary nodules are malignant. The physician who encounters a patient with a thyroid nodule must be able to determine its clinical significance, especially with regard to possible malignancy, compression of structures of the neck, or thyroid dysfunction. In this document, important elements of the history, physical examination, and laboratory evaluation are reviewed, and a suggested management strategy is presented. This outline is not intended to be all-inclusive, nor does it preclude additional evaluation, according to the specific clinical situation. Multinodularity, especially if the nodules all have the same consistency, is consistent with a benign multinodular goiter. A nodule or mass that is dominant in size or has a different consistency than other nodules within the gland should be evaluated by employing the same criteria as those outlined for single nodules. A midline nodule over the hyoid bone that moves up with protrusion of the tongue is likely to be a thyroglossal duct cyst.

    Extractions: (This is the story of Clondike, a Great Pyrenees bitch who was diagnosed with canine lymphoma in May of 1998 at the age of 10 1/2. Employing a complementary approach including chemotherapy, improved nutrition and various immune enhancing therapies, she has managed to attain three remissions, the third lasting 16 months (and counting). While there have been some bumps along the road, on September 28, 2000 she was 28 months from diagnosis and approaching her 13th birthday. This article is an updated version of one written for the Great Pyrenees Club of America's Bulletin where it appeared in the July/August 1999 edition under the title "Another Year with Clondike.") The Diagnosis. It began one evening in May, 1998 with what was supposed to be a routine appointment to allow our regular vet to re-check Clondike's anal sacs. Before the visit was over, the vet was in tearsnever a good sign! I almost didn't mention the "funny" soft lump I had felt in the folds of Clondike's neck while brushing her that week. I expected to be told that it was some normal structure that I had never noticed before, like the time I rushed Clondike in to have a lump on her stomach checked, only to discover that she has a completely normal belly button! The vet felt Clondike's neck and became really serious. She said anytime a dog has swollen lymph nodes you have to consider cancer as one possibility, and that she usually puts dogs on antibiotics for two weeks to see if the nodes go down before doing a biopsy. When I reminded her that Clondike was just completing a three-week course of antibiotics for the anal sac infection, she felt some more and said that, while the nodes didn't feel exactly like lymphoma patients she had diagnosed in the past, it was something we definitely needed to consider.

Psychosocial Hematology/Oncology BURKITT'S LYMPHOMA Burkitt's lymphoma is a B cell neoplasm characterized by small noncleaved cells that are uniform in appearance and that produce a diffuse pattern of tissue involvement. This neoplasm is one of the fastest growing malignancies in humans. The cells of Burkitt's lymphoma are characterized by a specific cytogenetic defect, a balanced, reciprocal translocation of genetic material from the long arm of chromosome 8 to the long arm of chromosome 14. Two variants of Burkitt's lymphoma are recognized: African and non- African; although very similar in histologic and cytologic features, they have very different epidemiologic patterns and clinical presentations. African Burkitt's lymphoma presents most often as a jaw or orbital tumor and occurs endemically in central Africa. It is interesting to note that this same central African region is also the endemic area for malaria and yellow fever. In contrast non- African Burkitt's lymphoma occurs outside this endemic region and presents primarily as an abdominal mass. In the African type, the Epstein- Barr virus has been strongly implicated, while for the non- african type the relationship is less clear. Burkitt's lymphoma is a monoclonal proliferation of B lymphocytes. The lymphocytes have receptors for the EBV and are its specific target. African children who develop Burkitt's lymphoma are thought to be unable to mount an appropriate immune response to primary EBV infection, possibly because of co- existent malaria which is immunosuppressive. Months to years then pass during which excessive B cell proliferation occurs.
