F ew people are aware that macular degeneration is an incurable eye disease and that it is the leading cause of blindness for those aged 55 and older in the United States, affecting more than 10 million Americans. Macular degeneration is caused by the deterioration of the central portion of the retina, the inside back layer of the eye that records the images we see and sends them via the optic nerve from the eye to the brain. The retina's central portion, known as the macula, is responsible for focusing central vision in the eye, and it controls our ability to read, drive a car, recognize faces or colors, and see objects in fine detail. As people age, their chances for developing eye diseases increase dramatically. Unfortunately, the specific factors that cause macular degeneration are not conclusively known and research into this little-understood disease is limited by insufficient funding.

A 10-year follow up of the incidence and progression of retinal drusen, retinal pigmentary abnormalities, and signs of late age-related maculopathy was reported on 2,764 people living in Beaver Dam, WI who participated in the Beaver Dam Eye Study. The study's findings indicated that large numbers of hard drusen predict the incidence of soft drusen and pigmentary abnormalities, and that the presence of the latter lesions significantly increases the risk for the development of geographic atrophy and exudative macular degeneration. (More Info) Recent strides in identifying the molecular composition of drusen have led to the conclusion that drusen are a by-product of chronic local inflammation in the aging eye. These findings suggest that those forms of AMD characterized by the accumulation of drusen could benefit from therapeutic approaches that are designed to attenuate the local inflammatory events that precede the development of the more advanced stages of AMD. (More Info) Studies of over 500 pairs of identical and fraternal twins by genetic epidemiologists in Britain and at the Georgia Prevention Institute in Augusta, Georgia have confirmed that there is a significant genetic component in AMD. The study showed that concordance in the development of AMD among twins was nearly 40% in identical twins and about 20% in fraternal twins. The study also showed that the most significant inherited feature is the presence of large numbers of drusen.

Extractions: What is macular degeneration? Macular degeneration is the physical disturbance of the center of the retina called the macula. The macula, which is about the size of the capital letter "O" in this sentence, is the part of the retina which is capable of our most acute and detailed vision. We use the macula for reading, driving, recognizing faces, watching television, and fine work. Macular degeneration is the leading cause of legal blindness in people over age 55. (Legal blindness means that a person can see 20/200 or less with eyeglasses.) Even with a loss of central vision, however, color vision and peripheral vision may remain clear. Vision loss usually occurs gradually and typically affects both eyes at different rates.
Charles Bonnet Syndrome

Sometimes called visual hallucinations the things people see can take all kinds of forms from simple patterns of straight lines to detailed pictures of people or buildings. These can be enjoyable or sometimes upsetting. A Swiss philosopher named Charles Bonnet first described this condition in the 1760 when he noticed his grandfather who was blinded by cataracts describing seeing birds and buildings which were not there. Although the condition was described very early it is still largely unknown by ordinary doctors and nurses. The disease is most common in people who are age sixty and over, andfor this reasonit is often called age related macular degeneration (ARMD). About 10% of macular degeneration cases are the "wet" (or "exudative") form, in which newly-formed, immature blood vessels grow from the choroid and leak into the spaces above and below the retinal pigment epithelium (RPE). This leakage (neovascularization) can damage the photoreceptor cells and cause permanent central vision loss. There are currently three laser treatments which have been shown to temporarily stop the leakage: laser photocoagulation, photodynamic therapy (PDT), and transpupillary thermotherapy (TTT).
