Extractions: January 24, 2003 Introduction Age-related macular degeneration (AMD) can cause a progressive loss of central vision amongst people affected by the disease. Without treatment, the more aggressive form, so-called wet AMD, usually results in a devastating loss of central vision. AMD is the leading cause of legal blindness in people over 50 in the western world, affecting about 500,000 patients worldwide each year. Visual impairment can dramatically reduce the patient's quality of life. People with impaired vision are less independent, e.g. if they cannot drive, they are dependent on others for travel and shopping. They can also have difficulty with performing many easy daily tasks, such as using kitchen utensils, walking around a strange area, and recognizing facial features from across a room. The impact of reduced quality of life is also demonstrated by the fact that visually impaired people need more help from their families, use more community social support services, and are more likely to be admitted to a nursing home than the general elderly population.

Age-Related Macular Degeneration (AMD) is an eye disease that is the leading cause of vision loss in our senior population. This disease impairs central vision. People who are affected by AMD have problems reading, driving and performing activities that require clear central vision. Macular degeneration causes damage to the macula of the eye. The macula is the central part of the retina at the back of the eye that allows us to see fine details clearly. Macular degeneration makes close work like threading a needle or reading a book difficult or impossible. When the macula does not function correctly, we experience blurriness or darkness in the center of our vision. Although macular degeneration reduces vision in the central part of the retina, it does not affect peripheral vision. For example, you could see a clock but not be able to tell what time it is. The eye-health organization Prevent Blindness America estimates that 13 million Americans have evidence of AMD. The disease breaks down the macula, the light-sensitive part of the retina responsible for the sharp, direct vision needed to read or drive. Macular degeneration is diagnosed as either dry (atrophic) or wet (exudative). The dry form is more common than the wet, with about 90% of AMD patients diagnosed with dry AMD. The wet form of the disease usually leads to more serious vision loss. Macular degeneration is more common in people over age 65, and whites and females tend to get the disease more than their counterparts. Most cases of macular degeneration are related to aging. It also can occur as a side effect of some drugs, and it appears to run in families.

AMD is a common eye disease of the macula, a tiny area in the retina that helps produce sharp, central vision required for "straight ahead" activities such as reading, sewing, and driving. A person with AMD loses this clear, central vision; in some cases, vision loss is rapid and dramatic. AMD is a leading cause of severe visual impairment and blindness in the United States. It is estimated that AMD already causes visual impairment in approximately 1.7 million of the 34 million Americans over age 65. This report responds to the Committee's request by outlining areas where NEI-supported scientists have focused their research efforts in AMD. Research in this area is a high priority of the Institute; in fiscal year 1997, the NEI is funding approximately $75 million of research in areas generally related to age-related macular degeneration. INTRODUCTION: ANATOMY OF THE RETINA The retina is a tissue in the back of the eye. It is an extremely complex structure which contains millions of cells. The macula, an area of approximately 1/4 inch in diameter, is the central part of the retina and is responsible for central or straight ahead vision. The term macular degeneration, therefore, implies a disease in which a degenerative or aging process affects this particular area. The macula contains a very small central area called the FOVEA .
