Extractions: We have found the first evidence of neuronal degeneration in narcolepsy. After hearing the punch line of the joke, the teenager falls to the floor, almost as if actually punched. She remains there, completely unable to move. She hears her parents reassure her friends that they need not worry about her because she will be all right in a few minutes. She is embarrassed and frustrated as the episode continues, and her friends begin to leave. They bid her goodbye, but she is unable to respond. Although she cannot talk or move, she is otherwise in a state of high alertness, feeling, hearing and remembering everything that is going on around her. The episode lasts for five minutes, longer than her typical cataplexieswhich often last only secondsbut shorter than her longest episode, which lasted 25 minutes. Then it ends, almost as abruptly as it began. She gets up from the floor, and her everyday life resumes. Cataplexy, the loss of skeletal muscle tone without loss of consciousness, is one of the defining symptoms of a puzzling neurological disorder called narcolepsy. The cataplectic attacks of narcolepsy are frequently prompted by laughter; other times, embarrassment, social interactions with strangers, sudden anger, athletic exertion or sexual intercourse may trigger an episode.

Narcolepsy can also manipulate the normal functioning of REM to the degree that upon waking, the REM physical changes are still operative. Therefore, the patient's body is still in a limp state. He/she may not even be able to move any of his/her muscles. The REM state can also be triggered by emotional stimulus: laughing, becoming angry, stress. Any of these things can cause a sufferer to collapse in a heap, as the muscles suddenly become weak and limp.

It is estimated to afflict more than 20,000 people in the United Kingdom many of whom are undiagnosed. No cure for Narcolepsy is yet known and no wholly adequate treatment is available. The cause is suspected to be a central nervous system defect but the exact mechanism is not clearly understood. There is no evidence of a psychological cause. Narcolepsy (or a predisposition to develop it) may be an inheritable characteristic passed down in families. The symptoms usually appear independently of one another. Typically, they are rather mild at the start and increase in severity at a gradual rate over a period of years. However, the development and the severity of symptoms are very variable in different individuals. The two primary symptoms of narcolepsy are excessive daytime sleepiness and cataplexy . In severe cases either can result in complete disability. The other symptoms appear less frequently than the primary symptoms and usually present no serious problems. They are sometimes experienced by people who are not narcoleptic. EXCESSIVE DAYTIME SLEEPINESS (EDS) is usually the first symptom to be noticed. It may take the form of sleepiness, tiredness, lack of energy, or irresistable bouts of sleep. Although this pathological sleepiness is evident every day, its intensity varies through the day and the person suffering from it may find only the more severe episodes troublesome. Over twenty four hours the total time sleeping is usually no more than what is normal. The abnormality lies in the continuous susceptibility to sleepiness or to falling asleep and the circumstances in which this occurs.
