High-grade B-cell NHL is an AIDS-indicator condition, as is primary CNS lymphoma (with or without documented HIV infection). While many HIV-infected NHL patients present with a history of generalized lymphadenopathy, AIDS-related NHL is frequently an extranodal disease. Often disease is advanced at the time of presentation, with involvement of the bone marrow, CNS, GI tract, or liver. As patients with advanced HIV disease live longer, their risk for HIV-related neoplasms may increase. Lymphoma should be suspected in patients with advanced HIV disease who develop new adenopathy. S: Non-CNS: Patient complains of node swellings accompanied by persistent fevers, night sweats or weight loss; gastrointestinal bleeding; persistent perirectal pain; bony pain or tenderness; jaundice or light-colored stools. CNS (25% of reported cases): Patient complains of confusion, lethargy, memory loss, hemiparesis, aphasia, seizures, headaches, cranial nerve palsies.

The lymphatic system is a part of the immune system that defends the body against disease. NHL occurs when the body's B-cells or T-cells (types of white blood cells) start growing and dividing abnormally causing tumors to develop. NHL is separated into two major categories: systemic lymphomas and primary central nervous system (CNS) lymphomas. Systemic lymphomas occur mostly in people with CD4 counts over 50. CNS lymphomas occur mostly in people with CD4 counts 50 and under and who have had other opportunistic infections. In people with HIV, NHL often develops outside the lymph nodes, including the liver, bone marrow, stomach, brain, mouth and anus. What are the Signs and Symptoms of NHL? Swollen lymph nodes are the most common sign of NHL. Other symptoms include fevers, weight loss, fatigue, rectal pain, abnormal pain or vomiting. People with CNS lymphoma can develop confusion, loss of memory or speech, changes in personality, seizures, headaches or partial paralysis. Some of these symptoms could be caused by other HIV-related conditions. However, if these symptoms last for more than a week, see a doctor immediately because NHL can grow very rapidly. Most recently an Internet security consultant with Sun Microsystems, Ellis was one of the creators of Usenet, which linked computers and allowed people to share information and reply to messages. Usenet began in 1979 when Ellis and another Duke graduate student, Tom Truscott, thought of hooking together computers to share information. At the beginning of 1980, the network consisted of two sites at Duke and one at the University of North Carolina. Usenet quickly became a popular means of trading and sharing information internationally before the World Wide Web came into existence. Allan Fisher, chief executive officer of Carnegie Technology Education, a subsidiary of Carnegie Mellon University which develops Web-based courses, said Usenet could be considered "the first big community application" of an interconnected system of computers.

They are characterized by the abnormal growth of lymphocytes, the infection-fighting cells in the lymph nodes, spleen, and thymus. The tonsils, stomach, small intestine, and skin may also be affected. Primary lymphomas of the skin such as mycosis fungoides and Sezary's disease are extremely rare. Burkitt's lymphoma, rare in most of the world, is the most common childhood cancer in Central Africa, and is one of the most aggressive of all human cancers. Lymphomas are usually classified as Hodgkin's disease or non-Hodgkin's lymphoma. An estimated 45,000 new cases of non-Hodgkin's lymphoma were diagnosed in the United States in 1994 (Boring et al., 1994). Incidence per 100,000 is 18.6 among white males, 12.0 among white females, 12.8 among black males, and 8.1 among black females. In the United States, five-year survival is 53 percent for white patients and 45 percent for black patients. The highest incidence rates internationally for non-Hodgkin's lymphoma were observed for the United States and Canada (Parkin et al., 1992), although comparing incidence, survival, and mortality statistics among countries may be difficult because of differing classification systems used for these diseases. Between 1973 and 1991, the 73 percent increase in the incidence of non-Hodgkin's lymphoma was one of the largest among the major cancer sites in the United States (Ries et al., 1994). Part of the increase in incidence of non-Hodgkin's lymphoma in recent years has been related to AIDS. Non-Hodgkin's lymphoma is about 60 times more common in AIDS patients than in the general U.S. population (Beral et al., 1991). Gail et al. (1991) estimated that between 8 and 27 percent of all non-Hodgkin's lymphoma cases that occurred in the United States in 1992 were a direct consequence of infection with human immunodeficiency virus (HIV). AIDS, however, cannot explain the increase in non-Hodgkin's lymphoma observed over a period that began years before the AIDS epidemic surfaced, and continues to be observed in the absence of HIV infection.

Extractions: Possible side effects include headaches, fevers and flushing. In using sargramostim to treat people with HIV infection, there is concern that sargramostim may help spread HIV further throughout the body. Sargramostim stimulates macrophages, a crucial part of the immune system, and macrophages may be infected with HIV. Dosage Sargramostim treatment for individuals who have had bone marrow transplants is for 14 days and then discontinued for 7 days. If after this 7 day period, the graft has still not taken, a second 14 day course of treatment may be given. Dose may be increased for a third course of treatment, but if no improvement is seen after the third course of sargramostim therapy, alternative treatment will be necessary. Managing side effects If malignancy progression is observed during treatment, the drug should be discontinued. If severe adverse reaction occurs, dosage of the drug should be reduced or temporarily discontinued. This information is specific to the use of this medication for HIV-related conditions. Her spouse, Michael, is a teacher in the same school, head of the same department, and teaches English as well as Drama and Performing Arts. As you can see, we have absolutely nothing in common! Our beautiful daughter, Mary, is 16 years old. She is an accomplished dancer and baseball player, loves acting and singing, and is now finishing grade 11 at her mom and dad's school, and loves it. Our boy David passed away on June 20th 1998 after a long and struggle with non-Hodgkin’s’ lymphoma at the age of 11. The lymphatic system includes a network of thin tubes that branch, like blood vessels, into the tissues throughout the body. Lymphatic vessels carry lymph, a colorless, watery fluid that contains infection-fighting cells called lymphocytes. Along this network of vessels are groups of small, bean-shaped organs called lymph nodes that filter the lymph as it passes through the nodes. Clusters of lymph nodes are found in the underarm, groin, neck, and abdomen. Other parts of the lymphatic system are the spleen, thymus, tonsils, and bone marrow. Extractions: A recent outbreak of Kaposi's sarcoma, Pneumocystis carinii pneumonia, and other opportunistic infections (KSOI) involving homosexual males and associated with an acquired cellular immunodeficiency syndrome has been described (1,2). While the pathogenesis of these disorders among homosexual males in San Francisco was being studied, 4 cases of diffuse, undifferentiated non-Hodgkins lymphoma (DUNHL) were diagnosed between March 1981 and January 1982. Because of the rarity of this malignancy and the potential relationship of these cases to the KSOI syndrome, they are reported here. Patient 1: A 28-year-old hospital clerk complained of back and shoulder pain starting in early March 1981. Within a few days he had swelling of the right eye and an unsteady gait, and he was hospitalized on March 21. "Shotty" peripheral lymphadenopathy was present. A biopsy of an orbital mass and an enlarged cervical lymph node disclosed DUNHL. A myelogram revealed a T4-T6 block by an extradural mass. Radiation and chemotherapy led to complete remission. In September 1981, another tumor in the spinal cord was treated with radiation. The ensuing remission was temporary, and the patient died with disseminated DUNHL on January 15, 1982. Patient 2: A 33-year-old nurse developed a tumor in his left lower jaw in October 1981. Penicillin was given for a suspected abscess, but the mass enlarged. A biopsy on November 24 disclosed DUNHL. Tumor cells contained surface IgM, kappa type, indicating a B-cell tumor. The tumor involved a left axillary lymph node, the retroperitoneum, the bone marrow, and the meninges. Generalized "reactive" lymphadenopathy and mild splenomegaly were present. Systemic and intrathecal chemotherapy led to temporary tumor regression; the patient relapsed and died in March 1982.
