Non-Hodgkin's lymphomas vary in their malignancy according to the activity of the abnormal cells. Primitive cells, or those that are poorly specialized, are most malignant. They tend to take over the lymph nodes rapidly. Symptoms - There is usually painless swelling of the lymph nodes in the neck or groin. The liver and spleen may enlarge. The lymphoid tissue in the abdomen may be affected, and in rare cases, cause pain and abdominal bleeding from the intestines. If other organs are involved, symptoms may range from headache to skin ulceration. Unless it is controlled, the disease progressively impairs the immune system, leading to death from infections. Treatment - If the lymphoma is confined to a single group of lymph nodes, radiation therapy is recommended. If the disease has progressed to a more extensive level, anti-cancer drugs may be required. Chemotherapy has become the mainstay for most Non-Hodgkins Lymphoma patients and is curative in many forms even in Stage III-IV of the disease. Extractions: The female breast is a rare location of extra-nodal Non-Hodgkins lymphoma. We present a case of 60 year old post-menopausal female who presented with painless lump in left breast. Fine needle aspiration cytology (FNAC) suggested malignancy. Investigations ruled out metastasis, hence modified radical mastectomy (MRM) was done. Histopathology of specimen revealed Non-Hodgkins lymphoma of breast. Investigations to rule out any other site of lymphoma proved it to be primary Non-Hodgkins lymphoma of breast with no metastasis. Patient post-operatively was given chemotherapy. Patient is clinically disease free in post-operative follow-up of one year. Primary Non-Hodgkins lymphoma of Breast is a rare disease. It is estimated that the number of published cases in Western literature is approximately 250 cases. Even better insight into the frequency of this disease is achieved by determining the ratio of primary malignant lymphomas of the breast to total number of the breast malignancies in a given institution or geographic region. This number does not differ very much from publication to publication and puts the proportion of lymphomas in the range of 0.04%-0.52% of all the malignant tumours of the breast.

Extractions: Cancer in New Hampshire, 1996 Non-Hodgkins Lymphoma Non-Hodgkin's lymphomas (NHL) comprise the majority of the malignancies of the lymphoid cells of which there are several types. During 1996, 164 new cases of NHL were diagnosed in New Hampshire. Within the US during 1990-1994, the overall incidence of NHL was 18.0/100,000 for males and 12.0/100,000 for females. For New Hampshire during the same time period, the incidence rate for males was 17.7/100,000 and 12.2/100,000 for females. In general, NHL is observed in all age groups, with incidence increasing with age. While NHLs are considered to be relatively rare, over the past two decades, their overall incidence has steadily increased, largely due to the association of this group of malignancies with HIV infection. Improvements in a variety of different treatment regimens have contributed to better survival, however, the overall five-year relative survival for NHLs is only about 50%. Adequate screening methods have not been identified for NHL and their etiology is not well understood. Increased risks are associated with acquired immune disorders and some viral agents. Exposures to herbicides and chemicals used in agriculture and forestry are also known to increase the risk of disease.

Extractions: The University Record, June 25, 1996 Therapy shows promise in fight against non-Hodgkins lymphoma An innovative treatment using radioactive antibodies that target and destroy cancer cells appears to shrink tumors in patients who have failed numerous conventional therapies for the most common form of non-Hodgkin's lymphoma. Comprehensive Cancer Center researchers presented findings from a Phase I/II study of radioimmunotherapy in patients with B-cell lymphoma at the American Society of Clinical Oncology annual meeting in May. The therapy, which involves injecting into the blood stream monoclonal antibodies armed with radioactive iodine to zero in on cancer cells and destroy them, caused tumors to greatly shrink or completely disappear in nearly three-fourths of the 47 patients treated, with minimal or no side effects in many of the treated patients. "Results are particularly promising in patients with recurrent low-grade lymphoma-a slow-growing but relentlessly progressive and incurable form of the disease that accounts for nearly 40 percent of newly diagnosed B-cell lymphoma cases," says Mark S. Kaminski, associate professor of internal medicine and principal investigator for the study funded by the National Institutes of Health. Two-thirds of the low-grade lymphoma patients had complete remissions (no evidence of cancer), and 19 of 21 responded to the treatment. The monoclonal antibody used in this study, known as anti-B1 and produced by Coulter Pharmaceutical of Palo Alto, Calif., is engineered to attach to a protein found only on the surface of B-cells, a type of white blood cell that produces disease-fighting antibodies. The targeted tumor cells receive a greater accumulation of lethal radiation while normal tissue is spared. Once bound to the cell, the antibodies also stimulate the immune system to kill any remaining cancer.
