DSPN causes few serious abnormalities and mostly results in numbness or tingling of the feet and slowed reflexes at the ankles. It generally occurs with more severe immunosuppression and is steadily progressive. Treatment with tricyclic antidepressants, such as Elavil, relieves symptoms but does not affect the underlying nerve damage. AIDP/CIDP.

Extractions: Peripheral neuropathy is a common neurological disorder resulting from damage to the peripheral nerves. It may be caused by diseases of the nerves or as the result of systemic illnesses. Many neuropathies have well-defined causes such as diabetes, uremia, AIDs, or nutritional deficiencies. In fact, diabetes is one of the most common causes of peripheral neuropathy. Other causes include mechanical pressure such as compression or entrapment, direct trauma, penetrating injuries, contusions, fracture or dislocated bones; pressure involving the superficial nerves (ulnar, radial, or peroneal) which can result from prolonged use of crutches or staying in one position for too long, or from a tumor; intraneural hemorrhage; exposure to cold or radiation or, rarely, certain medicines or toxic substances; and vascular or collagen disorders such as atherosclerosis, systemic lupus erythematosus, scleroderma, sarcoidosis, rheumatoid arthritis, and polyarteritis nodosa.

The peripheral nervous system includes nerves that run from the brain and spinal cord to the rest of the body. Many of these nerves are involved with sensation and feeling things such as pain, temperature and touch. Up to one-third of people with HIV may get some symptoms of peripheral neuropathy. Peripheral neuropathy is usually felt at first as tingling and numbness in the hands and feet. Symptoms can be described as burning, shooting pain, throbbing, aching, and "feels like frostbite" or "walking on a bed of coals." Peripheral neuropathy can sometimes be caused by HIV but is most commonly a side effect of drugs. Sometimes neuropathy can be caused by vitamin deficiencies or diabetes. Diagnosis of peripheral neuropathy is done by a physical exam. Tests of vitamin B12 levels, thyroid function and glucose levels are also used to check for vitamin deficiencies or diabetes. If peripheral neuropathy is caused by a drug, the symptoms usually get better once the drug is stopped, although it can take 6-8 weeks and the pain can actually get worse for a while.
