Extractions: Mixed Connective Tissue Disorder is an autoimmune process with symptoms of Scleroderma, Myositis, Systemic Lupus Erythematosus , Rheumatoid Arthritis and/or other autoimmune diseases. It is a description of a group of symptoms occurring together. This is different from a diagnosis that describes the underlying process. It is a disorder of the immune system, which normally functions to protect the body against invading infections and cancers, and toxins. In Mixed Connective Tissue Disorder, as in other autoimmune diseases parts of the complex immune system is over-active and produces increased amounts of abnormal antibodies that attack the patient's own organs. Sometimes MCTD is called Overlap Syndrome or Undifferentiated Mix Connective Tissue Disorder. Some writers discriminate between them claiming that each of these are different. Some argue MCTD is not a distinct entity at all, and is just one of the forms of Lupus or Scleroderma. MCTD can affect many parts of the body, including the joints, skin, kidneys, lungs, heart, endocrine, digestive, and nervous system, blood vessels which all are made up of connective tissue. There are reports of psychiatric disturbances similar to those found in Lupus. The signs and symptoms of MCTD differ from person to person, with the symptoms of either Lupus, or Scleroderma, or Myositis or other autoimmune disease being most prevalent. Since its manifestations can vary it is often misdiagnosed. The disease can range from mild to life threatening. It can be mild for many years, and then after about 20 years of having the disease a medical crisis may occur. The lungs and kidneys are particularly at risk. Lungs and kidneys symptoms are particularly serious involvement and should be monitored vigilantly by both the doctor and the patient.

Despite extensive research the etiology of JRA remains unclear. JRA is arbitrarily defined as beginning before the age of 16 years. Approximately 20% have acute systemic JRA which is characterized by febrile onset, variable joint manifestations, rash, generalized lymphadenopathy, splenomegaly, liver disease, and rarely by GI tract disease. Other children with JRA primarily have arthritis that is present at the onset of illness. In 30% only a single joint, usually the knee, is involved; in 50% multiple joints are involved. The disease is termed polyarticular if more than 4 joints are involved and pauciarticular if 4 or fewer joints are affected. The acute systemic onset of JRA is often referred to as Still's disease because of Still's classic description of the disease in 1897. The systemic symptoms of JRA may precede the onset of overt arthritis for a variable periods of time ranging from months to years. The most characteristic features of systemic JRA are a high-spiking fever and the rheumatoid rash. The temperature peaks once or twice daily, often in the late afternoon or evening, to a level of 39

Much evidence points to a major role for immunologic reactivity in perpetuating rheumatoid inflammation. Unidentified initiating factors probably stimulate a synovitis that triggers the local synthesis if IgG and rheumatoid factor (IgM anti- IgG) by plasma cells within the joint. Activation of the complement system within the joint space generates several materials that attract PMN's into the joint cavity, where they phagocytize the immune complex, thus leading to the formation of the rheumatoid arthritis cells. The neutrophils, following phagocytosis, are stimulated to discharge hydrolase from lysosomal granules. These enzymes together with fibrinogen may initiate the destructive changes characteristic of rheumatoid joint disease. JRA is arbitrarily defined as beginning before the age of 16 years. It is slightly more common in females than in males and is said to affect as many as 250,000 children in the United States. Although JRA may begin at any age, an increased incidence of onset generally occurs from 1- 3 years of age and later from 8- 12 years of age. Approximately 20% have acute systemic JRA which is characterized by febrile onset, variable joint manifestations, rash, generalized lymphadenopathy, splenomegaly, liver disease, and rarely by GI tract disease.

Rheumatoid arthritis (RA) is a chronic disease that causes pain, stiffness, swelling and loss of function in the joints and inflammation in other body organs. CAUSE While the cause of RA remains unknown, rapidly advancing research is discovering the bodily reactions that lead to arthritis and joint damage. Recent studies show that certain people inherit a tendency to develop RA. HEALTH IMPACT DIAGNOSIS RA can be difficult to diagnose early because it may begin gradually with subtle symptoms. Blood tests and X-rays may be normal initially. The disease varies among individuals with respect to symptoms, joints affected and the nature of other organs involved, such as the eyes, lungs or skin. Other types of arthritis may mimic RA. Skill and experience are essential to reach a precise diagnosis and to arrive at the most appropriate treatment. The classification criteria established by the American College of Rheumatology include prolonged morning stiffness in the joints, characteristic nodules under the skin, joint erosions apparent on X-ray tests, and blood tests of an antibody known as rheumatoid factor.

Most patients exhibit a chronic fluctuating course of disease that, if left untreated, results in progressive joint destruction, deformity, disability, and premature death (2). RA results in more than 9 million physician visits and more than 250,000 hospitalizations per year (3). It frequently affects patients in their most productive years, and thus, disability results in major economic loss. It has been estimated that working individuals with arthritis have a yearly earnings deficit of $17.6 billion (1986 dollars) compared with earnings of individuals without arthritis, and $6.5 billion of this shortfall is attributable to RA (3). RA affects 1% of the adult population (2). This low prevalence means that the average physician often develops little experience with its diagnosis or management. The following guidelines for the management of RA assume a correct diagnosis, which may be difficult in the earlier stages (4). If there is any uncertainty about the diagnosis, consultation with a rheumatologist should be considered.
