Valley fever, a regional disease caused by mold spores in the ground, can cause symptoms ranging from mild flu-like feelings to severe illness and even death in especially vulnerable victims. The largest spike came in November and December last year when 383 and 531 cases were reported, respectively. That compares with an average 139 cases per month for the first 10 months of the year. "The honest answer is, we don't know why," said Dr. John Galgiani, director of the Valley Fever Center for Excellence at the University of Arizona in Tucson. "The population is growing, but the percentage of increased cases is greater than the growth in residents." The center, the only one in the country that studies valley fever, is working on a vaccine and better treatment for the worst cases. Researchers believe 60,000 Arizona residents are infected with the soil fungus, Coccidioides immitis, each year. But only a third of those, or 20,000 people, become ill enough to miss work or school and seek medical attention.

During the acute illness it may be possible to culture the fungus from the sputum. Chest X-rays may show a pneumonia that looks similar to a bacterial pneumonia. As mentioned above, for most people with normal immunity, no anti-fungal treatment is usually required. If valley fever is suspected as the cause of the illness, blood tests may be done, which demonstrate antibodies to the fungus and confirm a diagnosis of valley fever. As a general rule, anti-fungal medicines are only used when it appears that the infection will overwhelm the immune defense of the patient. Traditionally the most potent anti-fungal has been a drug that is given by IV, called amphotericin. This drug has many side effects, especially involving the kidney. Several anti-fungals in pill form are now available. They are not as effective as antibiotics are in treating bacterial infections, but can be effective in controlling the fungal infection.

Graham Easton of BBC Science has this report on the outbreak, and the nature of the disease. Experts initially thought the outbreak in Kenya was due to anthrax, but now scientists from the World Health Organisation have found the Rift Valley Fever virus in samples from several human victims of the mysterious outbreak. Rift Valley fever is a viral disease spread by mosquitoes. It's primarily an illness of livestock such as sheep, goats and camels. During epidemics in livestock the virus causes spontaneous abortions and deaths in young animals. But the disease can have devastating effects on humans too. Symptoms include fever, vomiting and often severe bleeding After an incubation period of about 3-6 days, the early symptoms in humans are fever and headache which can progress to bleeding from the gums and nose, and sometimes a fatal coma. Rarely the infection results in damage to the eyes, inflammation of the brain, and, as in this latest outbreak, severe bleeding.

Yemen's Health Minister, Abdelwali Nasher, said teams of doctors had been rushed to the affected area - a remote part of Yemen near the Saudi border. About half of all livestock in the Wadi Moor area in Al-Hudaydah province are said to have died. Yemen has appealed to the World Health Organisation and the US Centre for Disease Control in Atlanta for help. Mr Nasher told the BBC that at least 17 people had died from the fever, but another Yemeni Health Ministry official put the death toll as high as 77. Insecticide Mr Nasher said Saudi Arabia would soon be sending planes across the border to spray insecticide to kill the mosquitoes which are thought to be spreading the disease. At least 16 people died from Rift Valley Fever in Saudi Arabia last week, in the first outbreak of the viral disease outside Africa.

Extractions: Sunday, November 10, 2002 Secession lost heavily in the city and maintaining a 4,000 vote majority in the San Fernando Valley. But secession's legacy is important to this city. Already, the issues raised by secessionists are becoming top priorities for the city. Boroughs are getting a revival. Businesses too are benefiting from the attention paid by cityhood advocates on the city's jobkilling regulations. As for the candidates who ran for Valley offices, many of them are heartened by the efforts so much so that they are running again, this time for the Los Angeles City Council. The winners of Valley offices are not retreating from the good fight, they have formed the "Collective" to press on for reforms. More importantly, the independence movement forced people all over to pay attention to the Valley's unique culture. Before, what the country knew about the Valley was filtered through Hollywood's lens, that of a bland suburbia peopled with vapid white girls and their obnoxious dialect. Now, the ethnic diversity, the slumminess, its relationship with Los Angeles and its pastoral roots are what the country is seeing. I prefer the nuanced gritty picture to the plastic fantastic vision pitched by developers. I've always felt secession was a radical plan but it was the only solution offered and not a bad one at that. I can't pass on any judgment on whether the voters' decision was a disappointing colossal mistake until I see the sort of city Los Angeles will fashion itself as come the next city council elections in April.
