I'm posting my story here to communicate how profound an impact such a diagnosis can have. Many get a diagnosis of Meniere's disease, perilymph fistula, or other vestibular disorder, have a few dizzy spells, and, eventually, get back on track. Many publications affirm that experience of vestibular disorders. For some of us, though, vestibular disorders prove catastrophic. They disrupt our lives and make it necessary for us to make extraordinary demands on social services, health care professionals, and loved ones. Often these significant others are confused and overwhelmed. They have no idea of vestibular disorders. Well-meaning people in power, such as Social Security officials, might suggest to us that our diagnosis is no big thing, and, that if we aren't working as we used to, we are probably malingerers, or hypochondriacs. Often we ourselves have no idea that our experience is not unique. Again and again, those receiving a new diagnosis ask, "I experience a lessened ability to concentrate." "I get terrible headaches." "I can't walk down tile hallways with sharp patterns." "Am I normal? Am I a malingerer? Am I a hypochondriac and am I imagining these symptoms? Am I the only one?" We need to educate ourselves and those who interact with us, including health professionals, that vestibular disorders can be catastrophically disruptive of life, and that that experience is not imagined by, nor the fault of, the sufferer. A lot, unfortunately. Up until the 1990's, in the US, the trade name Dramamine represented one drug, the antihistamine and anti-motion sickness drug dimenhydrinate. Then a second medication under the trade name Dramamine was placed on the market. It was called Dramamine II. It did not contain dimenhydrinate; instead its active ingredient was meclizine hydrochloride, another antihistamine/anti-motion sickness drug. Recently Pharmacia/UpJohn, makers of Dramamine, changed the names of its two Dramamines again. The dimenhydrinate Dramamine is now sold as "Dramamine-Original Formula" and the meclizine type of Dramamine as "Dramamine-Less Drowsy Fromula." This series of drug and name changes illustrates the point that anyone taking a drug needs to know both the trade and generic (scientific name) names of the medication to be sure they are taking the proper drug. Their prevalence increases with age: approximately 1/4 to 1/3 of the elderly complain of some form of dizziness. Dizziness can be related to a vast number of causal factors, but in most patients one or several of a handful of etiologies can easily be identified. The causes of dizziness are rarely secondary to a serious ailment. New facts: Traditionally, vestibular disorders have been evaluated and managed by various medical specialists, including otologists. Over the last three decades, otoneurology (or neuro-otology) has started to emerge as a new subspecialty within otolaryngology and, most recently, within neurology. Currently, there are only about a dozen neurologists in this country who specialize in the evaluation of vestibular disorders. The shift in specialty reflects the realization that many cases of dizziness don't require ear surgery and are actually neurological. Another factor prompting specialization in vestibular disorders is their presentation, which consists of a vague, evasive, and rather complicated set of symptoms. Dizziness can mean various sensations to different patients. It can present as vertigo (spinning), lightheadedness, near-fainting, disequilibrium, or a combination of these. Most commonly, dizziness is used by patients to mean any and all of the above. It is a subjective feeling, difficult to describe, and it can be caused by many classes of diseases. Almost half of patients with vestibular problems indeed have more than one type of dizziness.

Extractions: Exercise Therapy Quick Review for Patients It is important to recognize that Benign Positional Paroxysmal Vertigo (BPPV) is responsible for at least fifty percent of all cases of vertigo and exercise therapy may be curative in up to ninety percent of patients. The primary therapeutic option is one form or other of exercise therapy. The severity of the individual attacks and accompanying nausea may be lessened by medical therapy, however, this does not prevent future attacks. Exercise therapy is indicated for all patients with BPPV. There are two general approaches to therapy: (a) a single treatment session in an outpatient office setting, (The Epley Maneuver and its variations) and (b) a series of exercises performed by the patient at home (The Brandt-Daroff exercises). Each will be briefly described. a). Office single treatment approach. Among the single treatment approaches are the Canal Repositioning Maneuver (CRP) and its modifications (Figure 9, Panel 1-6, Modified for this Web page exclusively, after Epley).

When the symptom complex is of spinning or rotation, the cause is almost always the inner ear or peripheral vestibular system. Although it is true that some patients experience a definite sense of environmental spin or self-rotation, the majority do not present solely with true spinning vertigo. The most common complaint is one of dizziness , a term that represents a variety of symptoms (Table 1): TABLE 1. Symptoms encompassed by the term, dizziness Vertigo Bouncing Unsteadiness Falling Imbalance Swimming Spinning Staggering Floating Weaving Fainting Moving Lightheadedness Passing out Swaying Tilting Twisting Listing Blurry vision Rocking Disorientation Oscillating Poor equilibrium Rolling The examiner should elicit an exact description of what the patient is experiencing. Is it a spinning sensation that could be characterized as vertigo, pointing to the peripheral vestibular apparatus? Is it a sensation of falling without rotation? Is it a sensation of unsteadiness or imbalance? Is there a particular direction in which the patient tends to fall? When the patient's complaint is actually of incoordination or clumsiness, the cause may be cerebellar dysfunction or peripheral neuropathy. When the symptom complex is of "lightheadedness" or "swimmy-headedness," the examiner should think of presyncope or syncope and consider systemic factors such as postural hypotension, vasodepressor syncope, or cardiac arrhythmia.
