Dyspareunia: Painful Sex for Women What is dyspareunia? What are some of the causes of dyspareunia?
Any of the genital parts can cause pain during sex. Some conditions affect the skin around the vagina. Some women have a viral infection or vaginal yeast infections, but sometimes the cause is unknown. The pain from these conditions is usually felt when a tampon or penis is inserted into the vagina. Sometimes, even sitting or wearing pants can cause discomfort. We know that the mind and the body work together. This is seen with sexual problems. Often the problem that first caused the pain may go away, but you may have learned to expect the pain. This can lead to further problems because you may be tense or you may be unable to become aroused. The problem can then become a cycle and you are caught in the middle. How can my doctor tell what is causing my pain? Your doctor may ask you to describe your pain, when it began and any associated problems, and may ask you to describe what you have tried in the past. For example, is it painful every time you try to have sex? Are there other problems associated with sex? These are some of the questions that your doctor will need to discuss with you. Your doctor may want to examine your genital area. What will the exam be like?

Painful Intercourse (Dyspareunia)
Painful intercourse can be caused by many different physical causes, a such as endometriosis; inflammation or infection, urinary tract infections vaginal yeast infections, or sexually transmitted diseases; vaginal dryness due to lack of lubrication, inadequate foreplay before intercourse, thinning of the vaginal lining in postmenopausal women; an allergic reaction to clothing, spermicides or douches.
Persistent pain requires an evaluation by a gynecologist. Once the physical cause of pain is ruled out, psychological factors can be evaluated. For dyspareunia that has no apparent physical cause and has existed for months or years - psychological counseling is suggested to address stress or anxiety regarding sexual intercourse. Women frequently have tension, fear or anxiety accompanying intercourse. Some have psychological trauma, often stemming from a past history of sexual abuse or trauma like rape. Vaginismus This is a disorder in which the muscles surrounding the vagina opening, go into involuntary spasms making sexual intercourse difficult or impossible.

This can occur in both men and women. Considerations Return to top It is believed that dyspareunia is caused by organic (physiological) factors at least 75% to 80% of the time. Pain that occurs initially at entry but decreases over time is commonly caused by inadequate lubrication. This is frequently due to a lack of sexual arousal and effective stimulation, and sometimes due to medication that decreases vaginal lubrication (such as antihistamines). Psychological factors are infrequently involved and will most commonly be associated with previous sexual trauma ( rape or abuse), feelings of guilt, or negative attitudes toward sex.

Dyspareunia is pain on intercourse, and it may be superficial or deep. Superficial dyspareunia is felt at the entrance to the vagina and may be due to, for example, infection, or it may be the result of spasm in the vaginal muscles -called vaginismus - caused by tension, fear and lack of sexual arousal, producing a vicious circle. Deep dyspareunia is pain felt in the pelvis during intercourse and may be due to pelvic inflammatory disease, endometriosis, cervicitis, uterine infection or ovarian problems. Superficial dyspareunia due to vaginismus may respond to psychosexual therapy: other causes should have treatment as appropriate. return to female genital tract disorders

Background: Proponents of therapeutic ultrasound suggest it can decrease pain by resolution of inflammation processes and reducing the pressure on pain sensitive structures by haematoma and oedema. Objectives: The objective of this review was to assess the effects of therapeutic ultrasound for treating acute perineal pain, persistent perineal pain and/or dyspareunia, following childbirth. Search strategy: The Cochrane Pregnancy and Childbirth Group trials register (to July 2001), EMBASE, CINAHL (to May 1996), Physiotherapy Index (1985 to May 1996) and World Congress of Physical Therapy 1994 Conference Proceedings. Selection criteria: Randomised and quasi-randomised trials comparing active therapeutic ultrasound with no treatment, placebo ultrasound, or any other 'standard' or active treatment for women with acute or persistent perineal pain and/or dyspareunia following childbirth.
